The technical review and comments are intended to evaluate the completeness q[ the erosion and sediment control plan for the
project. The erosion and sediment control plan submitted was not reviewed for the adequacy of the engineering design All
practices included in the plan, as well as those recommended in the comments should be evaluated as to their feasihility by a
qualified individual with structural practices designed by a qualified engineer. The plan has not been reviewedfor local, state,
or federal permits that may be required to proceed with this project. Additional in[ormalion, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control plan has been reviewed and it has been determined that the plan:
Satisfies the minimum requirements and intent of 327IAC 15-5 (Rule 5). Notification will be forwarded to the
Indiana Department of Environ mental Management.
X Docs not satisfy the minimum requirements and intent of 327IAC ]5-5 (Rule 5); deficiencies are noted in the
checklist and in the comments section. Deficiencies constitute potential violations of the rule and must be
adequately addressed for compliance. The information necessary to satisfy the deficiencies must be submitted:
Proper implementation of the erosion and sediment control plan and inspections q[ the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction

Note: All erosion and sediment cOlltrol measures shown on tire plan.~ and referenced in this review must meet tire design
criteria, standard.." and specifications outlined in the "Indiana Handbook for Erosion Control in Developing Areas" from the
Indiana Department o.lNatural Resources, Division of Soil Conservation or similar Guidance Documents.
Items checked no in sections 1,2, and 3 on page 2 need to be addressed. Many of these items can be
addressed in tabular or narrative form.
4C. A temporary swale/diversion or other practice is recommended along the west property line. Silt fence is
not adequate by itself.
4E. A temporary slotted riser is the preferred temporary outlet of the dry basin. However, a rock dam could
work. The existing detail for the rock dam will hold water instead of slowly draining the water out of the
basin. A typical detail shows the dam made from riprap or #28 and the face of the dam covered with 12" of
#8 stone to help filter and slow the flow. Provide calculations that the sediment basin provides the required
storage- 1800 cu. ftj acre of disturbed area and the stone weir has the correct length to pass the potential
now.
Note: This site is less than 5 acres but is part of a larger common plan of development. The developer will
need to submit an NOI to IDEM.
Note: IDEM will be notified when this plan is approved.

1. No landscape materials in the planting strip on the east side of the
building. The US 31 Overlay Ordinance Section 23B.I0.2 requires landscaping
in the planting strip by your parking lot and along the foundation (2 Sbade
trees, 1 Ornamental tree, and then at least 10 shrubs per 100 linear feet

4C. A temporary swale/diversion or other practice is recommended along the west property line. Silt fence is
not adequate by itself.
4E. A temporary slotted riser is the preferred temporary outlet of the dry basin. However, a rock dam could
work. The existing detail for the rock dam will hold water instead of slowly draining the water out of the
basin. A typical detail shows the dam made from riprap or #2s and the face of the dam covered with 12" of
#8 stone to help filter and slow the flow. Provide calculations that the sediment basin provides the required
storage- 1800 cu. ft./ acre of disturbed area and the stone weir has the correct length to pass the potential
flow.

inspector. You will also need to have an approved stamped set of prints before starting any construction.
Tracer wire must be installed with all water main laid in Carmel.

PROJECT NARRATIVE & REQUEST FOR ADDRESS
This letter is in response to your request In regard to the current and future addressing of both lots
in the proposed MERIDIAN NORTH MEDICAL subdivision. CurrentlY. the 2.349-Acre sIte is
unoccupied and an existing medfcal building lies to the east of the site (13590 North Meridian
Street).
We propose to install a 21,110 sq. ft. two-story medical office building on the vacant sIte. We
expect the structure to have 3-5 professIonal or medical office tenants. Access IS provided at the
existing entrance to the Beeson SurgeI)' Center Northeast of the proposed building. This existing
entrance is a common ingressJegress entrance for both properties. We are requesting that the
address for the proposed building be 13590 North Meridian Street, Building 8 per t!"lS conversation
at TAC.
Once the City of Carmel installs the future Illinois street extension the eXisting entrance will be
vacated and ,beth properties will have a new entry drive onto Illinois Street. As a part of the
primary and secondary plat approval we are requesting a future address for both buildings on
illinois Street Beth buildings will adopt the illinois Street address at the time of the vacation of the
136lt1 Street entrance. This will be shown on the Secondary Plat, similar to a comer lot in a
residential subdivision.

us 31 OVERLA Y ORDINANCE Section 238.7 does not preclude the sale or transfer of .any parcel
of land within a parent tract after the approval of 8 Development Plan for the entire tract.
However, the development oltha p8rceJ must still conform to the DP for the parent tract as
approved or amended by the Commission, and all other appficabre requirements contained in the
Zoning Ordinance.
McCordsville Partners, LLC purchased the 2.349-Acre site from K1ack Associates, Inc. per
Warranty Deed recorded on June 28. 2000.

Indiana Department ofNt/tura/Resources, Division of Soil Conservation or similar Guidance Documents.
Items checked no in sections 1,2, and 3 on page 2 need to be addressed. Many of these items can be
addressed in tabular or narrative form.
4A. In Phase II erosion control sequence instructions, please change permanent seeding to temporary
seedi ng in item # 1. Provide temporary seeding after utilities and stonn sewer are installed in item #2.
4C. A temporary swale/diversion or other practice is recommended along the west property line. Silt fence is
not adequate by itself.
4E. A temporary slotted riser is the preferred temporary outlet of the dry basin. However, a rock dam could
work. The existing detail for the rock dam will hold water instead of slowly draining the water out of the
basin. A typical detail shows the dam made from riprap or #2s and the face of the dam covered with] 2" of
#8 stone to help filter and slow the flow. Provide calculations that the sediment basin provides the required
storage- 1 SOO cu. ft.! acre of disturbed area and the stone weir has the correct length to pass the potential
flow.
Note: This site is less than 5 acres but is part of a larger common plan of development. The developer will
need to submit an Nor to IDEM.

COMMITMENTS CONCERNING THE USE
AND DEVELOPMENT OF REAL ESTATE
WILLIAM BEESON, M.D., the contract purchaser of the Real Estate, (hereafter,
"Owner"), make the following Commitments to the Carmel Plan Commission (hereafter,
"Commission").
1. DescriptWn of Real Estate:
See Exhibit "A" ("Real Estate").
2. Docket No.: 60-96-DPIADLS
3. Statement of Commitments
A. Owner commits to make application to the State of Indiana for permission
to remove the existing fence adjacent to the right-of-way of U. S. 31 prior
to the issuance of a Certificate of Occupancy for the building to be
constructed on the Real Estate;
B. Owner further commits to seek the approval of the appropriate
governmental authorities for the closure of the entrance to the Real Estate
from 136th Street upon the extension of Oakridge Road south of 136th
Street (Oakridge Road Extension), by persons or entities other than the
Owner, and all approvals and rights-of-way being provided to Owner to
permit access to the Real Estate to and from the Oakridge Road Extension.
4. Binding on Successors and Assigns. These Commitments are binding on the
Owner of the Real Estate, each subsequent Owner of the Real Estate, and each other person
acquiring an interest in the Real Estate, unless modified or terminated by the Commission. These
commitments may be modified or terminated only by a decision of the Commission made at a
public hearing after notice as provided by the rules of the Commission.
5. Effective Date. The Commitments contained herein shall be effective upon the
approval of the applications filed with the Commission under Docket No. 60-96-DP/ADLS and
the issuance of a Certificate of Occupancy for the office building described in the plans approved
by the Commission.
6. RecordinJ(. The undersigned hereby authorizes the Secretary of the Commission
to record these Commitments in the Office of the Recorder of Hamilton County, Indiana, upon
the granting of the approvals referred to in paragraph 5, above.
7. Enforcement. These Commitments may be enforced by the Commission and the
City of Carmel, Indiana.
8. Compliance Confirmation. The Director of the. Department of Community
Services, Carrnelt Indianat shall, when requested by the Owner, give written assurance, in letter
form, that the Owner has or has not complied with these Commitments.
