FINAL PROJECT COMMENTS-CONSTRUCTION PLANS
1. The drive at the northeast corner of the site should show connection to the approved Providence
Phase II plan. The Phase II approved plan in this area needs to be incorporated into the Meridian
Music plans.
2. Please incorporate the Old Meridian Street. construction plans into the Meridian Music plans for
that portion of Old Meridian contiguous lo Meridian Music. These plans may be obtained
electronically from Dan Isaacs, Butler Fairman and Seufert Engineers at (317) 713-4615.
3. Have the sidewalk/path plans for this development been approved by the Department of
Community Services in accordance with the Old Meridian Districtsidewalklmulti-use path plans?
Has there been a discussion regarding the construction of this sidewalk/path with the Old Meridian
road project and placing construction costs for the portion of the sidewalklpath adjacent to this site
into an escrow account?

The technical review and comments are intended to evaluate the completeness of the erosion and sediment control plan for the
project. The erosion and sediment control plan submitted Mias not reviewed for the adequacy of the engineering design. All
practices included in the plan, as well as those recommended in the cominents should be evaluated as to their(easibility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state,
orfederal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control phm has been reviewed and it has been determined that the plan:
X Satisfies the minimum requirements and intent Of 327IAC 15-5 (Rule 5). Notification will be forwarded to the
Indiana Department of .Environ mental Management.
Proper implementation of the erosion and sediment control plan and inspections ()lthe construction site by the developer or a
representative are necessary to minimize offsite sedimentation. 771e developer should he aware that unforeseen construction
activities and weather conditions might affect the performance of a practice or the erosion and sediment control plan. The plan
must be aflexible document, with provisions to modifY or substitute practices as necessary.

Note: All erosion and sediment control measuresslwwn or' the plans and referenced in this review must meet the design
criteria, standards, and specifications outlined in the "Indiana Ham/book for ErOS;0I.1 Control in Developing Areas" from the
Indiana Department of Natural Resources, Divi.'.ion of Soil Conservation or similar Guidance Documents.
Attached is a draft construction sequence for small commercial sites. I try to stress establishing the stone lot
before the building construction starts and seeding the disturbed areas while tile building is being
constructed. Final grading, landscaping and seeding are completed at the end of the project.
Erosion and Sediment Control Sequence for Typical Commercial Construction Site
1. Before construction, evaluate, mark; and protect important trees and associated rooting zones, unique
areas (e.g., wetlands) to be preserved, and vegetation suitable for filter strips
2. Install construction entrance
3. Install perimeter BMPs
. Provide earthmoving instructions that compliment erosion and sediment control pLan
4. Start mass earthwork
Establish stone entrance and staging area
Start building construclion
Finish grade and seed basins, slopes, swaks and mounds
Temporary seed all.disturbed areas
5. Install storm and sanitary sewer and utilities
6. Install inlet protection as storm inlets are completed
7. Complete parking lots, roadways and curbs
8. Temporary seed all disturbed areas
9. CompLete buiLding
10. Final grade and Landscaping
11. Complete fmal seeding
12. Maintain all erosion and sediment control practices until all disturbed areas are permanently
stabilized.

This is a follow-up to my phone meassage to you earlier today.
At around 12:30 PM today, my Client informed me that we will be reducing
the size of the Building, which will possibly change the Site Layout and
facades. I think it best that we table this Project scheduled for this
evening until we understand the magnitude of the changes.
I respectfully request that the Project not corne before the Plan Commission
and that we be rescheduled, pending revisions to our ADLS Submittal.
I understand that if we decide to modify the Site Plan and building
elevations, we will be required to present the modifications to the Special
Studies Co~~ittee.
I will call you next week to schedule an appointment to discuss the
proposed changes.

comments:
GENERAL INFORMATION
1. The proj ecl site is currently located within current City of Carmel Corporate Limits. The following
jurisdictions will apply.
2. Jurisdictions:
.. Street and Right of Way ~ City of Carulel (Old Meridian Street. The interior streets of Providence
are private streets).
.. Water and Sanitary Sewer - City of Carmel
.. Stoml Sewer ~ City of Carmel (the onsite storm sewers arc assumed to be a private system).
3. Board of Public Works and Safety Approval Requirements:
. Water and Sanitary Sewer Availability.
. Commercial Curb Cut. Does not appear to be applicable since the curb cut is on a private street.
. Temporary Construction Entrance. Same:requirements as for the Commercial Curb Cut. Again,
this does not appear to be applicable since the temporary entrance is on a private street.
. Any permanent improvement. to be installed within dedicated right of way.
.. Consent to Encroach Agreements for any permanent improvement to be installed within dedicated
easements and/or dedicated right of way. If an irrigation system is planned for this project,
additional Water Availability approval will be required from the Board and additional Water
Connection Fees will be due.

upon the retail classification from the average user factor tables. This classification and
calculation is based upon 1.0 EOU per 1,000 square feet. Please provide this office with the total
square footage of this building, in order that Connection Fees may be calculated. When this
project was presented to the Technical Advisory Cormnittee for review, the petitioners stated that
this is a proposed 2-story, 20,000 square foot retail building. Based upon our retail classification,
20,000 square feet @ 1.0 EDU/1 ,000 square feet equals 20.0 EDU's. The following Connection
Fees would be applicable:
Water 20.0 EDU's@ $1,310.00/EOU = $ 26,200.00
Sanitary Sewers 20.0 EDU's @ $ 795.00/EOU = $ 15.900.00
Total Connection 20.0 EDU's @ $2,lOS.00/EOU = $ 42,100.00
I am enc losing copies of our Average User Factors for Water and Sanitary Sewers and our current
fee schedule. I have also highlighted the Average UsefFactors that might apply to this
development.
Note: Any deviation/rom the Connection Fee calculations listed brour Ordinance will require
documentation and Carmel Utilities and Board of Public Works and Safety approvals.
5. T AC ReviewlDrawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing and
be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for
construction until all issues have been resolved. All Carmel Utility issues must also re resolved. The
design engineer must certify all draw'ings submitted for final approva1. This office will require a
minimum of five-sets (maximum of seven-sets) of approved drawings after all issues have been
resolved. The drawings will be stamped approved and signed by both the City Engineer and Director
Carmel Utilities. The Developer/Ov,rnenvill receive one-set which is tobe maintained on the
construction site at all times. Carmel Utilities will receive two-sets. Otlr Public Works Inspector will
receive one-set and one-set will be maintained in the Department of Engineering. If more than the
minimum five-sets are requested to be approved, the additional sets (maximum total of two additional)
must be submitted with the five required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
easements and/or right of way will require a Consent to Encroach Agreement between the
Owner/Developer and the'City of CarmeL This Agreement requires Board of Public Works and Safety
approval. Installations within dedicated right of way require separate Board approval. A copy of our
Residential Subdivision and CorrnnercialIrrigation System Requirements are available upon request.
7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all water
main and sanitary sewer main locations.
8. Carmel Utilities will provide a separate review of this project for water and sanitary sewer issues.

BONDING REQUIREMENTS
10. Upon initial review, it appears the following bonding requirements may apply to this project:

Please provide individual, detailed (materials; quantities, sizes, unit costs, etc.) certified Engineers
Estimates for 100% of the cost of labor and materials for the above improvement. Post a Performance
Guarantee in the amount of the Engineer's Estimate. A three-year Maintenance Guarantees, in the

amount of 10% of the Performance Amount, will be required upon release of the Performance
Guarantee.
Street CutfRight of Way Permit and Bonding Requirements
Any work in the dedicated right of way of Old Meridian Street will require a Right of Way Permit and
a License and Permit Bond. The bond amount will be determined by the number of instances of work
in the right of way (curb cut, storm sewer work, etc.) @ $2,000.00 per instance. Open pavement cuts
in dedicated streets require approval by our Board of Public Works and Safety. It appears thatthe
water and gas service lines will require permits and bonding. Please contact our Right of Way
Manager, Fred Glaser, to arrange permitting and bonding.
PROJECT COMMENTS-CONSTRUCTION PLANS
11. Please provide a set of drainage calculations for this project. Was this area included in the overall
drainage plan for the Providence at Old Meridian?
12. Please dimension, identify and label the right of way line of Old Meridian Street on all sheets.
13. Sheet C20 I indicates a pOliion of a future building encroaching upon this lot and plat. Is that an
acceptable condition?
14. Is thc building tobe constmcted on the existing right of way line of Old Meridian?
15. On Sheet C20 I, what do the lines at the southwest corner of the building represent, steps?
16. We reserve the opportunity for fmiher review and submission of corrunents regarding drainage and the
plans upon receipt of drainage calculations and review of the calculations and plans by the City
Engineer.
PROJECT COMMENTS-SECONDARY PLAT
17. Normally, concrete monuments are required at the perimeter corners of plats. Has this requirement
been waived for this project?
18. Are there no requirements for easements on the lot?
The above comments are based upon the Engineering Department's initial review. Please provide our
office with a follow up letter regarding this review and revised plans, indicating all revisions. A second
review will focus only on detailed construction plans. It is critical that' our office be made aware of all
modifications made on the plans being re-submitted, particularly if any such changes are considered "new"

Note: All erosion and sediment control mea,'iures shown on the plans and referenced in t/lis review must meet the design
criteria, standards, and specifications outlined in the "Indiana Handbookfor Erosion Control in Developing Arelrs"from the
Indiana Department of Natural Resource,'i, Division of Soil Conservation or ,\"imilar Guidance Documents.
Note: This plan must meet the requirements of Rule 5 because it is part of a larger common plan of
development.
Please address items marked no in sections one through three on page two.
4A. A construction sequence is needed before the p1<m is approved.
4C. Recommend the silt fence be shown on the nOlihwest side of the property parallel with Old Meridian
Street. Is the existing swale along Old Meridian Street in good shape? Are there any plans to disturb it?
Comment- Should you extend the 15" storm sewer north to the property line to facilitate drainage for the
future project?
Note: IDEM will be notified when the plans are approved.

Indiana Development Training Center----No comments
Meridian North Medicaln-- The address far this property yvill be off , whatwiill be corrected as W Smokey Row Rd(not W
136th St). It will not get a Meridian 8t address. (Ask Laurence for c1arifiaction on 1his...Public Safety will not allow it to be off
of Meridian 5t unless there is access off of there). The Medical office next door was addressed incorrectly when it was
assigned a long time ago. I'm not going to continue addressing incorrectly, just because someone else did it that way. I
believe Adam was advised of an address a couple years ago on this. The only change..is it should be W Smokey Row Rd,
not W 136th St.(as mentioned above).
Mark Swanson Building---Will have to get back with them on address...need to check to make sure the address they say
works in that area.
Town Pointe----I will need a street name for "A" and "B"(stub street). I would like to see a monumnet sign put up as you
leave Village of West Clay and enter this neighborhood(President Street). Easier for Public Safety to find it.
Wexley Chase----working on addresses now--would like to see Monument signs seperating the two developments they
border with(Aberdeen Bend & Hayden Run).
Ballantrae----Need 1 street name
Eagle Ridge----Revised addresses completed---lf Dave has not received them..please let me know and [ can get him the
list.

The undersigned has reviewed the proposed plans for
Meridian Music
and has approved the plans subject to the following:
1, Is the proposed structure to be protected by an automatic fire sprinkler system? If so, a meeting
will need to be scheduled with our office to discuss the location of the fire department
connection. We will also request an exterior access to the sprinkler riser room.
2. What is the height of the structure and will the structure contain a basement?
3. We are requested the purchase of a Knox box for the building for fire department emergency
access, If a sprinkler system is required, we also will require fire department connection caps
also available through the Knox Company, An application for Knox products ca be obtained
through our office.
