The City has received your comment letter and drawings dated February 15, 2008 based upon the City
review of December 13, 2007.
The comments from the City review have been satisfactorily addressed with the following exceptions:
CONSTRUCTION PLAN REVIEW COMMENTS

2. Previous comment 42 -The Department rescinds this comment.
3. Previous comment 43 -Please verify that the location of all existing storm piping with top of curb
and invert elevations, pipe sizes and types, and flow direction are noted on the plans. Thank you
for your response. Is this information being obtained?
4. Previous comment 46 -All technical information requved by Section 100 of the Storm Water
Technical Standards shall be indicated on the plans. Section 100 requires specific information
for the plan sheet. Please refer to 102.02 (zi)(m) sod Section 102.02 in general.
5. Previous comment 63 -Please define MFPG on the plans per the Storm Water Technical
Standards Manual. Please add the definition to the plan sheets. Please contact us to obtain
the definition from the Storm Water Technical Standards Manual.

transportation. I have reviewed the drawings dated Febmary 14, 2008 and offer the following comments:

Thanks for taking my call. As discussed, my current comments on this project are as follows:
1. I need a detail of the planter area on the sidewalk to determine how much soil volume is being supplied for the
Japanese lilac trees in the planters. Near 200 cubic feet (the 5' x 12' x 3' depth you described) would be
adequate to support the small to medium trees for some number of years, but if further root space is needed,
something like structural soils or Jim Urban's Silva- Cell technology under the pavement should be utilized.

2. The two trees in the islands in front of the bank should be shade trees (I think your others are locust species), not
small growing trees.
3. Some shade trees should be planted along the north perimeter, even if that are could potentially be developed
into a parking lot. If planted near the pond, or in potential islands, they would not have to be moved. Or if planted
at this time on site, they could be tree spaded to a slightly different location a few years from now.
4. On the east bufferyard, the potential for conflicts with overhead utility lines means that close attention must be
paid to the planting, and maintenance, of the tree species there. You have chosen 'Moraine' variety of honey
locust, you might also consider'Skycole' or'Wandell' variety or something like a 'Shangra-La' or 'Princeton
Sentry' ginkgo as well.
Please contact me if there are any further questions, by email or phone. These comments should, besides improving the
health and look of the landscaping, should add value for the future of the bank in the West Clay Community.

The City has received the plans submitted for review at the December 19, 2007 Technical Advisory
Committee meeting. We offer the following comments:
GENERAL INFORMATION
1. These comments represent the Department of Engineering's first review of the construction
plans for this project. We reserve the right to ptovide additional review comments as the
project progesses thorough the approval process.
2. We request that all responses to our comments be provided in writing and be accompanied by
a drawing reflecting the requested revisions. Failure to provide written responses may result
in the delay of the review process.
3. It is critical that this office be made awaze of all modifications made on the plans being re-
submitted, particulazly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans including all revisions. Please notify us of
any changes and specifically state any changes, including changes resulting from Plan
Commission, BZA or other committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and dminage
calculations submitted to this office for review. If you have not already done so, please
provide a set of drainage plans and calculations to their office for review. We will share
Crossroad's comments as they are received.
5. Final drawings will not be approved for construction until:
a. All Carmel Engineering Department, Carmel Utility Department and Hamilton County
Surveyor issues have been resolved.
b. All bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) aze obtained.
d. All off-site easements necessary to install utilities to serve the development are secured.
e. Fees have been paid.
f. Digital file submissions have been made.
6. The Department reserves the right to provide additional comments based upon subsequent
reviews.
7, An approved Storm Water Management Permit is required prior to commencing any earth
disturbing activity.
8. An approved right-of--way permit is required prior to commencing any work in the public
right-of--way.
9. If it will be necessary to relocate existing utilities, the cost for such relocations shall be borne
by the developer. If utility poles must be relocated, they must be relocated to within one foot
of the proposed right-of--way.
10. Compliance with the Thoroughfare Plan includes construction of the improvements
prescribed by the Thoroughfare Plan across the frontage controlled by the development

(including sections with no access). This may apply to the Harleston Street and Towne Road
Street frontages. Typically, developers provide a commitment equal to the value to construct
these improvements for deposit into the City's non-reverting thoroughfare fund. Credit is
typically given towards this commitment for construction of the improvements prescribed by
the 20-Year Thoroughfare Plan and any milling and resurfacing of the perimeter streets.
11. The Department requires that the construction drawings be developed in accordance with the
City of Carmel digital submission standards and that all required submittals for primary plat,
secondary plat, and construction drawings be made. The digital files must be submitted to
the Department of Engineering prior to the approval of the construction plans. Please contact
the City GIS Department for the requirements.
12. Jurisdictions:
a. The project site is located within current City of Carmel Corporate Limits.
b. Streets and Right of Way -City of Carmel (Harleston Street, Towne Road)
c. Water -City of Carmel Utilities
d. Sanitary Sewers -Clay Township Regional Waste District (CTRWD)
e. Storm Sewers/Drainage -Hamilton County Surveyor's Office/City of Carmel.
f. Legal Drains -Hamilton County Surveyor's Office.
13. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require a minimum of 4 sets of drawings for approval after all issues have
been resolved. The drawings will be stamped as approved and signed by the City
Engineer and by Carmel Utilities. The Owner will receive 1 set which must be
maintained on the construction site at all times. If additional approved sets are desired,
we will approve a maximum of 2 additional sets. However, the additional sets must be
submitted with the requved minimum 5 sets.
14. Carmel Utilities will provide separate reviews of this project for water and sanitary sewer
issues. Please assure that copies of all drawings are sent to:

15. Carmel Utilities does subscribe to "Holey Moley" who should be contacted directly for all
water main locations.
16. Digital versions of the following information are available upon request. Please share these
requirements with the developer and future contractors associated with the project:

b. Perfarmance/Maintenance Guarantee Requirements
c. Performance/Maintenance Guarantee Release Procedure
d. Engineering Requvements for Building Permit Approval

BOARD OF PUBLIC WORKS AND SAFETY
17. A schedule for Board of Public Works and Safety meeting dates and agenda deadlines was
previously submitted to you for your use. Please use the Engineering Department deadlines
for submissions to the Board.
18. Any submission to the Board requires prior approval by the Carmel Cley Plao
Commission and/or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the

Bosrd's agenda must include the appropriate Docket Number and the date (or dates) mf
approval by the Plan Commission and/or the Board of Zoning Appeals (if applicable).
19. Water and Availability (EDU) approval from the Board is based upon the usage of the
proposed buildings within the development. Reference Items #34-38 below far a more
detailed explanation. If an irrigation system is planned far this development, approval for
additional Water Availability must be obtained from the Board.
20. Commercial Curb Cut Approval -This approval will require a written request from the
developer or the developer's engineer far each curb cut. The request must include 8'/~ x 11
exhibits of the curb cut or cuts with all pertinent data included such as radii, width, opposing
existing curb cuts, etc.
21. Temporazy Construction Entrance approval if the location is at a site other than the location
of the proposed permanent entrance to the development.
22. The installation of any permanent, privately owned and/or maintained improvement (signs,
decorative street signs, walls, streetlights, etc.) or outdoor seating or other areas within
dedicated right of way or dedicated easements requires the execution of a Consent to
Encroach Agreement between the Owner and the City of Carmel. Such agreements aze
executed by the Boazd of Public Works and Safety. The City Engineer may approve
irrigation system agreements.
23. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats far Board of Public Works and Safety approval.
24. Dedication of Right of Way if not platted. Appropriate dedication documents aze available
upon request.
25. Open pavement cuts of Towne Road or Hazleston Street would requve Board approval.
BONDING REOUII2EMENTS
26. Upon initial review, it appears performance guarantees shall be provided for the following:
a. Right of Way Improvements. This could include acceleration/deceleration lanes, passing
blister, pavement widening, milling, resurfacing, thermoplastic striping, stone shoulders,
curbs, etc.
b. Water Mains (service laterals and associated construction)
c. Erosion and Sediment Control and Storm Water Quality. Please contact Amanda Foley,
Engineering Department Stornwater Administrator, fot details regazding engineer's
estimates/performance guarantee requu•ements for these items.
27. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
28. Upon completion and release of individual Performance Guarantees, athree-yeaz
Maintenance Guarantee will be required. The Maintenance Guazantee amount is based upon
15% of the Performance amount for Streets and Curbs and 10% of the Performance amount
for all other improvements.
29. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
30. Please reference the enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
31. Any work in the dedicated right of way of Towne Road or Hazleston Street will requve an
approved Right of Way Permit and a License & Permit Bond.
32. The bond amount is determined by the number of instances of work in the right of way at
$2,000.00 per instance. However, if the work is included in the scope of work of a requred

and posted Performance Guarantee, the Performance Guarantee may be used to satisfy the
bond requirements of the Right of Way Permit.
33. Please contact our Right of Way Manager, Fred Glaser, to amnge right of way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
34. The Water Availability and Connection Fees are current as of this date but are subject to
future revisions.
35. Water Availability (acreage) Fees aze based on total platted acreage or legal description
acreage at the current rate of $1,010.00 per acre. The acreage indicated on the legal
description on Sheet C102 does not appeaz to include all of the property requred to support
the construction of the building such as parking lots, etc. If the Developer does not intend to
pay the total remaining Availability Fees due for Section 6003B, we will need a revised legal
description for this property in order to calculate the fee. Additionally, as of July 2, 2007, a
Development Application Review Fee of $150.00 will be assessed. This is based upon
developments with one customer requiring a water connection. This fee is to be paid with the
Availability Fees.
36. Water Connection Fees are based upon the proposed usage of the site at the current rate of
$1,396.00 per EDU, including a Tap Inspection Fee of $86.00/EDU effective July 2, 2007.
This Fee is based upon EDUs and should not be confused with Water Availability Fees,
which are acreage based. The EDU calculation for the proposed bank is:
3,945 ft' @ 0.5 EDUs/1000 ft' (Office Classification) = 1.973 EDUs. 1.973 EDUs @
$1,396.00 Water Connection FeeBDU = $2,754.31.
37. Water Availability and Connection Fees are to be paid prior to pre-submittal review by
Building Code Services.
38. If an irrigation system is planned for this development, additional Water Availability Fees
and Water Connection Fees (and Sanitary Sewer Availability and Connection Fees if
applicable) will be assessed based upon the size and usage of the system and upon the
recommendations of the Director of Carmel Utilities.
39. General Comments
a. This project is subject to the Storm Water Quality Ordinances. The City acknowledges
that the storm water management system was previously master planned.
b. Please contact Amanda Foley to review storm water quality/treatment requirements.
c. Be advised that the Department of Engineering does no[ have approval authority of
Landscaping and Lighting. Please do not include these items in future construction plans
submitted for review and/or approval.
40. Please confirm with the Department of Community Services that the right-of- way sidewalk
plans for this development are acceptable.
41. Please add the email for the main contact at the engineering fum and for the developer.
42. Please include the calculated building coverage ratio on the cover sheet.
43. Please verify that the location of all existing storm piping with top of curb and invert
elevations, pipe sizes and types, and flow direction aze noted on the plans.
44. Please indicate any proposed on-site roof drains.
45. There aze no easements indicated on site. Are any required or proposed? Is an easement
required over the off-site storm piping?
46. All technical information requved by Section 100 of the Storm Water Technical Standards
shall be indicated on the plans.
47. Please verify that on-site turning radii aze appropriate for emergency vehicles and any
vehicles expected to regulazly access the site.

48. Bicycle parking is requved. Please confirm with DOCS.
49. Are any cuts of the existing street anticipated?
50. Please verify that the fire hydrant indicated is the only one within 300 feet of the proposed
development. Is this the only hydrant serving the site?
51. Please verify that the location of all electrical vaults, electrical transformers, and overhead
and underground electrical, telecomm, and CATV lines and any gas lines are indicated an the
plans.
52. Why is the width of the "entrance only" curb cut so wide? Seems excessive for one-way
traffic and may create a conflict.
53. Sawcut and removal of the mainline pavement for the construction of any new curbing, the
new entrances, and on-street parking spaces shall be indicated. Patching and restoration of
such mainline pavement per City standard street cut patch detail shall be noted.
54. Please indicate City standard depressed curb across each entrance and on-street parking
spaces unless the normal crown of roadway is continuing into the spaces.
55. The curb return radii are not 30 feet. Please verify that the 15' radii aze consistent with other
entrances in the Village.
56. Any utility relocations requved by the project shall be the responsibility of the developer.
Utility line relocations requved for road projects that result in a conflict with the proposed
development shall be the responsibility of the developer to resolve with the utility. Existing
pole lines requiring relocation shall be relocated to within one-foot of the proposed right-of-
way line at the sole cost of the developer.
57. A maintenance of traffic plan shall be provided for the construction of the entrances and on-
street pazking. The plan shall conform to the INDOT Work Zone Safety Manual.
58. Please indicate the City standards depressed curb detail.
59. Please indicate curb removal to the nearest joint past the requred removal limits and Sawcut
of the full joint at this location.
60. Please verify that the proposed "c" value of the site does not exceed that value anticipated
during master planning.
61. Add the following note to all sheets: All paving in the existing and proposed City right-of=
way shall conform to the requirements of the Department of Engineering. Contractor shall
contact the Department of Engineering to schedule apre-construction meeting to review the
Department's construction requvements, staff notification requirements, required inspections
for certain stages of the work and to review the authority of the Department as it relates to
work within the existing and proposed City right-of--way. Please update the note on C102
with the full language.
62. Please verify that other commercial entrances in the village are asphalt.
63. Please define MFPG on the plans per the Storm Water Technical Standards Manual.
64. Please provide the note per 102.20 (vii) of the Storm Water Technical Standards Manual
unless storn system is private.
65. Please verify if the Urban Forester will require structural soil on site.
66. Please replace details 1/C80] and 2/C801 with current details from website.
67. Please place an "x" over brick surface tactile warning element on Detail 4/C801. The depth
of the ribbing is 3/8: '
68. Please hatch the right-of--way pavement section noted by keynote M for the portion of the
entrances and on-street pazking in the right-of--way on Sheet C101.
69. The walls outside of the right-of--way shall be in a pedestrian I/E easement.
70. Why is the curbing being installed on the north side only? Why not leave off for future
expansion similaz to the west curb line?
71. What is the disposition of water to the west in the short term?
72. Will the existing curbing be removed and replaced for the water tap? Will a walk closure be
requred?

The following letter represents comments for this project specifically addressing the area
of alternative transportation. I have reviewed the drawings submitted for the December
19, 2007 Technical Advisory Committee meeting and offer the following comments:
ALTERNATIVE TRANSPORTATION REVIEW COMMENTS
1) The City of Carmel Parking Ordinance requires all commercial venues to include
bicycle parking. Bicycle parking spaces are required at a rate of five (5) bicycle
parking spaces per one hundred (100) automobile parking spaces with a minimum
of four (4) spaces. In this instance it is highly recommended that at least four (4)
bicycle parking spaces are provided. Please revise your plans to show the bicycle
parking spaces on your site. Please see the City of Carmel's Parking Ordinance in
order to determine appropriate locations and construction details for the above.
We request that all responses to our comments be provided in writing. Failure to provide
written responses may result in delay of the review process.
It is critical that this office be made aware of all modification made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans indicating all revisions. Please notify us
of any changes and specifically state any changes, including changes resulting from Plan
Commission, Special Studies or other committee meetings.
