additional measures may be required to control erosion and sedimentation. All practices recommended in this report should be
evaluated as to their feasibility by a qualified individual with structural practices designed by a qualtfied engineer.
All erosion and sediment control measures shall meet the design criteria, standards, and specifications outlined in the "Indiana
Handbookfor Erosion Control in Developing Areas" or similar guidance documents,

Dear Dave:
This letter is in response to the Replat of Section l0004-A of the Village of West Clay. The
following issues must be resolved prior to signature of the plat by the director:
1. The Secondary Plat Docket number is 08-03 SP. Move to sheet 1.
2. Add dimensions and bearings to Plat (turn the layer back on).
3. Have Mylar signed by property owner and surveyor (with R.L.S. seal) prior to
signature by director.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes
have been made and comments addressed, please produce a Mylar copy for signature by this
Department. Two paper copies of the plat must be filed with the Mylar prior to recording, and
three paper copies and a Mylar of the recorded plat must be submitted following recording. Our
approval of the secondary plat (signature) will be forthcoming upon revision of the plans
according to the items listed above.

We have received your fax dated January 14, 2003, which was a review of the construction plans
and secondary plat for the above referenced project I have addressed each of your items
individually with a numbering system consistent to your comment letter. Please find enclosed
one set of revised site construction plans for the Village of West Clay Section 10004-A for your
review and comments.
Secondary Plat
1. Currently shows a 35' triangular RfW cut, I understand this to be consistent with a
previous variance.
2. Area is now indicated asa '(PA) planting area. In regards to your question, no use that
would require a common area.
3. 2' NAE was added, see plat or C101JC102

5. -Bonds will be based of actual contract amount once all revisions are complete.
Construction Plans
6. The storm sewer has been relocated outside the right-of-way into the 20 DU&SE Per a
meeting with I.W.C., please note changes on water distribution plans C701-702.

8. The passing Blister extends to the Laurel Lakes accellane; the existing pavement will be

This letter serves to acknowledge receipt of a transmittal dated 01/03103 containing the
plans for the above~mentloned project. After reviewing the plans the Highway
Department has the following comments:

1. A 50' triangular right of way cut is needed at the intersection of Shaftesbury Road
and Towne Road.
2. 15 a guardhouse or any special treatment planned for the entrance that would
require a common area to be in the median?
3. A two-foot non:..access easement is needed behind the Towne Road right of way.
The easement should @):tend on Shaftesbury R.oad to the all~y easement.
4. Please change the Commissioners signature block to show that Christine' Altman
has replaced Sharon Clark.
5. Before any approvals are granted for the Secondary Plat, an Engineers Estimate
broken into the following categories will need to be submitted. along with a
Performance Bond or Letter of Credit in the amount of 100% of the estimate:

the SSD stub. Blind taps of the RCP will not be permitted.
Does the passing blister on the west side of Towne Road connect to t.he accel
taper of Laural Lakes? Please show the Laurel Lakes entrance on the site plans
and entrance details.
Please note on the entrance detail that pavement for the entrance or accelldecel
lanes and passing blisters cannot be placed until all utilities have been relocated.
A striping plan will be needed 011 the entrance detail. A 1" surface course of HMA
will also be required from the south end of the decel taper to the nortn property
line.

11. Please show the necessary drainage along the passing blister on the west side
of Towne Road.
12. A three-foot stone shoulder is needed along all of the widening.

14. A note is needed at the connection of the s1reet to the roundabout stating the
existing pavement must be saw cut to provide a smooth trans~tion.
15, A pavement section is needed for the Towne Road right of way widening anCf
entrance. The section should consist of a 14" stone bass. 5" HAC bInder and 1"
HAC surface.
16. Discu5sion of the eyebrow will be presented at the January 15 T AC meeting.
17. The rcundabol,lt ahead signs, similar to 1hose in other sections,. are needed on
the Traffic Control Plan. Yield signs are also needed at the eyebrow. A 20 MPH
sign is needed on Montcalm Street and s of Blisland Street on both sides of
Shaftesbury Road.
18. Please include the 20 MPH design speed on the cover sheet.
19. Correct structure #656 in the profile on sheet C604, to the correct structure
number of 654.
20. Change general note four on the water distribution plans from all to NO.
21. Are ADA ramps needed to access common area A?
22. The ADA ramps must be shown In each direction at the intersections. One
depression in the radius wBl not be accepted. The same Is true or the path at the
intersection of Shaftesbury Road and Montcalm street
23. If construction traffic is to. enter off of Towne Road, a construction drive must be
shown on the erosion control plans. One is also needed at the connection to the
roundabout.
24. Towne road will need to be widened across the entire frontage, to a 12' travel
lane, The accellane should also be extended to form a passing blister for the
entrance into Laurel LaKes,

manhole, lateral and bedding details which are in accordance with District specifications.

criteria, standards, and specificatioll.~ outlined in the "Indiana Handhook/or Erosion Control ill Developing Areas" from the
Indiana Department of Natural Resources, Division o/Soil Conservation or similar Guidance Documents.
4A.& 4C. Statements 1,3,4 & 5 in the Erosion Control and Sequencing are too general for the intended
purpose. Perimeter protection installed prior to construction for this project should include the constmction
entrance and a diversion swale from the west end of Block B pond to the existing pond in Block C. Rough
grading can commence after these practices are i)1stalled. Statement three needs to be removed from the
sequence and put with general notes. The words "ground activity" should be changed to "rough grading".
The pond banks and swales need to be seeded and blanketed immediately upon completion. Pads, common
areas and most of the lots should be temporarily seeded immediately after rough grading. Statement 5 is too
general for the sequence.
4C. Does the existing pond in Block C meet the sediment basin storage requirements for the disturbed area?
4E. Silt fence at Stmcture 662 is not adequate. A rock donut would be appropriate.
4 I. Show a construction entrance on the plans. One entrance could serve this section and Sec. I 0004- B if it
were located north of lot 928.
