This evaluation is intended to assess the level of compliance with 327 lAC J 5-5 (Rule 5). It is also intended to identijjl areas where
additional measures may be required to control erosion and sedimentation. AIlpractices recommended in this report should he
evaluated as to their feasibility hy a qualified individual with structural practices designed by a qualified engineer.

practices included in the plan, as well as those recommended in the comments should be evaluated as to theirfeasibility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state,
or federal permits that may he required to proceed with thisproject. Additional information, including design calculations may
be requested tofiJrther evaluate the erosion and sediment control plan.
The erosion and sediment control plan has been reviewed and it has been determined that the plan:

This letter serves to ackr'lowledge receipt of a transmittal received 4/13/03 containing the
plans for the above-mentioned project. After reviewing the plans the Highway
Department has the 'followIng comments (I have maintained the numberlng from the
February 25, 2003 and March 31 r 2003 letters):

4C. The proposed sediment basin does not have an outlet or enough construction information to build it.
Please show depth, contours and outlet information. Storage above the permanent pool level is used to
calculate the volume required for the sediment basin. Calculations are needed. The sediment basin detail is
not appropriate. This detail looks like an energy dissipater. Please provide grades for the temporary
diversion swale.
References to the Soil ConservatioIl Service in the construction sequence should be changed to the Hamilton
County Soil and Water Conservation District.

should include a pavement cross~section and layout of the accel/decellanes as well
as the dimensions for the passing blister on the north side of 141 Sl Street Other
things which will need to be are the pavement cross section for 141$1 Street (12"
stone base, 4" HMA Intermediate course and' 1" HMA surface course), lengths of the
tapers and lanes and a striping plan for the passing blister plus any other pertinent

The monuments and markers should be asrequired under Section 8.2 of the SCO.

Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have
been made and comments addressed, please produce a Mylar copy for signature by this Department.
Two paper copies of the plat must be filed with the Mylar prior to recording, and three paper copies and a
Mylar of the recorded plat must be submitted following recording. OUf approval of the secondary plat
(signature) will be forthcoming upon revision of the plans according to the items listed above.

This letter serves to acknowledge receipt of a transmittal received 2f6/03 containing the
plans for the above-mentioned project. After revIewing the plans the Highway
Department has the following comments:
1. Before any approvals are granted for the secondary plat, performance bonds must
be submitted for 100% of the construction cost of the following items: Stone Base.
HAC Binder, HAC surface, Concrete Curbs and Street Name and Regulatory Signs.
2. Note on the Site Development Plan that connections to existing pavement and curb
must be saw cut.
3. Is the profile for Street B shown on sheet [) correct? It does not appear to match the
Intersection detail sheet.
4. Please provide a street profile for the stub street at the southwest comer of the
section.
5. On Sheet 9, a 25 MPHsIgn is needed north of Street B. A legend should also be
included Indicating each sign type and the number of signs needed for each sign
type.
6. On the water distribution sheets, the following notes need to be InCluded: No water
main valves will be permitted between the curbs, and Full depth granular l:lackflllls
required at all street crossings.

This letter serves to acknowledge receipt of a transmittal received 216/03 containing the
plans for the above-mentioned project. After reviewing the plans the Highway
Department has the following comments:
1. Before any approvals are granted far the secondary plat, performance bonds must
be submitted for 100% of the construction cost of the followtng items: Stone Base,
HAC Binder, HAC surface, Concrete Curbs and Street Name and Regulatory Signs.
2. Please prepare a detail of the entrance into The Ridge at Hayden Run. The detail
should include a pavement cros5sectlan and layout of the accelldecellanes as well
as the dimensions for the passing blister on the north side of 141 in Street. Other
things which wHl need to be are 'the pavement cross section for 141~' Street (12"
stone base. 4" HMA Intermediate course and 1" HMA surface course), lengths of the
tapers and lanes and a striping plan for the passing blister plus any other pertinent
information.
3. 14191 Street will need to be resurfaced aoross the frontage of The Ridge at Hayden
Run.
4. The Highway Department is requiring that the widening of the eastbound lane of
141'1 Street be continued ~cro$$ the Boone property. The addItional right of way will
need to be acquired to make the improvements.
5. Concrete ADA ramps will be required at the street crossing at the entrance.
6. Road cut permits will be required for the installation of the 12~ RCP under 14191
Street. and the removal of the existing 16" CMP~
7. please verify that the trees located in the south of the entrance median will not
interfere with sight distance at the intersection.
S. the trees shown on the east side of Street A must be located outside of the right of
way.
9. the note for pavement marking replacement on the entrance plan must say 141et
Street, not Towne Road.
10. a 3' stone shoulder is needed along the widening. passing blister and acceVdecel
lanes.

I would iike the plans to show the dimensions and elevations for an appropriately sized sediment basin
in the event the ground is too wet to build the full lake. Please provide calculations. A diversion is
needed along the east side to divert storm water into lake #2. The construction sequence needs to
incorporate this information. In the sequence all disturbed areas need to be seeded after rough grading is
completed.
Note: A specification for dewatering would be appropriate.
