Aineri'vest landscape plan changes
Dear Chuck:
On August 12, 2003, I made a [mal site inspection for Keystone Office Park, Building 5
(Amerivest), for the landscaping and parking lot. There were substantial differences
between the approved plan and what was installed. The original landscape plan (docket
no. 86-02 ADLS/DP) was approved by the Carmel/Clay Plan Commission on July 16,
2002.
You may either file an ADLS/DP amendment with the Department of Community
Services or make changes to bring this building into compliance with the original plan
approved by the Carmel/Clay Plan Commission. To file an ADLS/DP amendment, you
may contact Jon Dobosiewicz of the planning department at (317) 571-2417.
The list of substantial changes that have been made are as follows:
Ornamental pear trees have been substituted in front of the building.
The size ofthe shrubs is listed as twenty- four inches; most are ten to fourteen
inches.
The plan shows hawthorn trees near the parking lot entrance. There are
currently large white pines existing there. The existing vegetation should be
kept.
There is a difference of seven total spruce trees between the plan and what
was actually installed on the north perimeter (need 7 more planted).
Serviceberry trees were substituted for dogwood trees.
The east side ofthe building (rear) is shown on the plan as a twenty-five foot
access easement with the remainder to be preserved in native vegetation. This
has changed substantially with the addition of curvilinear sidewalk and patios.
The foundation plantings shown on the approved plan are substantially
different than what was actually planted.

Several existing white pines located in the east entrance of the complex were
removed recently (August 12,2003). All existing trees and shrubs that are in
healthy condition are to be kept and not removed. Vegetation that is
unhealthy or dead and must be removed must be replaced with a similar
speCIes.

contact Sarah Lillard of the Building Permits Department at (317) 571-2444.
A commercial re-inspection fee of $90.00 will be assessed. You may contact me at the
time you wish to set up another site inspection for the final Certificate of Occupancy.
