By Ordinance, our Department must periodically review- building permit files for compliance. We
find the above referenced project has not been issued a Certificate of Occupancy, (C/O) which is
required by local ordinance, and states that the project complies with all building codes and local
zoning regulations.' This Certificate was not issued because:
The required Final Site Inspection by Urban Forestry at the completion of the project was not
scheduled, as required. Our records also indicate $96.25 due for re-inspection fees. Inspection
procedures were provided when this hudlding permit was issued.
Please contact our office between 8:00 a.m. and 5:00 p.m. at: (317) 571-2444. We will be glad to
assist you in bringing this project into compliance and to answer any questions you may have. In
most instances, these inspections will have to be coordinated with the owner or tenant. Late fees
may be assessed to you, the builder, if we find this construction is being used or occupied.'' If we do
not hear from you within 30 days from the date of this letter, or October 24, 2007, our office will
not issue any further permits to you, your firm, or any individual representing your firm until all
permits, inspections, and Certificates of Occupancy have been issued and all fees paid.'` This
project file may be turned over to the City Attorney.

We apologize for issuing you the certified letter regarding an outstanding site inspection
by Urban Forestry, for your firm's permit for the community pool at the Cherry Creek
Estates. It was only noted in a small notation area that we were not requiring this
inspection (as we typically do) for your permit. There remains an outstanding re-
inspection fee balance of $96.25, for the first final inspection was failed, on 5/26/05. Once
this has been paid, the full Certificate of occupancy can be issued.
Additionally, it has come to our attention that there also remains a balance on the permit
your firm pulled for the community pool for the Lakeside Park subdivision. $93.50 is due
for a failed bonding Est grounding inspection which occurred on 7/7/04. A copy of the C/O
can be issued to you when this paid.

SWIMMING POOL INSPECTIONS
All pool construction requires AT LEAST TWO (2).inspections.
Larger or specially constructed pools may require additional
inspections. For most inspections; a minimum 24-hour notice is
required.
First Inspection: Bonding and Grounding. Pool builders may use
the "Footing Line" (571-2449) between 8:00 and 9:00 to schedule
bonding inspection for SAME DAY INSPECTION. For
scheduling inspections further out in time, call the Permit Services
office at 571-2444. FOR THIS INSPECTION: The hole needs to
be excavated, and plumbing and electrical installed; Corners of the
pool deck must be marked, preferably staked. A maior aspect of
the bonding inspection is the common bonding grid, as called for
in the National Electrical Code; Section 680-22.
Second Inspection: Final inspection. This inspection is
scheduled with the Permit Services office (571-2444) once the
project is complete and all grading and seeding is done. This
inspection is to verify ALL aspects of Indiana State Law as it
appears in the Indiana Swimming Pool Code; Second Edition,

POOL BUILDERS WILL BE RESPONSIBLE FOR: -,?o
Notification to pool owners that these inspections are to be made,
and also will relate the various codes applicable. Pool builders
shall coordinate with owners for inspector's access to site on days
inspections are scheduled.
Under Carmel / Clay Zoning Ordinance Amendment Z-289,
Chapter 29. 0, Sea 29.6.8: Late fees may be assessed if request
for inspections are not called in and completed by our inspectors
at the proper time.
