Dear Mr. Butz:
The City has received your construction plans that were presented for review for the February 20, 2008
Technical Advisory Committee Meeting. We offer the following comments:
GENERAL INFORMATION
1. These comments represent the Department of Engineering's first review of the construction
plans for this project at this location.
2. We request that all responses to our comments be provided in writing and be accompanied by
a drawing reflecting the requested revisions. Failure to provide written responses may result
in the delay of the review process.
3. It is critical that this office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans including all revisions. Please notify us of
any changes and specifically state any changes, including changes resulting from Plan
Commission, BZA or other committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review. If you have not already done so, please
provide a set of drainage plans and calculations to their office for review. We will share
Crossroad's comments.as they are received.
5. Final drawings will not be approved for construction until:
a. All Engineering Department and Utility Department and Hamilton County Surveyor (if
applicable) issues have been resolved.
b. AU bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) are obtained.
d. All off-site easements necessary to install utilities to serve the development are secured.
e. Submission of required digital files.
f. All required fees have been paid.
6. The Department reserves the right to provide additional comments based upon subsequent
reviews.
7. An approved Storm Water Management Permit is required prior to commencing any earth
disturbing activity.
8. An approved right-of-way permit is required prior to commencing any work in the public
right-of-way.
9. The developer is responsible for all costs associated with utility relocations required for this
project. If it will be necessary to relocate existing utility poles in the right-of-way, they must
be relocated to within one foot of the proposed right-of-way.
10. Compliance with the Thoroughfare Plan includes construction of the improvements
prescribed by the Thoroughfare Plan across the frontage controlled by the development
(including sections with no access). Typically, developers provide a commitment equal to

the value to construct these improvements for deposit into the City's non-reverting
thoroughfare fund. Such compliance is applicable to the Old Meridian Street frontage.
11. The Department requires that the construction drawings be developed in accordance with the
City of Carmel digital submission standards and that all required submittals for primary plat,
secondary plat, and construction drawings be made. The digital files must be submitted to
the Department of Engineering prior to the approval of the construction plans. Please contact
the City GIS Department for the requirements.
12. Jurisdictions:
a. The project site is located within current City of Carmel Corporate Limits.
b. Streets and Right of Way - City of Carmel (Old Meridian Street)
c. Water - City of Carmel Utilities
d. Sanitary Sewers - City of Carmel Utilities
e. Storm SewerslDrainage - City of Carmel.
f. Legal Drains - Hamilton County Surveyor's Office.
13. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require a minimum of 5-sets of drawings for approval after all issues
have been resolved. The drawings will be stamped as approved and signed by the City
Engineer and by Carmel Utilities. The O\mer will receive I-approved set which must be
maintained on the construction site at all times. If additional approved sets are desired,
we will approve a maximum of 2-additional sets. However, the additional sets must be
submitted with the required minimum 5-sets.
14. Carmel Utilities will provide separate reviews of this project for water issues. Please assure
that copies oj all drawings are sent to:

15. Carmel Utilities does subscribe to "Holey Moley" who should be contacted directly for all
water main locations.
16. The following Engineering Requirements will be forwarded to your attention via email at
your request. Please assure that the OwnerlDeveloper and Contractors to be associated with
this project receive copies of these requirements:

BOARD OF PUBLIC WORKS AND SAFETY
17. r am enclosing a schedule for Board of Public Works and Safety meeting dates and agenda
deadlines for your use. Please use the Engineering Department deadlines for submissions to
the Board.
18. Any submission to the Board requires prior approval by the Carmel Clay Plan
Commission and/or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Docket Number and the date (or dates) of
approval by the Plan Commission andlor the Board of Zoning Appeals (if applicable).
19 . Water Availability (EDU) approval from the Board is based upon the proposed usage of the
building. Reference item #34-37 below for a more detailed explanation. Ifan irrigation

system is to be included with this development, approval for additional Water Availability
must be obtained from the Board and additional Water Connection Fees will be assessed.
20. Board approval will be required for the Old Meridian Street Commercia] Curb Cut and for
the Old Meridian Street median modifications. Please provide 8Yz x ]] exhibits with your
request for Board approval. The exhibits should include all pertinent data including drive
width, drive width at street, radii, opposing curb cuts, etc.
21. Temporary Construction Entrance approval is required if the location is at a site other than
the location of the proposed permanent entrance to the development.
22. The installation of any permanent, privately owned andlor maintained improvement (signs,
decorative street signs, walls, streetlights, etc.) within dedicated right of way or dedicated
easements requires the execution of a Consent to Encroach Agreement between the Owner
and the City of CarmeL Such agreements are executed by the Board of Public Works and
Safety. The City Engineer, representing the Board, may approve irrigation system
agreements.
23. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats for Board of Public Works and Safety approvaL
24. Dedication of Right of Way if applicable. Appropriate dedication documents are available
on request.
25. Any open pavement cuts within Old Meridian Street. It appears that the gas service line may
require open pavement cut approval.
BONDING REOUIREMENTS
26. Upon initial review, it appears performance guarantees shall be provided for the following:
a. Concrete Sidewalk within the right of way of Old Meridian Street.
b. Right of Way Improvements. This would all other construction activities within the right
of way including demolition, curbing, parallel parking spaces and entrance apron
construction.
c. Sanitary Sewers (new manholes, piping, service laterals, core drilling, etc.)
d. Water Mains (tap existing line, valves, domestic and flre service laterals, etc.)
e. Perimeter Monumentation at property corners.
f. Erosion and Sediment Control and Storm Water Quality. Please contact Amanda Foley,
Engineering Department Stormwater Administrator, for details regarding engineer's
estimates/performance guarantee requirements for these items.
27. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc. Copies of
Certified Enf!ineerJs Estimotes are to be Drovided to the Enf!ineerinf! Deoartment Drior to
or at the same time that thev are Drovided to the Developer.
28. Upon completion and release of individual Performance Guarantees, a three-year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based upon
]5% of the Performance amount for Streets and Curbs and 10% of the Performance amount
for all other improvements.
29. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
30. Please reference the available enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
31. Any work in the dedicated right of way of Old Meridian Street will require an approved
Right of Way Permit and a License & Permit Bond.
32. The bond amount is determined by the number of instances of work in the right of way at
$2,000.00 per instance. However, if the work is included in the scope of work ofa required

and posted Performance Guarantee, the Performance Guarantee may be used to satisfY the
bond requirements of the Right of Way Permit.
33. Please contact our Right of Way Manager, Fred Glaser, to arrange right of way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
34. New water related fees have been authorized effective July 2,2007. The revisions are:
a. Tap Inspection Fee-An $86.00 tap inspection fee shall be charged for each new user
within Cannel's water system. The current Water Connection Fee is $1,310.00. This has
not been revised. Thus $1,3 I 0.00 Water Connection Fee/I.O EDU plus Water Inspection
Fee of$86.00/EDU = $1,396.00/EDU
b. Development Application Review Fee-This fee will be assessed at the time the Water
Availability Fee is paid. The current Water Availability Fee is $1,OlO.OO/acre. This has
not been revised. The review fee is $150.00 for single commercial, business and
domestic fire service lines and $300.00 for developments with more than one customer
requiring a water connection.
35. Availability (acreage) and Connection (EDU) Fees must be paid after all other Engineering
Department requirements have been satisfied and prior to pre-submittal and approval and
start of construction activities.
a. Availability Fees are based upon total platted acreage or legal description acreage for the
development at the current rate of$I,01O.00 per acre for Water Availability, $1,365.00
per acre for Sanitary Sewer Availability plus a new Development Application Review
Fee of$300.00 ($150 for one user, $300.00 for two or more) for this project. According
to the Secondary Replat, the project area is 2.848 acres. Based on this, tentative
Availability Fees are:
Water - 2.848 Acres@ $1,OlO.OO/Acre = $ 2,876.48
Sanitary Sewer - 2.848 Acres @ $1,365.00/Acre = $ 3.887.52
Total Availability Fees $ 6,764.00
Development Application Review Fee 300.00
b. Connection Fees are based upon the planned use of the development.
EDU Calculation
Please provide a breakdown of uses for the 60,000 ft2 building. Retail EDUs are
calculated on the basis of 1.0 EDU per 1,000 ft2. Office EDUs are calculated on the basis
of 0.5 EDU per 1,000 ft2. If there are uses planned that would result in a higher EDU
calculation, those higher calculations would apply. For example, Restaurant EDUs are
based upon the total seats in the facility. This would include outside seating and bar
and/or cocktail lounge seating. Other uses are listed in our Equivalent User Contribution
Multiplier tables, copies of which are available on request. Please provide this
information in order to calculate Water and Sanitary Sewer Connection Fees for this
project. Connection Fees for the future 11,400 ft2 building would be payable when
developed.
36. In addition to the above fee calculations, additional Connection Fees will be assessed:
a. If an irrigation system is planned for the facility, additional Water Connection Fees will
be assessed based upon the size and usage of the system and upon the recommendations
of the Director ofCanne] Utilities.
SECONDARY REPLA T REVIEW COMMENTS
37. Per our Subdivision Control Ordinance, concrete monuments are required at aU property
corners.

40. All required Performance Guarantees must be posted with the Department of Engineering
before the plat will be submitted to the Board of Public Works and Safety for approval.
CONSTRUCTION DRAWING REVIEW COMMENTS
41. General Comments
a. This project is subject to the City's Stonn Water Management and Storm Water Quality
Ordinances.
b. The City has adopted a new curbing policy. Please revise any notes, specifications, and
details to accommodate this policy. The language of this policy must be added to a
specification sheet on the fmal construction plans.
c. Please provide the requisite storm water/drainage information required by Section 100 of
the Storm Water Technical Standards Manual on. the plan sheets.
d. Please contact Amanda Foley to review storm water quality/treatment requirements.
e. For the plans submitted for Engineering Department approval, please remove the
landscape plan and detail sheets and the site lighting plan sheet from the plans and
remove the titles of these sheets from the Index on the Cover Sheet. The Engineering
Department does not have approval authority over these items and they should not be
included in plans submitted for approval by Engineering.
f. The City is .not requesting entrance auxiliary lanes, widening, milling, or resurfacing of
Old Meridian Street across the frontage.
g. It would be helpful to include lot dimensions on all applicable construction plans.
h. A Minimum Flood Protection Grade and Lowest Adjacent Grade as required by the
Storm Water Technical Standards Manual shall be established and indicated for this
property. The defmitions per the Storm Water Technical Standards Manual shall be
added to the plans.
42. Sheet C6.1
a. CURB JOINT DETAIL - Please revise the maximum transverse expansion joint spacing
to 50 feet. Be sure to label this as a maximum spacing. Also indicate that the maximum
transverse contraction joint spacing shall be 10 feet on tangent sections and 5 feet on
radius sections.
b. Please revise the pavement sections using sieve size designations for the aggregate in the
various courses rather that metric superpave designations unless you are indeed planning
to use superpave asphalt mix.
43. Sheet C6.2
a. The City requires sewer manholes with integral bases rather than separate precast or
poured in place concrete bases. Please revise this sheet and any related sheets
accordingly.
44. Please verify the parking stall dimensions from edge of mainline travel lane to face of curb
with Crossroad Engineers.
45. Board of Public Works and Safety approval is required to close the existing sidewalk and for
any lane restrictions for constructing the on-street parking.
46. 36-inch chairback curb is noted in the right-of-way. Any proposed curbing in the right-of-
way shall match the existing right-of-way curbing. I believe INDOT standard curb was
utilized on the Old Meridian Street project.
47. Please include the following information on the Cover Sheet: (1) Engineer's email address.
(2) Name and email address of engineering project manager, project architect and project
surveyor. (3) Developer's name, address, phone number, fax number and email address.
48. A legal description of the site is required on the Cover Sheet.
49. Please indicate the current and proposed zoning on the Cover Sheet.
50. An access easement to the south (as we discussed previously) is not indicated on the current
plan set.
51. Please indicate sub-surface drain under all curbing in the right-of-way.

52. The Department expects that all storm drainage is to be part of a private system. Therefore,
easements over the piping and the pond are not required. However, an easement will be
required for the pipe stubs out to the right-of-way if the City Engineer determines that
detention of the right-of-way is required and piping is to be extended to the right-of-way as
required by the Storm Water Technical Standards Manual. If it is determined that this will
not be a requirement. A formal variance from this standard win be required. As there are no
pipes extended to the right-of-way on the current plan, the Department assumes that such
detention is not provided.
53. The Storm Water Technical Standards require a 25-foot setback from the lOO-year elevation
of the pond to any structure to be occupied. As the building adjacent to the pond is a future
building, the Department requests that this setback be formally indicated on the drawings as a
building setback line.
54. All technical information required by Section 100 of the Storm Water Technical Standards
Manual shall be indicated ON THE PLANS.
55, Are there existing culverts under the existing driveway serving the site? What is referenced
by the "Twin 18-inch" note on Sheet C 1.2 in the vicinity of the existing drive?
56. ADA ramps are not indicated where the proposed sidewalk crosses the proposed access
drive.
57. It is not clear where bicycle parking is being provided.
58. There can be no direct discharge areas. Swales are required along the north and south
property lines to direct runoff to the detention pond and to treat the water, Such swales are
required to have sub-surface drains. Is it possible to make these swales bio-retention or
inf11tration swales?
59. Please add a note to the plans that there shall be no frames or castings (expect curb inlets)
associated with sanitary, storm or water utility systems in pavement, sidewalk or curbing in
the City rigbtooQf-way.
60. Please verify that the location of all existing fire hydrants within the development or within
300-feet of the development are indicated on the plans.
61. Please verify that the location of all known overhead and undergounrd electric, CATV,
telecommunications and gas utilites is indicated on the drawings.
62. The plans shall be updated to indicate the final configuration of the improvements to the
existing median and opposite curb line per recent discussions with the City Engineer. The
Department is still reviewing the proposed modifications.
63, Please indicate all existing pavement markings on the drawing sheets.
64, Thank you for indicating the sidewalk improvements within the right-of-way. The City's
standard is for such walk to be at the back edge of the right-of-way. Please verify with the
DOCS that the proposed configuration is acceptable.
65. Please add a note to the drawings requiring thermoplastic pavement striping in the right-of-
way.
66. All new pavement in the right-of-way shall be indicated by hatching, including the parking
spaces and any new pavement associated with the median work. The hatching legend shall
reference the City pavement section for mainline right-of-way. The pavement section for
mainline right-of-way shall match that pavement section of the Old Meridian Street project
and shall be added to the plans. Please add a note to the right-of-way pavement section that
requires all paving to be completed in one season.
67. Please indicate the portion of the proposed entrance within the right-of-way to be 6.inch
minimum concrete or that pavement section adequate for the largest vehicle expected to
regularly access the site.
68. Please indicate City standard depressed curb across the entrance.
69. A maintenance of traffic plan in accordance with the INDOT Work Zone Safety Manual shall
be developed for all work in the right-of-way.
70. Please include the City standard sub-surface drain detail on one of the detail sheets. All sub-
surface drain shall be double wall Hancor Hi-Q or equivalent.

71. Please add the revised detaiI604-SWCR-02 (available from our office) for walk in the right-
of-way and add a note to the site plan referencing these details for such ramps in the right-of-
way. If on-site ADA ramps will conform to the current details, then both details are required.
72. Please incorporate the City's standard backfill requirements for water and sewer main and
lateral installation in the right-of-way into any notes and details.
73. Are any street cuts planned for utilities? If so, the City's street cut and patch detail shall be
added to the plans. Such cuts require BPW approval and will be discouraged since Old
Meridian Street was just recently completed.
74. Please include the City standard reinforced concrete pipe detail for the storm sewer in the
right-of-way.
75. Please add the City's standard depressed curb detaiL Such depressed curb shall be indicated
across the proposed entrance as well as the work for the future entrance on the opposite side
of Old Meridian Street.
76. A plan and profile will need to be developed for the short storm pipe run in the right-of-way.
All existing and proposed utility crossing shaH be indicated on this profile. Please add a note
to this sheet per the STSM. Please contact this office to obtain this note.
n. The City's paving policy shall be included on the plan sheets for work in the right-of-way.
78. Please add the following note to all sheets: "No earth disturbing activity may commence
without an approved storm water management permit."
79. Please add the following note to the appropriate sheets: "All paving within the existing and
proposed City right-of-way shall conform to the requirements of the Department of
Engineering. The Contractor shall contact the Department of Engineering to schedule a pre-
construction meeting to review the Department's construction requirements, staff notification
requirements, required inspections for certain stages of the work and to review the authority
of the Department as it relates to work within the existing and proposed City right-of.way."
80. For all existing curb demolition in the right-of-way, please add a note that requires removal
up to the first joint past the required limits of removal. The existing curb shall be sawcut and
removed at this joint.
81. Please add a note requiring the exiting asphalt path to be sawcut at the limit of removal.
82. A formal variance for the outfall at the property line shall be obtained from the Board of
Public Works and safety.
83. The proposed pond does not meet the minimum area requirements for a wet pond. Is the
pond intended to be a wet or dry pond?
84. Does any part of the existing right-of-way behind the back of curb drain across the property?
Does any runoff from the adjacent properties currently drain into the site?
85. The normal crown of the roadway shall be maintained into the parking spaces. An inlet will
be needed and a pipe extended from the existing inlet in the right-or-way at the entrance. I
believe this is how the on-street parking is being configured elsewhere on Old Meridian
Street.
86. Note II on Sheet C2.1, Note 6 on Sheet C3.1, Note "E" on Sheets C3.2 and C3.3 and Note 6
on Sheet C4.I shall be modified to require City backfill standards where such installation is
in the right-of-way.
87. Note 12 on Sheet C2.1 shall be revised to conform to the City's paving policy.

6. Previous Comment 51. Thank you for your response. The City standard is to have no part of the
entrance be wider than 30-feet in the ri.ght-of-way (excepting the area created by the return radii).
It appears that the return radii on the north side of the drive varies and allows a portion of the
throat to become wider than 30-feet. Please make the revisions per tbe attached sketch.

The following letter represents comments for this project specifically addressing the area of alternative transportation.
I have reviewed tbe drawings submitted for the February 20, 2008 Technical Advisory Committee meeting, and offer
the following comments:
ALTERNATIVE TRANSPORTATION REVIEW COMMENTS
1) The City of Cannel Parking Ordinance requires that all commercial and retail venues include bicycle
parking. Bicycle parking spaces are required at a rate of five (5) bicycle parking spaces per one hundred
(100) automobile parking spaces with a minimum offoUT (4) spaces. Please revise your plans to show
the required bicycle parking spaces on your site. Please see the City of Carmel's Parking Ordinance in
order to determine appropriate locations, specifications and construction details for the above.
2) Please indicate ADA compliant handicap ramps at all crossings.
We request that all responses to our comments be provided in writing. Failure to provide written responses may result
in delay of the review process.
It is critical that this office be made aware of all modification made on the plans being re-submitted, particularly if any
such changes are considered "new" or fall outside of our previous reviews. Please provide revised plans indicating all
revisions. Please notify us of any changes and specifically state any changes, including changes resulting from Plan
Commission, Special Studies or other committee meetings.
The Department of Community Services reserves the right to provide additional comments based on subsequent
reviews.

should do the same here, What about Buckingham farther to the souh Have they established a streetscape in front of
Providence Shops?

This is a new development from American Structurepoint along Old Meridian for the Keystone Veterinary Group.
Do you know if the Streetscape package for Old Meridian will be built for them by the City, or if they will be responsible for

We hl1ve reviewed the construction plans submitted. to the Hamilto.n County Surveyor's
Office on January 18.2008 for this project and have the following comments:
1. This proposed project falls within the incorporated area and MS4 jurisdiction of the
City of CanneL
2. The proposed project DOES NOT fall in a Carmel Well Head Protection Zone.
3. The proposed project falls partially within W.R. Fertig Regulated Drain watershed,
and partly in the Little Cool Creek watershed. The W.R. Fertig drain has an allowable
release rate of 0.25 cfs per acre.
4. It would appear all the developed storm water runoff is being directed to Little Cool
Cree.k, but that can not be accurately verified off of the construction plans. If that is
true, no pennits will be needs from HCSO, as all approvals will be granted by the City
of Carmel Engineering Department.
5. Please submit draina.ge calcul$.tions so we can verify drainage shed area for this
proj eet.
6. Please note that further conunents may be necessary at a later date.
