This evaluation i5 intended to assess the level of compliance with 327IAC I S-S (Rule 5). It is also intended to identify areas
where additions! measures may be required !o control erosion and sedimentation. A[[practices recommended in this report
should be evaluated as [o their feasibi[iry by a qualified individual with structural practices designed by a qualified engineer.
All erosion and sediment control measures shall meet the design criteria, standards, and specifrcations outlined in the "/ndiana
Handbook /or Erosion Control in Developing Areas" or similar guidance documents.
Current Site Information:
Majority of the dirt work is completed. Storm sewer is started. Pond slopes have not been stabilized. Grass has been
maintained on much of the site. The Swale on the south end is cut but not

Please find attached recorded your requested copies of the Secondary Plat for the
Village of Mount Carmel, Section Ten; Instrument # 20030051323, PC 3, Slide 178. As
we discussed by a-mail we will proceed to Quit Claim the sliver of property to the
landowner of Lot #23, Section One, in order to `cleanup' the gaps in the subdivision.

~Ve have reviewed the secondary plat and the revised plans submitted for this project and offer the
following comments:
PROJECT COMMENTS-SECONDARY PLAT
Sheet 1
1. The Legend has a symbol for 4" x 4" x 36" concrete monuments. We do not see concrete
monuments indicated on the plat. It is our understanding that these are required.
2. The right of way line of Village Drive should be identified.
3. There is a small sliver of property between this development and the property identified as being
owned by Ralph L. Wilfong, II & Denise M. Wilfong. What is this and what will happen to this
small piece of real estate?

4. In-our opinion, there should be an easement established along the four-perimeter boundaries at the
northwest portion of the plat (Lot 6).
5. There is an easement indicated on March property. Is this easement a part of the plat or is it
existing?

6. Are there any requirements or variances [o be approved regarding Amalfi Drive and the fact it will
no[ be extended?

PROJECT COMMENTS-REVISED CONSTRUCTION PLANS
1. Sheet 3 & 4 -Cannot find clear instmc[ions, detail or dimensions for the pavement widening,
painted median widths, median lengths, etc. at the entrance off of Village Drive. We would like to
see more information on the Entrance Detail such as dimensioning the striped medians, pavement
widths, taper widths and lengths, etc. Please add sufficient details to enable contractor to build per
the design.

2. Sheet 11-Regarding [he Village N. Drive Standard Cross-Section Detail, a portion of Village
Drive will be wider than indicated, due to the atypical widening at the entrance. Please make a
note of this on the detail.
3. Sheet 11-The Typical Section-Local Street Detail should show curb underdtains.
4. Sheet 12 -There is a note on this sheet that must be removed prior to approval of the plans.
The note is as follows:
A'ote: The Engineer and/or Owner disclaim any role in the construction means and methods
associated with dhe project as set forth in these plans.
Since [he engineer is designing the proposed improvements, he/she is responsible for peoviding
the appropriate means and methods for doing so, and would therefore be responsible in this aspect
of design. Additionally, the owner is procuring the contractor doing the work, and is responsible
for seeing that he builds according to these same means and methods as shown on the plans.
Therefore, this note releasing bothfrom having any role in the construction means and methods is
not acceptable.
STORM DRAINAGE CObIMENTS
When this department initially reviewed the plans for this development, it was unclear as to who would
have jurisdictional authority for approval, as the area had not been officially annexed. That being the case,
we provided initial comments. We now understand that this jurisdiction will be shared between the City of
Carmel and the Hamilton County Surveyor's Office, and therefore submit our comments/concerns
regarding the drainage plan. We are also motivated to do so due to the multitude of drainage issues now
being brought to [he City of Carmel for assistance by [he residents of the Mt. Carmel development.
Sheet 3
1. The detention area should be labeled as a "wet" detention basin.
2. Please provide cross-section detail for the detention basin. Will this pond be lined?
3. Between Lots 4 and 5, we request that structure #12 and associated riprap be eliminated, the pipe
extended to the rear of the lot and a beehive structure be installed. This type of structure and rip
rap are not appropriate [o place along side yards of lots, and we receive many complaints/requests
for the city to remove them due to residential complaints of safety and childien playing near them.
4. We would suggest the creation of a high point point along the middle of the north line of Lot 5,
with the front half of the lot draining to Structure 10, which could be a beehive grate. From the
mid point of the north line, a Swale could be created [o drain to the west and back around the rear
of the lot to the beehive inlet as proposed at the rear of Lots 4 and 5, as suggested above in #3.
This translates to just over a 400 foot length of Swale.
5. The mounding as shown proposed to the north of Lot 5 appears to be in part upon private property.
Is there an easement for which this work can legally take place?
6. Please provide spot elevations on the lot lines in the rear of lots 5, 4, 3 and 2 indicating finished,
proposed grades, and flowline Swale elevations.
7. The length for which drainage is expected to travel in swales from the high point in the rear of lo[
4 to the southeast, then northeast is excessively far considering it's ultimate outlet point (currently
proposed Str. #8). We are concerned that this surface flow and the swales will not function
properly when stretched along such distances, and we request additional storm sewer pipes be
provided to serve as outlets for the rear yard drainage. We request [hat [he pipe [o the end section
structure 8 be extended to-the Cum point mid-way along the rear of Lot 2, and a beehive be placed
at this location. Additionally, continue the pipe in a southwesterly direction to a beehive inlet
placed between Lots 2 and 3, such that surface flow by way of rear yard swales is no longer than
600 feet.

8. Suggest a beehive replace end section structure 3, if grades can be made to work. Otherwise, see
note #10 below.

9. Please show the normal pool elevation on the outlet structure #4 profile sheet and that the
upsheam end of the pipe is at the normal pool.

10. If a beehive is not installed in place of end section structure 3, we would suggest slope protection,
grate, etc. be provided due to its slope and proximity to the edge of the road.

criteria, standards, and specifications outlined in the 'Indiana Handbook for Erosion Control in Developing Areas" from the
Indiana Department of Natural Resources, Division of Soil Conservation or similar Guidance Documents.
Please address items checked no in sections 2 & 3.
4A. Recommendations to improve the erosion and sediment control sequence (construction sequence).
• Include in line a. - Installation of perimeter silt fencing, inlet protection for str#1 and strawbale dams
in the roadside Swale.
• line c. - Install storm sewer from str#I to str#4 and place slotted riser as a temporary outlet on str#4.
Construct a sediment basin as shown on the drawing. (approx. 4'x30'x75') 1800 cu ft/ ac storage is
required.
• line d. -Replace dry basin with wet basin. Remove wording about the outlet structure and the
perimeter silt fence. Add temporary seeding of disturbed areas and permanent seeding of the mound.
• Add to line e. -Install inlet protection immediately after the structure is completed. Swales will be
finished graded, blanketed and seeded after the storm sewers aze installed.
• line g. -Change this statement to reflect previous changes.
4G. An inlet protection detail is needed for the flared end sections. Make sure the straw bales or silt fence is
shown to extend up the bank to the top of the pipe.
4J. The statement for home construction is a bit weak. I will fax a drawing that could be used with these
statements:
• Before home construction starts, install the stone construction entrance and protect any storm inlets.
• Install silt fence behind the curb and along the back property line to protect the Swale.
• Install downspout extensions to the curb or lake after the gutters are installed.
• Immediately seed and mulch the yard after the home is finished.
On sheet 9 the following changes ar`e recommended:
Change EROSION CONTROL heading to Temporary Seeding.
Delete the whole section titled Erosion and Sediment Control Measures
Combine the section titled Erosion Control Statement with Erosion Control SpeciScations. These
sections have similar or duplicate ideas and statements.
Note: IDEM will be notified when the plans are approved.
