January 5, 2004 r \ U u u u u u u u u U I J 11 U U !1 U U U U U U 193878_' U Summary of Approvals ReQuested North Meridian Medical Pavilion Metro Acquisitions, LLC, a part of Lauth Property Group, is proposing to develop approximately twelve (12) acres of real estate generally located west of US 31, north of Ritz Charles and within the US 31 overlay zone, having a common address of 12202 North Meridian Street. The proposed project will consist of wo three story buildings, each containing 88,878 square feet, to be constructed of a combination of glass, precast and metal panels, the buildings to be used primarily for medical office and other clinical facilities, with related parking, landscaping and signage. With the sole exception of matters pending herein for consideration, all other necessary land use approvals, public and private, have already been received, including approvals required from the Carmel Plan Commission and the Carmel Common Council to permit the proposed project to proceed. The proposed rezoning, partial plat vacation and amended DP-ADLS approvals now being sought are necessary to allow a portion of Lot 9 in Williams Creek Farms Estates Subdivision to also be included in the proposed project. The portion of Lot 9 proposed to be included re~resents that portion of the Lot which will lie east of Illinois Street as extended from 116 Street to Carmel Drive, along the route recently identified by the City of Carmel. The east part of three other Lots in Williams Creek Farms Subdivision that will also be bisected by Illinois Street extended have already received all necessary approvals, public and private, to become a part of the proposed project. Amended DP and ADPLS approvals are also needed because changes in building signage previously approved are also proposed. Building signage is now proposed only on the east face of the building, toward Meridian Street. Because of the nature of the building/tenant identification signage proposed, a variance will also be needed from applicable development requirements from the Board of Zoning Appeals. Low-level monument signs are proposed on the north side of the southeast driveway entrance and at the south driveway entrance to Illinois Street extended. The one-piece, die-cast aluminum luminaire complements the architectural integrity of virtually any site, Standard design features include IP65 rating which completely seals the Challenger housing from penetration of external contaminants. Designed around a standard 400 Watt and a 1000 Watt reduced envelope lamp size, the Challenger gives you a choice of either horizontal or vertical lamp orientation, plus a variety of lamp types and reflectors to best meet your application. The compact, low profile shape creates lower EPAs, which may allow use of lighter gauge poles. The Challenger is easy to install and to maintain. SPECIFleA TIONS HOUSING One-piece, die-cast aluminum in a multi-radiused, rectangular shape with mounting arm cast in as an integral part of the housing. All hardware is stainless steel or electro-zinc plated steel. DOOR FRAME The Challenger's one-piece, die-cast aluminum door frame secures to the housing with two internal hinges, Two stainless steel captive door fasteners allow easy access. Door frame may then be removed from housing without tools. A one-piece extruded silicone gasket seals the door frame against the housing. LENS/GASKET The Challenger is available with a contoured clear tempered glass lens for vertical lamp optics. A tempered flat clear glass lens or contoured clear tempered glass lens is offered for horizontal lamp optics. A one~piece extruded silicone gasket seals the lens to the die-cast aluminum door frame, SOCKETS Porcelain mogul-base sockets. All sockets are factory prewired with a disconnect plug for the ballast. All sockets are pUlse-rated. LIGHT SOURCES Pulse-Start Metal Halide, Super Metal Halide, Metal Halide, Metal Halide Reduced Envelope, or High Pressure Sodium, Clear lamp is supplied as standard.
