The follo"Wing comments represent our final review for plan approval for tills project:
Pavement Issues
o Label and show limits of saw cutting curb and/or gutter. Saw cut pavement to minimum 7".
Protect edge of asphalt pavement of Hazel Dell. Any edges broken and/or damaged will be
repaired to the satisfaction of the City of CamleL
. Concrete Median Section: Plans need to show lane lines (pavement markings) to clearly show
where the median section is to be installed. The median should be 2' wide.
o Extend the median/concrete center curb to the stop bar at the 146th Street intersection (see Plans,
Sheet CI02A). See Type D Concrete Curb Standard (2'0"), which is enclosed.
Ri!.!ht-of-Wav Issues
& Identify and label all trees within the landscaping easement belonging to the City of Carmel
adjacent to the right of way lines. Show where trees are to be removed and/or relocated. Check
with our Urban Forester, Scott Brewer, for his recommendations. Out inspector reports that 3 or 4
trees will need to be removed and that the entrance was staked on October 9u,.
e The drawings must include a Maintenance of Traffic Plan for median work, existing drive removal
and the installation of the newenh'ance.
o Do the plans show limits of path to be removed and reinstalled or sidewalk closed signs to be
placed? If not, please do so.
Draina2e Issue
. The existing culvert under Hazel Dell is submerged and needs to be cleaned out at the outlet. The
31:ea from this point dO\vn (ditch section) "ill need to be cleared so that drainage has the ability to
flow through it. Our inspector notes that a large pile of rip rap is obstructing the flow of water in
the ditch (copies of photos coc losed).
Other
o On Sheet B 1 0 1, the Hazel Dell path is shown as concrete. It is asphalt.
Be advised that any drawings submitted for approval by this office will not be approved with the statement
above the certification block on the Title sheet ("These drawings are subject to periodic revisions by the
Schneider Corporation. The holder is responsible for verifying that these documents are. the most current
prior to use").

When these revisions have been made to the plans, this office will approve the plans and the remainder of
construction (Hazel Dell Parkway right of way, water and sanitary sewer) may proceed. We will require
that all required Performance Guarantees be posted and Availability (acreage) Fees be paid. We require a
minimum of 5-sets be submitted for approval. However, we v{ill approve and sign up to a maximum of 7~
sets if so desired.

This letter is in response to Docket No. 09-03-PP pertaining to the real estate located at the
southeast corner of 146th Street and Hazel Dell Parkway. Our property borders on the location
in question, and we are adamantly against this proposed rezoning request. We built our home over
seven years ago in Ashmore Trace, and we have (as unified neighbors) fought against commercial
rezoning the entire time we have lived here, Every year or two, a new proposal is brought before your
cOnurllssion to allow commercial development here, and each and every time, all ufthe neighborhoods
that would be directly (and negatively) impacted by such development fight against it. It appears that
the developers (Plum Creek Partners) hope that either we eventually give up, or that your commission
disregards our opinions and allows them to go forward. It is true that we tire of constantly battling
with these people, and each time a proposal comes forward for commercial zoning, more of our
neighbors tire of the endless struggle, but we remain unified in our opposition to this request for
rezoning or any future commercial rezoning EVER. We ask you to once again reject this commercial
development and stand with us in maintaining this property as it was designated originally as
residential property.

Dear 1\1r. Frankenberger:
We have review'ed the drawing submitted for this project and offer the following comments:
1. To my knowledge, we have not received a \vritten response to our T.A.C. review letter dated
November 19,2002. There were several issues raised including:
. Request for a Traffic Study.
.. Concern regarding the locatiOll of the Hazel Dell curb cut.
.. Existing Hazel Dell Parkway issues.
.. Drainage - Calculations or Report.
There was a meeting after the November T.A.C. that included the Schneider Corporation, the City
Engineer and the Department of Community Services. If these issues were discussed and there
was resolution of the concerns, please provide written response/comments.
2. Is there a retaining wall indicated at the southwest comer of the detention (labeled retention pond)
pond? Does the line, with clusters of 3-small circles, around the pond indicate normal pool
elevation? The drawing is confusing regarding the banks of the pond ending at the southwest
comer of the pond. Also, the primary plat is confusing in this area.
3. It appears that the interior streets/drives within the development will be private. Therefore the
starn) sewer/stom1 drainage system will be private.
4. Culverts under drives (l46'h Street) in right of way? Provide additional information on drainage.
5. Label and dimension all right of way lines.
6. Based upon the legal description of 11.09 acres, Availability (acreage) Fees for this project would
be:
Water 11.09A @ $l,OIO.OO/Acre = $11,201.00
Sanitary Sewers 11.09A (iV, $1,365.00/Acre = $15,138.00
Total 11.09A @ S2,375.00/Acre = $26,339.00
Connection Fee calculation \vould be based upon the intended use of the building or buildings to
be constructed on the site.
7. The area map on the Title Sheet should read <, Hazel Dell Parkway", not "Hazeldell".

Soils Information:
Brookston silty clay loam- this soil is dark colored, silty in texture and on depressional uplands.
It is deep and very poorly drained with moderate permeability. It has high available water for
plant growth and high organic matter content. It has compact till starting at a depth of 40 to 60
inches. The main soil features that affect the urban development uses are seasonal high water
table, high potential frost action, moderate shrink-swell potential, moderate permeability and
ponded surface water.
Crosby silt loam, 0-3 percent slopes- this soil is light colored, silty in texture and on sloping
uplands. It is deep and somewhat poorly drained with slow permeability. It has high available
water for plant growth and medium organic matter content. The soil has compact till starting at a
depth between 20 -40 inches. The main soil features that affect urban development uses are
seasonal high water table, moderate shrink-swell potential, high potential frost action and slow
permeability. .
Miami silt loam, 2-6 percent slopes- this soil is light colored, silty in texture and on sloping
uplands. It is deep and well drained with moderate permeability. It has moderate available water
for plant growth and a medium organic matter content. It has compact till starting at a depth
between 20- 40 inches. The main soil features that adversely affect urban development uses are
moderate potential frost action, moderate shrink-swell potential, moderately slow permeability,
low strength and erosion during construction.

Ockley silt loam, O~2 % slopes- This nearly level, deep well drained soil is mainly on brpad
terraces. Permeability and available water capacity is moderate. This soil is suitable for urban
development. Features that adversely affect engineering uses of this soil are moderate frost
action, moderate shrink-swell potential, moderate permeability in the subsoil and very rapid
penneability in the under lying material.
Westland silty clay loam - This nearly level, deep, very poorly drained soil is in depressions,
swales, and narrow drainageways on outwash plains. Runoff from higher adjacent soils is
ponded on this soil. Permeability is slow. The water table is commonly at tbe surface or is at a
depth of less than one foot in winter and early spring. Available water capacity is high. Surface
runoff is ponded or is very slow. The main soil features that adversely affect engineering uses of
this soil are a seasonal high water table, high potential frost action, moderate shrink-swell
potential, and slow permeability. The base material for roads needs to be replaced or
strengthened with suitable material.
Additional information about soils is available in the "Soil Survey of Hamilton County". The engineer and/or
developer should consider the need for an onsite soils investigation.
I have reviewed the plans for this project and have the following comments:
e Improvements are needed to the existing drainage way at the southwest corner. An
improved channel or pipe needs to connect the existing 53"x34"RCP to tbe improved
channel that exists on the church property.
o Existing wells need to be capped properly and according to local, county and state
regulations.
., The erosion control plan will be reviewed with the construction plans.

and has approved the plans subject to the following:
1. Are any of the proposed buildings to be sprinklered buildings? If so a meeting will need to be set
up to discuss the location of the Fire Department Connection. We would also request an exterior
access door directly to the riser room.
2. Our office would like to request the addition of two fire hydrants on the site. One located at the
entrance of the service road on the north island and the second on at the north east corner of the
Carepoint Immediate Medical Center.
3. What IS the height of the buildings and will either building have a basement?
4. If the building(s) is provided with a fire alarm system, we request that the fire alarm enunciator
panel be located at the main entrance of each building(s).
5. We are requesting that a Knox Box be installed on each building for fire department emergency
access.
6.
Please respond to the above noted condition(s) in writing and submit to our office prior to the
scheduled Technical Advisory Meeting for this project.

We have reviewed the drawing submitted for this project and offer the following comments:
GENERAL INFORMATION
1. The project site is cUlTently located outside of current City of Carmel Corporate Limits. However, the
area is being proposed for annexation via Annexation Ordinance C223. Tius annexation should be
approved and recorded in early 2003. Based upon annexation, the following jurisdictions will apply.
2. Jurisdictions:
II Street and Right of Way - City ofCarmeJ/Hazel Dell Parkway.
Hamilton County Highway/146th Street
.. Water, Sanitary Sewer, Storm Sewer - City of Carmel.
3. Board of Public Works and Safety Approval Requirements:
II Water and Sanitary Sewer Availability.
II Commercial Curb Cut. Please provide an exhibit that shows the location of the curb cut and
acell/decellanes if applicable; appropriate dimensions including drive width, overall width at the
street, acell/decel dimensions, radii, etc.; and location of existing opposing curb cuts, if applicable.
The exhibit should be 8 Y:z x 11 or of a size that can be reduced to 8 Y:z x 11 such as 8 Y:z x 14 or 11
x 17. Hanlilton County Highw'ay will have jurisdiction over any curb cuts on 146m Street. Carmel
Board of Pubhc Works will have jurisdiction over any curb cuts on Hazel Dell Parkway.
II Temporary Construction Entrance. Same requirements as for the Commercial Curb Cut.
II Any permanent improvement to be installed within dedicated right of way.
o Consent to Encroach Agreements for any permanent improvemcnt to be installed within dedicated
easements. If an irrigation system is planned for this project, additional Water and Sewer
Availability approval will be required from the Board.

and $795.00 per EDU for sanitary sewer. We will eventually require detailed data regarding
proposed use ofthe buildings, square footage, etc.
I am enclosing copies of our Average User Factors for Water and Sanitary Se\vers and our current
fee schedule. I have also highlighted the Average User Factors that might apply to this
development (Medical Office and Immediate Care Center).
Note: AllY deviation from the Connection Fee calculations listed ill our Ordinance will require
Carmel Utilities and Board of Public Works and Safety approvals.
5. TAC Review/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in \...-riting and
be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for
conshllction until all issues have been resolved. All Carmel Utility issues must also re resolved. The
design engineer must cedify all drawings submitted for final approvaL This office will require five-
sets of approved drawings after all issues have been resolved. The drawings will be stamped approved
and signed by both the clty Engineer and Director Carmel Utilities. The Developer/Owner will
receive onc-set which is to be maintained on the conshllction site at all times. Cannel Utilities will
receive two-sets. Our Public Works Inspector wiIl receive one-set and one-set will be maintained in
the Department of Engineering. Ifmore than five-sets are requested to be approved, the additional sets
must be submitted with the five required sets.
G. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
easements will require a Consent to Encroach Agreement between the Owner/Developer and the City
of Cannel. This Agreement requires Board of Public Works and Safety approval. Installations within
dedicated right of way require separate Board approval.
7. Carmel Utilitics does not subscribe to "Holey Moley" and should be contacted directly for all water
main and sanitary sewer main locations.
8. Carolel Utilities will provide a separate review of this project for water and sanitary sewer issues.
9. I am including copies ofthe following with this correspondence:
o Commercial Project Approval Procedures.
o Performance Release Procedure.
o Permit Data, Contacts, etc.
G Commercial Permitting Procedures.
J will send a copy of our Residential Subdivision and Corrunercial Irrigation System Requirements

BONDING REQUIREMENTS
10. Upon initial review, it appears the following bonding requirements may apply to this project:
Performance Guarantees
o Strects (Curb Cut)
o Monument & Markers (if a platted development).
.. Sidewalk/Path (in right of way)
Please provide individual, detailed certified Engineers Estimates for 100% of the cost of labor and
materials for the above improvements. Post individual Perfonnance Guarantees in the amount of the
Engineer's Estimates. Three-year Maintenance Guarantees will be required upon release of the

Street CutfRight of Way l)ermit and Bonding Requirements
Any work in dedicated right of way \vill require a Street Cut/Right of 'lNay Permit and a License and
PenTlit Bond The bond amount will be dcterm.ined by the number of instances of work in the right of
way (curb cut, storm sewer work, etc.) @ $2,000.00 per instance.
PROJECT COMMENTS
11. Please provide drainage calculations. We reserve the oPPoltuni!y for fUlther review and submission of
comments regarding drainage upon receipt of such calculations.

12. It is our assumption that the storm sewer system, with the exception ofright of way structures and
pipe, will be private.
13. Has there been a Traffic Study completed. Please share a copy with this department.
14. 146th Street access, curb cut, median cut and other right of way issues are under the jurisdiction of the
Hamilton County Highway Department.
15. The curb cut on Hazel Dell Parkway is under the jurisdiction of the City ofCarmcl Board of Public
Works and Safety. We will require additional information prior to giving final approval regarding the
drive on Hazel Dell:
" Copy of Traffic Study.
" Show existing pavement markings on Hazel Dcll Parkw'ay.
" Show the existing Hazel Dell Christian Church curb cut on the drawings.
" How many lanes wide at Hazel Dell.
" We are concerned with the proximity ofthe drive to the intersection of 146th Street and Hazel Dell
Parkway. We would request alternative locations be provided or perhaps a shared drive
arrangement with the adjacent Hazel Dell Christian Church be pursued.
16. What is the "Future Cross Access" along south border?
17. The storm sewer connection to the Ashmore Trace subdivision will require the submission of drainage
calculations and may possibly require Board of Public Works and Safety approval since the area has
not been fOffi1ally annexed.
18. Show, label and dimension the Hazel Dell Parkway on the drawings.
19. Show, label and dimension the Landscape Easement along Hazel Dell Parkway.
We reserve the right to provide additional review and comments upon receipt of detailed construction
drawings and drainage calculations.
The above comments are based upon the Engineering Depa11ment's initial review. Please provide our
office with a follow up letter and revised plans, indicating all revisions. A second review will focus only
on revised items. It is critical that our office be made aware of all modifications made on the plans being
re-submitted, particularly if any such changes are considered "new" or fall outside of our initial review. If
