I, Paul G. Reis, do hereby certify that Notice of Public Hearing before the Carmel Board Of
Zoning Appeals considering Docket Numbers 08020019V and 08020027V was (1) registered
and mailed at least twenty-five (25) days prior to the date of the public hearing to the adjacent
property owners listed in the exhibit attached hereto and made a part hereof~ and (2) published in

Attached to this letter is a notice of a fOl1hcoming meeting of the Carmel Board of Zoning
Appeals to be held on Monday, March 24, 2008, at 6:00 p.m. at Camlel City Hall Council Chambers.
Our client, North Meridian Medical Partners One, LLC is requesting approval of Its application for
vanances to allow for:
(i) an increase in the height of a monument ground sign to twelve (12) feet; and

Notice is hereby given that the Carmel Board of Zoning Appeals on the 24th day of March, 2008
at 6:00 p.m, in the Council Chambers, One Civic Square, Cannel, Indiana 46032 will hold a
Public Hearing upon a Development Standards Variance application for North Meridian Medical
Partners One, LLC ("Applicant"). Applicant is requesting certain Development Standards
Variances relating to a new ground sign for existing medical office buildings at the North
Meridian Medical Pavilion located on an approximately 6.046 acre parcel located at 12188 North
Meridian Street, Cannel, Indiana.
These requests are as follows: Docket No. 080200 19V - to increase the height of a monument
ground sign to twelve (12) teet; and 08020027V - allow for an additional sign for the multi-
tenant multi-level buildings. The real estate affected by said application is legally described in
Exhibit A attached hereto.
All interested persons desiring to present their views on the above application, either in writing
or verbally, will be given an opportunity to be heard at the above-mentioned time and place.
Interested persons may also me written comments prior to the meeting. The hearing may be
continued from time to time as may be found necessary.
The petition and plans may be viewed at the City of Cannel Department of Community Services,
One Civic Square, Carmel, Indiana, 46032.
All interested persons desiring to present their vicws on the above application, cithcr in writing
or verbally, will be given an opportunity to be heard at the above mentioned time and place, or
may file written comments with the Department of Community Services prior to the hearing.
