HomeMy WebLinkAboutI0825506 Performance Insurance CDsDocument Approval Client / Tenant Date Title Property Manager Architect / Interior Designer Date Upon execution, the client and property manager approve all aspects of the scope of work shown in these documents, including design content, scope of construction and finish selection for the following phases: Space Planning Preliminary Pricing Construction Documents Other:_____________________________________ All costs incurred due to misinterpretation of these documents or changes to the project requested after the execution of this approval shall not be the responsibility of Carson Design Associates and shall be the sole responsibility of the client / tenant. 1. Contractor is responsible for all the design required to obtain approvals and permits and is required for complete installation of Mechanical, Electrical and Plumbing systems. 2. The design and installation of Mechanical, Electrical and Plumbing systems shall comply with all requirements and standards of all applicable codes, governmental regulations, ordinances and state and local requirements. 3. Contractor shall provide construction drawings and calculations stamped by a professional engineer licensed in the state of Indiana. 4. Contractor shall submit construction drawings and an application for construction design release to the state of Indiana Plan Review Division of the Indiana Department of Homeland Security for the purpose of obtaining the required construction design release(s) for the Mechanical, Electrical and Plumbing systems. 5. Contractor shall coordinate the requirements of the Mechanical, Electrical and Plumbing systems as required for complete installation of each system. 6. Contractor shall be responsible for providing appropriate heating, cooling and humidity control as applicable in all spaces, including existing spaces outside the scope of this project that are re-configured by the new construction. 7. Contractor shall be responsible for re-wiring existing light fixtures, switches, outlets and miscellaneous devices as required, including existing light fixtures, switches, outlets and miscellaneous devices outside the scope of this project that are re-configured by the new construction. Aerosol Adhesives are governed by Green Seal Product Standards and Certification GS-36 with its limits as listed below. Electrical Notes A. This drawing is intented to provide information relating to the location of wiring devices and outlets only. It is the responsibility of the electrical design/build contractor to provide necessary documents relating to the actual electrical installation to install all wiring devices in accordance with all applicable state and local codes. B. All dimensions are from finished wall surface or column centerline to centerline of outlets or centerline of pairs of outlets. C. CARPET SYSTEMS, CARPET Section 3: EQ Credit 3.1/3.2 LEED EB Carpet systems including the carpeting, seam sealers and adhesives must meet the criteria of the Carpet and Rug Institute’s Green Label Plus system. See Section 1, Adhesives and Sealants, above for the requirements of the carpet sealers and adhesives. Green Label Plus is an independent testing program that identifies carpets with very low emissions of volatile organic compounds (VOCs). It is an enhancement to the CRI Green Label Testing program which has been consistently upgraded since it was developed in 1992. Green Label Plus meets California’s CHPS (Collaborative for High Performance Schools) criteria. To receive Green Label Plus certification, carpets must undergo a 14-day testing process that measures emissions for a range of chemicals and is administered by an independent laboratory. The test methodology was developed in cooperation with the U.S. Environmental Protection Agency (EPA). Every carpet receiving Green Label Plus certification has been tested for emission levels for 13 chemicals, utilizing a 14-day test. The 13 chemicals measured are: Acetaldehyde, Benzene, Caprolactam, 2-Ethylhexanoic Acid, Formaldehyde, 1-Methyl-2-Pyrrolidinone, Naphthalene, Nonanal, Octanal, 4- Phenylcyclohexene, Styrene, Toluene, and Vinyl Acetate. Subsequent annual tests are based on 24-hour testing that targets 13 chemicals. Plus, quarterly testing will measure for the total level of volatile organic compounds (TVOC). Commercial carpet that meets the criteria for Green Label Plus certification is listed on the CRI websites available at www.carpet-rug.org. If the carpet being considered is not Green Label Plus certified, it can be submitted to the manufacturer for testing, in order to determine if it qualifies for Green Label Plus certification. CARPET SYSTEMS, CUSHION Section 4: EQ Credit 3.1/3.2 LEED EB Carpet cushion must meet the criteria of the Carpet and Rug Institute’s Green Label system. Criteria are based on a maximum emission factor as follows: Total Volatile Organic Compounds 1.00 mg/m2 . hr. BHT (butylated hydroxytoluene) 0.30 mg/m2 . hr. Formaldehyde 0.05 mg/m2 . hr. 4-PC (4 phenylcyclohexene) 0.05 mg/m2 . hr. CRI has established a program that tests for volatile organic compounds (VOCs) in cushion used under carpet. This program will help the carpet manufacturer, installer and end user to identify and utilize low-emission cushion. Cushion products that meet the emissions criteria can display the program’s green and white label. Products are re-tested regularly for continued compliance. Consumers seeking a low-emitting cushion can confidently select those bearing the label. Cushion products are characterized as: prime polyurethane, bonded polyurethane, mechanically frothed polyurethane, rubber – hair, rubber – jute, synthetic fiber, resinated or coated synthetic fiber, rubber and rubberized polyurethane. Cushions are tested for total volatile organic compounds, BHT (butylated hydroxytoluene), formaldehyde and 4-phenylcyclohexine (4-PC). Commercial carpet cushion that meets the criteria for Green Label certification is listed on the CRI websites available at www.carpet-rug.org. If the carpet cushion being considered is not Green Label certified, it can be submitted to the manufacturer for testing, in order to determine if it qualifies for Green Label certification. COMPOSITE PANELS & AGRIFIBER PRODUCTS Section 4: EQ Credit 3.1/3.2 LEED EB Formaldehyde is a natural component of wood products, but some wood glues and wood products made with these glues contain significantly higher amounts than does wood alone. Regarding composite panels and agrifiber products used on site it is required that no additional urea-formaldehyde be added to the products during the manufacturing process. Pressed wood products often contain urea-formaldehyde glues. Products that may add formaldehyde to the air include particleboard used as flooring underlayment, shelving, furniture and cabinets; MDF in cabinets and furniture; hardwood plywood wall panels, and urea-formaldehyde foam used as insulation or fiberglass insulation. The glue or adhesive in pressed wood products such as particleboard, hardwood plywood, and medium density fiberboard (MDF) are common sources. Avoid the use of foamed-in-place insulation containing formaldehyde, especially urea-formaldehyde foam insulation and use alternative products such as wood panel products not made with urea- formaldehyde glues. Composite wood and agrifiber products must contain no added urea-formaldehyde resins.
