The following notes were made on the approval for the CVS landscape plan:
Landscape changes for CVS:
Due to insect problems, (Japanese Beetle) associated with the Horsechestnut and the
proximity of those trees with the public, Brenwick has requested the following changes for all
street trees on Pettig'ru Street. '
Aesculus x cornea 'Briotii'/Rubyred Horsechestnut 2 % - 3" are required to be: Cladrastis
kentukea/Yellowwood 2 Y2 - 3"

For CVS this will involve 3 trees.

Dear Mr. Forgey:
The City has received your comment letter and drawings dated February 27,2007 based upon the City
review of February 20, 2007.
The comments from the City review have been satisfactorily addressed with the following exceptions:
PROJECT COMMENT8-CONSTRUCTIONPLANS
35. It is apparent that a small portion of the sanitary sewer lateral is to be installed within the Pettigru
right-of-way. The trenching and bedding details for plastic pipe (which appear to be duplicate) shall
be revised to conform to City standards for the portion of the installation in the right-of-way. Tbank
you. Please add a Dote requiring tbe #53 cap per the specifications where within the City's rigbt-
of-way
Based on additional review, the City provides additional comments:
1. Sheet C9.1, right side column - The backfill and minimum cover requirements are incorrectly labeled
"Paving Policy:'
These comments represent the Department of Engineering's third review of the construction plans for this
project. We request that all responses to our comments be provided in writing. Failure to provide written
responses may result in delay of the review process.
It is critical that this office be made aware of all modification made on the plans being re-submitted,
particularly if any such changes are considered "new" or fall outside of our previous reviews. Please
provide revised plans indicating all revisions. Please notify us of any changes and specifically state any
changes, including cbanges resulting from Plan Commission, Special Studies or other committee meetings.
AIL bonds and performance guarantees (ifrequired) must be posted prior to Engineering Department
approval. Also, Board of Public Works and Safety approval and any other governing agency approvals (if
required) must be obtained prior to Engineering Department approval.
Comments from the City's consultant related to the drainage review will be forwarded to your office when
the review is complete. Please confirm that plans and drainage calculations were submitted to Crossroad
Engineers for the T AC meeting.
The Department of Engineering reserves the right to provide additional comments based on subsequent
reviews.

The City has received your comment letter and drawings dated February 7, 2007 based upon the City
review of January 18, 2007.
The comments from the City review have been satisfactorily addressed with the following exceptions:
PROJECT COMMENTS-CONSTRUCTION PLANS
14. The City has adopted a new curbing policy. Please revise any notes, specifications, and details to
accommodate this policy. The language of this policy must be added to a specification sheet on the
plans. All curbing on-site and in the right-of-way must conform to this policy. The polky bas not
been added.
15. The City has adopted a new paving policy. Please revise any notes, specifications, and details to
accommodate this policy. The language of this policy must be added to a specification sheet on the
plans. All paving in the right-of-way must conform to this policy. Tbe policy bas 110t been added.
16. Please remove the titles for the landscape plan and photometric plan from the Index on the Title Sheet.
Also please do not include the landscape plan and photometric plan in plan sets submitted to the
Department of Engineering for approval. The titles have not been removed from the Index on the
Title Sheet.
19. Please revise CURB JOINT DETAIL such that the maximum transverse expansion joint spacing is 50
feet, the maximum transverse contmctionjoint spacing on tangents is to feet, and the maximum
transverse contraction j oint spacing on radii is 5 feet. It needs to be indicated that each of tbese
spacings is a maximum spacing.
24. The sidewalk detail shall meet City standards for portions in the right-of-way. Thank you. However,
transverse joints shall be spaced DO greater than 1.25 times the width of the sidewalk in tbe rigbt-
of-way.
29. Is pedestrian access provided along Pettigru? If on-site, and easement shall be provided. Please have
Brenwlck confirm to City via e-mail.

Based on additional review, the City provides additional comments:
1. Add disclaimer that approval by the Department of Engineering of the site plan cannot be construed as
approval of the lighting plans by the Department ofCoDllllunity Services. Also, approval of the site
plan cannot be construed as the City taking maintenance and power cost responsibility for the lights
indicated in the right-of-way.
These comments represent the Department of Engineering's second review of the construction plans for

We have reviewed the drawings submitted for this project. We offer the following comments:
GENERAL INFORMATION
1. The project site is currently located within current City of Cannel Corporate Limits. The following
jurisdictions will apply.
2. Jurisdictions:
. Street and Right of Way - City of Carmel (Pettigru Drive, Moultrie Street, 13151 Street)
" Water Mains - Carmel Utilities service area.
. Sanitary Sewers - Clay Township Regional Waste District service area
" Storm Sewer - City of Carmel\Hamilton County Surveyor's Office
. Legal Drains - Hamilton County Surveyor's Office
3. Board of Public Works and Safety Approval Requirements:
III Commercial Curb Cut. Board of Public Works and Safety.approval will be required for new curb
cuts and modifications to curb cuts. [t appears that there are requirements for 4-curb cuts, 2-on
Pettigru Drive and 2-on Moultrie Street. Please provide 8Y:l x 11 exhibits with your request for
commercial curb cut approval. The exhibits should include all pertinent dimensional data
including widths, radi~ aceIJdeceVpassing blister (if applicable), existing and opposing streets or
drives, etc.
. Temporary Construction Entrance. Same requirements as for the Commercial Curb Cut. If the
temporary entrance is located at the site of a permanent future curb cut, separate Board approval is
not required.
. Water Availability approval from the Board is based upon the proposed commercial usage of the
building. Reference Item # 11 below for a more detailed explanation.
. Dedication of Right-of-Way (if applicable and ifnot platted).
. Any permanent improvement to be installed within dedicated right of way.
. Consent to Encroach Agreements for any permanent improvement to be installed within dedicated
easements and/or dedicated right of way.
A schedule for Board of Public Works and Safety meeting dates and agenda deadlines is being
provided with these comments. Please use the Engineering Department deadlines for submissions to
the Board if applicable.
Any submwJon to the Board requires prior final approval by the Carmel Clay Plan
Commission and/or the Board 01 Zoning Appealr; and completion of review by the Technical
A.dvlsory Commiltee. All written requests to, be place on the Board's agenda ItUIst include the
appropriate Docket Numbers and the date (or dates) of IlfJproval by the Plan Commission
and/or BZA.

4. T AC ReviewlDrawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing
and be accompanied by a drawing reflecting requested revisions. Final drawings will not be
approved for construction until all issues have been resolved. All Carmel Utility issues must also
be resolved. The design engineer must certifY all drawings submitted for final approval. Due to
the dual jurisdiction regarding storm drainage, this office will require eight sets of approved
drawings after all issues have been resolved. The drawings will be stamped, approved and signed
by the City Engineer and by Carmel Utilities. After Carmel Approval, the eight sets of drawings
wilI be submitted to the Hamilton County Surveyor's Office for approval. After Surveyor
approval, the Developer/Owner, the Engineer and Contractor will each receive one-set. The
Surveyor's Office wilI retain two sets and three-sets wilI be returned to the Engineering
Department.
5. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
easements and/or right of way will require a Consent to Encroach Agreement between the
OwnerlDeveloper and the City of Carmel. This Agreement requires Board of Public Works and Safety
approval.
6. Carmel Utilities will provide separate reviews of this project. Please assure that copies of all drawings
for review are sent to:

BONDING REQUIREMENTS
9. Upon initial review, it appears the following bonding requirements may apply to this project:
Performance Guarantees
Erosion Control. Please contact Amanda Foley, Engineering Department Stormwater
Administrator, for details regarding Engineer's Estimates/Performance Guarantee requirements for
erosion control. Other than erosion control, there do not appear to be any other Performance
Guarantee requirements by the City of Carmel.
Street CutJRigbt of Way Permit and Bonding Requirements
Any work in dedicated right of way ofPettigru Drive, Moultrie Street or 13 l51 Street will require a
Right of Way Permit and a License and Permit Bond. The bond amount will be determined by the
number of instances of work in the right of way (curb cut, stonn sewer work, etc.) @ $2,000.00
per instance. Open pavement cuts in dedicated streets require approvlI by our Board of Public
Works and Safety.
10. We nave engaged Crossroad Engineers, PC to review all drainage plans and drainage calculations
submitted to this office for review. We will share Crossroad's comments as they are received. Please
provide drawings and calculations directly to Crossroad.
AVAILABILITY AND CONNECTION FEES
] I. Water Availability (acreage) and Connection Fees must be paid after all other Engineering
Department requirements have been satisfied and prior to approval and start of construction activities.
Availability Fees are based upon total platted acreage or legal description acreage for the development
at the current rate of$I,010.00 per acre for Water Availability. This area has not been platted and we
request a copy of the legal description oftbe property. Based upon the site plan, the preliminary Water
Availability Fee calculation would be:
1.22 Acres @ $1,OlO.OO/Acre Current Water Availability Fee = $l,232.20 or $1,232.00
Connection Fees - Based upon data from the site plan, the preliminary Water Connection Fee
calculation would be:

18. The required sidewalk must be within an easement as it is not shown within right-of-way.
19. Please revise CURB JOINT DETAIL such that the maximum transverse expansion joint spacing is 50
feet, the maximum transverse contraction joint spacing on tangents is 10 feet, and the maximum
transverse contraction joint spacing on radii is 5 feet.
20. Open pavement cut connection to West Clay structure #912 requires Board approval. The City's RCP
bedding detail shall be included in the plan set for the portion of this storm sewer ron in the Moultrie
Street right-of~way.
21. Depressed curb detail shall be replaced by the City standard detail.
22. There are notes "by others" on Sheet C3.I. Is this infrastructure not installed?
23. The City utilizes 1" ribbing in Hew of the trWIcated domes for ADA ramps in the right-of-way.
24. The sidewalk detail shaIl.meet City standards for portions in the right-of-way.
25. Striping in the right-of-way shall be thermoplastic.
26. Is the walk on Moultrie Street already installed or to be installed with this project?
27. The proposed connection to West Clay structure #775 shall be by a bonded contractor.
28. Site Note 16 - Please revise to read "Existing pavement and curbing..."
29. Is pedestrian access provided along Pettigru? Ifon-site, and easement shall be provided.
30. Are cuts of Moultrie to be constructed with this project or with Moultrie Street?
31. Is the southbound aisle/travellane on Pettigru wide enough to allow proper maneuvering of cars
backing out?
32. The Department of Engineering acknowledges that the drainage management system was previously
master planned. Please contact Amanda Foley to review construction and post-construction storm
water quality requirements.
33. Final plans shall be approved for the CVS parcel and associated right-of-way improvements alone. All
other improvements indicated that have not been approved or are not existing shall not be considered
approved with this project.
34. Site Note 11, Grading Note 6, and Utility Note 6 shall be revised to require trenching and backfilling
procedures and materials to conform to City standards in the right-of-way.

The following letter represents comments from the Engineering Department for this project specifically
addressing the areas of storm water quality. We have reviewed the construction drawings submitted for
this project for the November 15, 2006 Technical Advisory Committee meeting and offer the following
comments:
BONDING REQUIREMENTS

paved areas. Specify another inlet protection measure for these areas.

Submit spill information to the City of Carmel, not the SWCD.
C2. Sequence describingstormwater quality measure implementation
The City of Carmel requires that all stormwater leaving a site (post cOllstmction) be treated by a
minimum of two different water quality BMPs. A second BMP (in addition to the pond) is
required for tllis project.

We have reviewed the constnlc::tion plans submitted to the Hamilton County Surveyor's Office
on November 6,2006 for this project and have the following comments:
1. This project falls within the corporate limits a-q,d MS4 jurisdiction of the City of
Carmel.
2. The proposed project DOES NOT fall in a Cannel Wellhead Protection Zone.
3. The proposed project falls in The Village of West Clay Regulated Drain Subdivisjon
and The Village of West Clay watershed. Th.e proposed project also falls jn the J.W.
Brendle Regulated Drain Watershed.
4. Storm water detention for the site is provided in the master plan for Village of West
Clay.
5. Please submit an outlet permit for the direct discharge into the Village of West Clay
Regulated Drain and an outlet pennit for the direct discharge into the J.W. Brendle
ReguJ.ated Drain. The applicat10n is available on our website at
http://www.co.hamilton.in..us. go to Departments, Surveyor, and Fonns.
6. The storm drains being installed with this projeet will be consider private and the
maintenance responsibility will be that of the property owner or association.

regulated drain that falls in this area.. A 20' R.D.E. will neOO. to be granted over the
storm sewer frgm Stt. 774 to 715 then to 91.S. How this is handled will depend on. the
fInal determination from comment #7 above.
9. Please contact Brandon Burke at Schneider to get the correct location for West Clay
Str. #912, as the current shown location does oot match the design we are approvi.ng
for the l.W. Brendle Drain Relocation.
10. The current landscape plans show a huge amount of landsca.ping in Regulated Drain
Easement and over the VilIa.ge of West Clay Regulated Drain. Please modify the
landscape plans to remove aU or minimize the landscape planting in the R.D.E.
