In addition, I have enclosed a copy of sheets C101 and CI02 of Section 9006 which include [he bottom elevation of the pond and sidewalk ramps in the vicinity of the round-a-bout at the North end of the site. It is also intended to identify areas where additional measures may be required to control erosion and sedimentation. All practices recommended in this report should be evaluated ns to their feasibility by a qualified individual with structural practices designed by a gualified engineer. The permanent pipe crossing has been recently installed. The pads and rear of the lots have been mulched. Silt fence near the crossing has been knocked down. The paved street is very muddy. Water flowing down the street will flow unfiltered into the ditch. Mass grading is progressing on the north side of the ditch. The road cut will direct stormwater to the ditch without protection. The temporary sediment basin installed on the north side of the ditch and to the west of the road is poorly constructed. The earth levee is below the rock spillway outlet. The altered erosion control plan is not adequate near the ditch. A revised design is needed by 1-28 that includes dimensions and elevations. It is the intent of the Division to resolve non-compliance issues and avoid referring this project to the Indiana Department of Environmental Management (IDEM) for an enforcement action. The enclosed On-Site Evaluation documents that an insufficient effort has been made to comply with the conditions and requirements of 327 IAC 15-5. The report cleazly outlines the deficiencies observed at the site and the requirements to bring the project back into compliance. Failure to implement and maintain appropriate erosion and sediment control measures is a serious violation and deserves your immediate attention. It is requested that on or before November 21, 2003 that corrective action is taken to address the deficiencies specified in the enclosed on-site evaluation. Once corrective action has been taken to address the deficiencies, please notify John South so a follow up inspection maybe conducted. Staff will continue to inspect this site to monitor progress and assess compliance with the Rule. Timely implementation and maintenance of erosion and sediment control measures is critical in preventing off-site sedimentation. Failure to correct deficiencies associated with this site will result in a referral to the Indiana Department of Environmental Management and the initiation of an enforcement action. Small sediment traps could be created in the rear yard swales behind lots 72 and 75. The rock check could be used as the riprap for the Swale outlets. A practice is needed to prevent stormwater from being channeled to the ditch via the street prof le south of the crossing. When can the diversion Swale be eliminated at station 5+75 on Dumbarton Street? Additional information is needed about the installation of the crossing on Elliot Creek. What practices will be used to prevent sedimentation of the stream? Is a construction crossing proposed? 4G. The storm inlets at station 4+50 need to work well all the time.
