The City has received the plans submitted for review at the December 19, 2007 Technical Advisory
Committee meeting. We offer the following comments:
GENERAL INFORMATION
l . These comments represent the Department of Engineering's first review of the construction
plans for this project. We reserve the right to provide additional review comments as the
project progresses thorough the approval process.
2. We request that all responses to our comments be provided in writing and be accompanied by
a drawing reflecting the requested revisions. Failure to provide written responses may result
in the delay of the review process.
3. It is critical that this office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans including all revisions. Please notify us of
any changes and specifically state any changes, including changes resulting from Plan
Commission, BZA or other committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review. If you have not already done so, please
provide a set of drainage plans and calculations to their office for review. We will share
Crossroad's comments as they are received.
5. Final drawings will not be approved for construction until:
a. All Carmel Engineering Department, Carmel Utility Department and Hamilton County
Surveyor issues have been resolved.
b. All bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if requued) are obtained.
d. All off-site easements necessary to install utilities to serve the development are secured.
e. Fees have been paid.
f. Digital file submissions have been made.
6. The Department reserves the right to provide additional comments based upon subsequent
reviews.
7. An approved Storm Water Management Permit is required prior to commencing any earth
disturbing activity.
8. An approvedright-of--way permit is requred prior to commencing any work in the public
right-of--way.
9. If it will be necessary to relocate existing utilities, the cost for such relocations shall be borne
by the developer. If utility poles must be relocated, they must be relocated to within one foot
of the proposed right-of--way.
10. The Department requires that the construction drawings be developed in accordance with the
City of Cannel digital submission standards and that all requred submittals for primary plat,

11. Jurisdictions:

e. Storm Sewers/Drainage -Hamilton County Surveyor's Office/City of Carmel.

12. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require a minimum of S sets of drawings for approval after all issues have
been resolved. The drawings will be stamped as approved and signed by the City
Engineer and by Carmel Utilities. The Owner will receive 1 set which must be
maintained on the construction site at all times. If additional approved sets are desired,
we will approve a maximum of 2 additional sets. However, the additional sets must be
submitted with the required minimum S sets.
13. Carmel Utilities will provide separate reviews of this project for water and sanitary sewer
issues. Please assure that copies of all drawings are sent to:

14. Carmel Utilities does subscribe to "Holey Moley" who should be contacted directly for all
water main locations.

BOARD OF PUBLIC WORKS AND SAFETY
16. A schedule for Board of Public Works and Safety meeting dates and agenda deadlines was
previously submitted to you for your use. Please use the Engineering Department deadlines
for submissions to the Board.
17. Any submission to the Board requires prior approval by the Carmel Clay Plen
Commission and/or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Docket Number and the date (or dates) of
approval by the Plan Commission and/or the Board of Zoning Appeals (if applicable).
18. Water and Availability (EDU) approval from the Board is based upon the usage of the
proposed buildings within the development. Reference Items #34-38 below for a more
detailed explanation. If an irrigation system is planned for this development, approval for
additional Water Availability must be obtained from the Board.
19. Commercial Curb Cut Approval -This approval will require a written request from the
developer or the developer's engineer for each curb cut. The request must include 8%: x 1 I

exhibits of the curb cut or cuts with all pertinent data included such as radii, width, opposing
existing curb_cuts, etc. _
20. Temporary Construction Entrance approval if the location is at a site other than the location
of the proposed permanent entrance to the development.
21. The installation of any permanent, privately owned and/or maintained improvement (signs,
decorative street signs, walls, streetlights, etc.) or outdoor seating or other areas within
dedicated right of way or dedicated easements requires the execution of a Consent to
Encroach Agreement between the Owner and the City of Carmel. Such agreements are
executed by the Board of Public Works and Safety. The City Engineer may approve
irrigation system agreements.
22. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats for Board of Public Works and Safety approval.
23. Dedication of Right of Way if not platted. Appropriate dedication documents are available
upon request. I believe that the right of way for Moultrie Street will have to be formally
dedicated through the BPWS since the recorded plat for Section 6003B does not include
Moultrie Street.
24. Open pavement cuts of any existing portion of Moultrie would require Board approval.
BONDWG REOUIIIEMENTS
25. Upon initial review, it appears performance guarantees shall be provided for the following:
a. Streets
b. Curb & Gutters
c. Water Mains (service laterals and associated construction)
d. Erosion and Sediment Control and Storm Water Quality. Please contact Amanda Foley,
Engineering Department Stormwater Administrator, For details regarding engineer's
estimates/performance guarantee requirements for these items.
26. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
27. Upon completion and release of individual Performance Guarantees, athree-year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based upon
15% of the Performance amount for Streets and Curbs and 10% of the Performance amount
far all other improvements.
28. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
29. Please reference the enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
30. Any work in the dedicated right of way of existing portions of Moultrie Street will require an
approved Right of Way Permit and a License & Permit Bond.
31. The bond amount is determined by the number of instances of work in the right of way at
$2,000.00 per instance. However, if the work is included in the scope of work of a required
and posted Performance Guarantee, the Performance Guarantee may be used to satisfy the
bond requirements of the Right of Way Permit.
32. Please contact our Right of Way Manager, Fred Glaser, to arrange right of way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
33. The Water Availability and Connection Fees aze current as of this date but aze subject to
future revisions.

34. Water Availability (acreage) Fees are based on total platted acreage or legal description
acreage at the current rate of$1;O10:00 peracre. The-acreage indicated on theiegal
description on Sheet C 101 does not appear to include all of the property required to support
the construction of the building such as parking lots, etc. If the Developer does not intend to
pay the total remaining Availability Fees due for Section 6003B, we will need a revised legal
description for this property in order to calculate the fee. Additionally, as of July 2, 2007, a
Development Application Review Fee of $150.00 will be assessed. This is based upon
developments with one customer requiring a water connection. This fee is to be paid with the
Availability Fees.
35. Water Connection Fees are based upon the proposed usage of the site at the current rate of
$1,396.00 per EDU, including a Tap Inspection Fee of $86.00/EDU effective July 2, 2007.
This Fee is based upon EDUs and should not be confused with Water Availability Fees,
which are acreage based. The EDU calculation for the proposed gym, per Schneider's letter
of November 19, 2007 is:
4,561 fN @ L0 EDUs/1000 ft' (Retail Classification) = 4.561 EDUs.
4.561 EDUs @ $1,396.00 Water Connection Fee/EDU = $6,367.16
36. Water Availability and Connection Fees are to be paid prior to pre-submittal review by
Building Code Services.
37. If an irrigation system is planned for this development, additional Water Availability Fees
and Water CannectiomFees (and Sanitary Sewer Availability and Connection Fees if
applicable) will be assessed based upon the size and usage of the system and upon the
recommendations of the Director of Carmel Utilities.
38. General Comments
a. This project is subject to the City's Storm Water Manangement and Storm Water Quality
Ordinances.
b. Please contact Amanda Foley to review storm water quality/treatment requirements.
c. Be advised that the Department of Engineering does not have approval authority of
Landscaping and Lighting. Please do not include these items in future construction plans
submitted for review and/or approval.
39. Please confirm with the Department of Community Services that the right-of--way sidewalk
plans for this development are acceptable.
40. Please ensure there are no frames, castings, etc. from storm, sanitary, or water main
structures, manholes, or valve boxes in path, walk, aprons, or pavement in the right-of--way.
41. Will the angled parking spaces off of Moultrie Street be included in the rightof--way
dedication?
o Please verify that the proposed °c" value does not exceed that value anticipated at master
planning.
o Please add the Developer's fax and a-mail address to cover sheet.
o Please indicated the calculated building coverage ratio on the cover sheet.
o Are any easements proposed within the project limits? Do any easements conflict with
landscaping or tree preservation easements or legal drain or other easements that cross
the parcel? Since pipe run 611.1-610.1 conveys runoff from the right-of--way, it must be
in a 30' drainage easement. Since all lateral runs and roof drains are off site, is the
parking area a blanket D.U. and S.E.7 Are easements requved7
o Please verify that the location of all storm pipe, culverts, and bridges with corresponding
top curb elevation, invert elevation, pipe size, pipe type, and direction of flow is
indicated.
o Please indicate all technical information required by Section100 ofthe Storm Water
Technical Standards Manual on Sheet C102.

o Please verify that all taming radii are appropriate and adequate for emergency response
vehicles-and vehicles expected to regularly accessthe site.--- -- - - - - ----
o Please verify that the curb cut is at a previously planned and approved location. The
Moultrie access points are not well aligned.
o Please verify if there are any direct discharge areas.
o Please verify if bicycle parking is required.
o Are any swales planned? All swales must have subsurface drains.
o Please verify that safe stopping sight distance is available for vehicles traveling an
Moultrie Street and Harleston Street and to see vehicles exiting the proposed cut.
o Please add a note to Sheet C 102 that states "There shall be no manholes or inlet frames or
castings with the exception of curb inlets or water valve boxes, within proposed
pavement, curb ar sidewalk in the right-of--way.
o Please verify that all above ground and underground electrical, telecomm, CATV, and
gas infrastructure is indicated on the plans.
o Please indicate the construction limits on each sheet.
o Please verify if a future curb cut is anticipated on the east side of Moultrie between the
existing cut and Harleston Street.
o Please update Note 5 on Sheet C201.
o Pavement markings in the right-of--way must be thermoplastic.
o Right-af--way permit required for use of Moultrie Street for construction traffic. [s
another access for such ttafic available?
o Please indicate with hatching the areas for right-of--way pavement noted with plan note
"M" on Sheet C 101. The portion of entrance within the right-of--way, provided that an
asphalt entrance is consistent with other entrances in the VOWC, shall be this pavement
section also.
o Please verify that 15' return radii at the entrance is consistent with other entrances in the
VOWC.
o Please indicate a sawcut of the existing pavement where the new pavement is to be
placed. The sawcut shall be back sufficient distance to allow full depth pavement in good
condition to be utilized as the joint. If surface has been placed on the existing Moultrie
Street, some amount of milling will be required to provide a butt joint with the fuuil
surface placed with the extension of the street across the frontage of this site.
o Please update chair back curb detail with detail available from the website.
o Please replace the valley gutter detail with the City standard depressed curb detail
available from the website.
o The City standard fmal backfill notes shall be added to the sanitary and water trench
details far the portion of these installations within the rightof--way.
o Please verify the proposed sidewalk width. Walk is indicated as 5'-0" on Section A-A
for Moultrie Street but detail 8/C801 is 4'-0". If walk is 5'-0" ,detail 8/C801 will need to
be revised for width and transverse joint spacing.
o Please indicate an "x" over "brick surface construction" on detail 6/801.
o Please verify if the relocated legal drain represents a flood hazard area? Is the 100-year
contained in the pipe? Is there an overflow route for off-site water? If this is the case,
there is no fill allowed in the floodplain.
o Please add the note per 102.20(vii) of the Storm Water Technical Standards Manual to
Sheet C601.
o Please add the definition of MFPG pet the Storm Water Technical Standards Manual to
the plan sheets. Please contact the Department of Engineering.
o The portion of the sidewalk and the parking spaces outside of the right-of--way must be in
an I/E easement.

o Will water be installed by an open trench method prior to Moultrie Street being

water main and service laterals shall be installed prior to wnstmcting roadway if by open
trench method.
o The roof drain connection to the existing storm structure to the north shall be core drilled
and is subject to inspection by the City. Will this roof drainage be treated?
o Verify that Harleston and Moultrie are not existing where sanitary crosses. City backfill
shall be required (and indicated on the profile) where the main crossed under the
proposed and the future street.
o Please verify if the Urban Forester will requve structural soils on site.

We have reviewed the construction plans submitted to the Hamilton County Surveyor's Office
on November 20, 2007 for this project and have the following comments:
1. The proposed project falls within the incorporated area and MS4 jurisdiction of the
City of Carmel.
2. This proposed site DOES NOT fall in a Caralel Well Head Protection Area.
3. The proposed project falls in The Village of West Clay Regulated Drain Subdivision
and The Village of Wast Clay watershed.
4- The proposed storm sewer associated with this project is marked as private. Since
this does provide drainage for a public roadway, it should became part of the Village
of West Clay Regulated Drain.
5. Please submit the following procedural items to this o tTrce, petition for regulated
subdivision, outlet pemlit (1 for VOWC Drain), subdivision non-enforcement,
engineer's catimate, and all associated bonds for 120% of construction cost. The

6. The easements for the proposed regulated drain should be shown on the plat as 10'
per half easements from the center line of the drain.
7. Please include the applicable Hamilton County Surveyor's Office Standards in the
construction plans.
8. Does the master detention basin for VOWC Section 6003 have capacity for this site in
the eurrtnt stage of construction the pond is in? 'Ilte pond is not completely built as
designed, but additional runoff keeps being directed toward the basin. Will it need to
be enlarged with this project?
9. Based on the supplied drainage calculations, it does not appear much area other then
right of way east of this site is being accounted for with this project. When the area
to the east develops, how will it get to the detention basin?
10. Please note the additional comments maybe warranted at a later date.

(including sections with no access). This may apply to the Moultrie Street frontage.
Typically, developers provide a commitment equal to the value to construct these
improvements for deposit into the City's non-reverting thoroughfare fund. Credit is typically
given towards this commitment far construction of the improvements prescribed by the 20-
Year Thoroughfare Plan and any milling and resurfacing of the perimeter streets.

Encroach Agreement between the Owner and the City of Carmel. Such agreements are
executed by the Board of Public Works and Safety. The City Engineer may approve
irrigation system agreements.
23. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats for Board of Public Works and Safety approval.
24. Dedication of Right of Way if not platted. Appropriate dedication documents are available
upon request. I believe that the right of way for Moultrie Street will have to be formally
dedicated through the BPWS since the recorded plat for Section 6003B does not include
Moultrie Street.
25. Open pavement cuts of any existing portion of Moultrie would require Board approval.
26. Upon initial review, it appears performance guarantees shall be provided for the following:
a. Streets
b. Curb & Gutters
c. Water Mains (service laterals and associated construction)
d. Erosion and Sediment Control and Storm Water Quality. Please contact Amanda Foley,
Engineering Department Stormwater Administrator, for details regarding engineer's
estimates/performance guarantee requirements for these items.
27. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100°~ of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
28. Upon completion and release of individual Performance Guarantees, athree-year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based upon
15% of the Performance amount for Streets and Curbs and 10% of the Performance amount
for all other improvements.
29. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
30. Please reference the enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMTf AND BONDING
31. Any work in the dedicated right of way of existing portions of Moultrie Street will require an
approved Right of Way Permit and a License & Permit Bond.
32. The bond amount is determined by the number of instances of work in the right of way at
$2,000.00 per instance. However, if the work is included in the scope of work of a required

and posted Performance Guarantee, the Performance Guarantee may be used to satisfy the
bond requvements of the Right of Way Permit.
33. Please contact our Right of Way Manager, Fred Glaser, to arrange right of way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
34. The Water Availability and Connection Fees are current as of this date but are subject to
future revisions.
35. Water Availability (acreage) Fees are based on total platted acreage or legal description
acreage at the current rate of $1,010.00 per acre. The acreage indicated on the legal
description on Sheet C 101 does not appear to include all of the property required to support
the construction of the building such as parking lots, etc. If the Developer does not intend to
pay the total remaining Availability Fees due for Section 6003B, we will need a revised legal
description for this property in order to calculate the fee. Additionally, as of July 2, 2007, a
Development Application Review Fee of $150.00 will be assessed. This is based upon
developments with one customer requiring a water connection. This fee is to be paid with the
Availability Fees.
36. Water Connection Fees are based upon the proposed usage of the site at the current rate of
$1,396.00 per EDU, including a Tap Inspection Fee of $86.OOBDU effective July 2, 2007.
This Fee is based upon EDUs and should not be confused with Water Availability Fees,
which are acreage based. The EDU calculation for the proposed gym, per Schneider's letter
of November 19, 2007 is:
4,561 ft' @ 1.0 EDUs/1000 ft' (Retail Classification) = 4.561 EDUs.
4.561 EDUs @ $1,396.00 Water Connection Fee/EDU = $6,367.16
37. Water Availability and Connection Fees are to be paid prior to pre-submittal review by
Building Code Services.
38. If an irrigation system is planned for this development, additional Water Availability Fees
and Water Connection Fees (and Sanitary Sewer Availability and Connection Fees if
applicable) will be assessed based upon the size and usage of the system and upon the
recommendations of the Director of Carmel Utilities.
CONSTRUCTION PLAN REVIEW COMMENTS
39. General Comments
a. This project is subject to the City's Storm Water Manangement and Storm Water Quality
Ordinances.
b. The City has adopted a new curbing and paving policy. Please revise any notes,
specifications, and details to accommodate these policies. The language of these policies
must be added to a specification sheet on the final construction plans.
c. Please contact Amanda Foley to review storm water quality/treatment requirements.
d. Be advised that the Department of Engineering does not have approval authority of
Landscaping and Lighting. Please do not include these items in future construction plans
submitted for review and/or approval.
40. Please confirm with the Department of Community Services that the right-of--way sidewalk
plans for this development are acceptable.
41. Please ensure there are no frames, castings, etc. from storm, sanitary, or water main
structures, manholes, or valve boxes in path, walk, aprons, or pavement in the right-of--way.
42. Will the angled parking spaces off of Moultrie Street be included in the right-of--way
dedication?

The following letter represents comments for this project specifically addressing the area
of alternative transportation. I have reviewed the drawings submitted for the December
19, 2007 Technical Advisory Committee meeting and offer the following comments:
ALTERNATIVE TRANSPORTATION REVIEW COMMENTS
1) The City of Carmel Parking Ordinance requires all commercial venues to include
bicycle parking. Bicycle parking spaces are required at a rate of five (5) bicycle
parking spaces per one hundred (100) automobile parking spaces with a minimum
of four (4) spaces. In this instance it is highly recommended that at least four (4)
bicycle parking spaces are provided. Please revise your plans to show the bicycle
parking spaces on your site. Please see the City of Carmel's Parking Ordinance in
order to determine appropriate locations and construction details for the above.
We request that all responses to our comments be provided in writing. Failure to provide
written responses may result in delay of the review process.
[t is critical that this office be made aware of all modification made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans indicating all revisions. Please notify us
of any changes and specifically state any changes, including changes resulting from Plan
Commission, Special Studies or other committee meetings.
