RE: Methodist Medical Plaza Parking Expansion-Project Review #1 Dear Mr, Jett: We have reviewed the construction plans submitted for this project for review for the October 18, 2006 Technical Advisory Committee. We offer the following comments: GENERAL INFORMATION I. The project site is located within current City of Carmel Corporate Limits. 2. Jurisdictions: . Streets and Right of Way, Pennsylvania Parkway - City of Carmel . Water - City of Carmel Water Utility service area . Sanitary Sewers - Clay Township Regional Waste District service area . Storm SewersfDrainage - City of Carmel . Regulated! Legal Drains - Hamilton County Surveyor's Office. 3. Board ofPubHc Works and Safety Requirements: Cl It does not appear that this project will require Board of Public Works and Safety approvals. l!l Please be adv~ed that any open cuts 01 existing perimeter-dedicated streets under City of Carmel jurlsdiction will require separate Board of Public Works and Safety approval . Any permanent improvement (including irrigation systems) to be installed within dedicated right of way or dedicated easements. This approval would require a Consent to Encroach Agreement between the Owner and the City of Carmel. . Right of Way Dedication (if applicable) I am enclosing a schedule for Board of Public Works and Safety meeting dates and agenda deadlines for your use. Please use the Engineering Department deadlines for submissions to the Board. Any submission to the Board requires prior approval by the Carmel Clay Plan Commission and/or the Board oj Zoning Appeals (if applicable) and completion of review by the Technical Advisory Committee. All written requests to be placed on the Board's agenda must include the appropriate Docket Number and the date (or dates) o[ approval by the Plan Commlsslon and/or the Board oj Zoning Appeals (if applicable). 4. T.A.C. ReviewfDrawings submitted for approval: We request that all comments and comment letters generated by this office be answered in writing and be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for construction until all Engineering Department and Carmel Utilities issues have been resolved. The design engineer must certifY all drawings submitted for fmal approval. We will require a minimum of four-sets of drawings for approval after all issues have been resolved. The drawings will be stamped as approved and will be signed by the City Engineer. One set will be distributed to the Owner/Developer to be maintained on the construction site. However, the additional sets must be submitted with the required four sets. 5. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated right of way or dedicated easements will require a Consent to Encroach Agreement with the City of Carmel's Board of Public Works andSafetv. 6. Upon initial review, it appears the following bonding requirements may apply to this project: Performance GuaranteeslEngineer's Estimates Erosion Control It is our assumption that the storm sewers/storm drainage system will be privately owned and maintained. Please contact Amanda Foley, Engineering Department Storm Water Administrator, for erosion control requirements. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for 100% of the cost oflabor and materials to construct the individual improvements, to be provided by the design engineer. Please provide detailed Engineers Estimates for each improvement including unit costs, quantities,. materials and types of materials, etc. Upon completion and release of individual Performance Guarantees, a three-year Maintenance Guarantee may be required. The Maintenance Guarantee amount is based upon 10% of the Performance amount. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of Credit. Please reference the enclosures for more detailed explanation of our procedures. Right of Way Permit and Bonding Any work in the dedicated right of way of Pennsylvania Parkway will require a Right of Way Permit. If the work is included in the scope of work ofa posted Performance Guarantee, no additional bonding will be required. If it is not included, a License & Permit type Right of Way bond will be required in the amount of $2,000.00 per instance of work in the right of way. Any open cut in the pavement of Pennsylvania Parkway will require separate Board of Public Works and Safety approval. 8. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage calculations submitted to this office for review. We will share Crossroad's comments as they are received. When construction plans are submitted for review for this development, please provide 2-sets of plans and calculations to allow us to send the extra set to Crossroad to save some time in the review process. Please contact Crossroad Engineers to obtain a drainage review checklist. The Depamnent will expect .the design submitted for the development of construction drawings to adhere to this checklist. CONSTRUCTION PLAN COMMENTS 9. A seven-page Plan Certification Form has been enclosed. This form was created with the desire and intent to streamline the plan review process. We request that you please complete this form and return it to the Department of Engineering as soon as possible. Additional comments will be provided by the Department of Engineering after the Certification Form is returned. Please treat each item on the form as a requirement of the Department of Engineering. Providing a "no" or "NI A" response to any ofthe items does not relieve the Developer of the responsibility of meeting the requirements of the Department of Engineering. The Department of Engineering will make the fmal determination as to the acceptability of any "no" or "N/A" responses. 10. The addition of an overall site plan that would include the parking expansion area would be very helpful. It is difficultto determine where this parking expansion area is located in relation to Pennsylvania Parkway and the Methodist Medical Plaza. 11. The Engineering Department does not review or approve landscape plans or lighting plans. Please remove Sheets C6.1, Landscape Plans and C7.l, Site Lighting Plan from sets presented to this ., -:' u u Department for approval. Please remove the reference to these sheets from the Index on Sheet CO.l 12. The Department requires that the construction drawings be developed in accordance with the City of Carmel digital submission standards and that all required submittals for primary plat, secondary plat, and construction drawings be made. The digital files must be submitted to the Department of Engineering prior to.approval of the construction plans. 13. The City has adopted a new curbing policy. Please revise any notes, specifications, and details to accommodate this policy. The language of this policy must be added to a specification sheet on the plans. 14. This project is subject to the Carmel Stormwater Management Ordinance, which was adopted February 6, 2006. 15. Erosion and sediment control measures and post-construction best management practices must be bonded. 16. .A storm water management permit is required prior to commencing any earth disturbing activity. This permit is not issued until the construction plans are approved. Please add a note to this effect to the plans and revise General Grading Note 2. 17. Please review erosion and sediment control measures and post-construction best management practice requirements with Amanda Foley. 18. City standard curb is required by ordinance. Please replace the curb detail on C5.l with the appropriate city standard details. 19. Please confum with the Urban Forester requirements for structural soil in the parking lot islands. 20. The City defers to the County for approval of the legal drain easement encroachinent and for approval of the installation of structure #3 on the legal drain. 21. Please indicate the outfall of the existing pipe to the east from existing structure #4. 22. Where is the storm water detention for this expansion provided? If the area was master plarmed, is the proposed "c" value the same value anticipated for this area at the time of master planning or is it less (or higher)? Does the acreage of the expansion match what was anticipated at the time of the master plarming? 23. Please verifY the capacity of the existing piping to convey the new flows from the expansion. 24. A portion of the parking lot drains to storm structure No.6, directly into the legal drain. Is this runoff detained in a downstream facility? If it is not, provisions shall be made to detain the increase in runoff. These comments represent the Department of Engineering's initial review of the development plan for this project. We request that all responses to our comments be provided in writing. Failure to provide written responses may result in the delay of the review process. It is critical that our office be made aware of all modifications made on the plans being resubmitted, particularly if any such changes are considered "new" or fall outside of our previous reviews. Please provide revised plans including all revisions. Please notify us of any changes and specifically state any changes, including changes resulting from Plan Commission, BZA or other committee meetings. All bonds and performance guarantees must be posted prior to Engineering Department approval of construction plans. Also, Board of Public Works and Safety approvals and any other governing agency approvals (if required) must be obtained prior to Engineering Department approval. The Department reserves the right to provide additional comments based on subsequent reviews. These comments have been satisfactorily addressed with the following exceptions: BONDING REQUIREMENTS I. Sediment control measures for storm sewer inlet protection The note on C4.1 should be corrected to reflect changes to planned inlet protection. 810. Location, dimensions, specifications, and construction details of each sturmwater quality measure The note on C4. I should reflect the statement in your response letter. B14. Monitoring and maintenance guidelines for each proposed stormwatcr quality measure Specific maintenance requirements for the Catch-All inlet protection are needed. C2. Sequence describing stonnwater quality measure implementation The City of Carmel requires that an stormwater leaving a site (post construction) be treated by a minimum of two different water quality BMPs and that the maj or pollutants [rom the project be treated. Although the existing pond was not originally designed as a water quality BMP and has not been demonstrated to meet the City o[ Carmel Stormwater Technical Standards Manual COT post-construction BMPs, the City is accepting it as one of the two required BMPs. This site is being converted from primarily woodland to impervious parking, which can have a significant impact on water quality. Taking these factors into account, a second BMP will still need to be planned, and the major pollutants listed in Cl will need to be addressed. C3. Legal Description of the Project Site I did not see this in the plans. B2. Sequence describing storm water quality measure implementation relative to land disturbing activities A preconstruction meeting should be scheduled with the City of Carmel Engineering Department. There is not any building construction. Make the sequence specific to this project. B4. Sediment control measures for sheet llow areas Where are the refetenced existing pond and wetland loeatcd? B6. Sediment control measures for storm sewer inlet protection The silt fence inlet pro'tectiondetail is the only inlet protect shown. This practice is not acceptable in paved areas. Specify another inlet protection measure for these areas. Straw bales are not a sufficient inlet protection measure either. Inlet protection is needed at the storm inlet in the existing parking lot nearest the conshuction entrance. Alllhe water appears to be draining to two existing storm inlets inside the project limits. Address how these inlets, and the Ihird inlet located inside the construction limits, will be handled during grading and construction. BI0. Location, dimensions, specifications, and construction details of each stormwater quality measure If the ponds are used for sediment control purposes, then they will need to be dredged following construction. The existing adjoining parking lot and all perimeter City slreets shall be monitored daily to prevent sediment from leaving the construction site. Only include those practices specific to this project. C2. Sequence describing.stnrmwater quality measure implementation The City of Carmel requires that all ~torniwater leaving a site (post construction) be treated by a minimum of two different water quality BMPs. A second EMP (in addition to the pond) is required for thi~ project. Due to the amount of trees that will be lost, we reconm1end you consider a permeable pavement'System as your second BMP. C3.
