amount of $582.50 for the filing. I have also included copies of the Hamilton County
Subdivision Review Sheets signed by the appropriate Hamilton County Offices.
Once Mr. Hollibaugtr has signed all the prints, please contact me and I will have the Secondary
Replat forwarded b file Hamilton County Highway Department for signatures by the County
Commissioners.

Remarks: Angelina, Per our phone conversation, attached is the revised replat per your 8/6/03 review letter
and our phone conversation. We were able to get is all on one sheet when we changed the scale to 1"=50'.

RE: Replat Nortb Augusta Subdivision, Lot 13- Secondary Plat Application

This letter is in response to your Secondary Plat application for Replat of North Augusta Subdivision, -
Lot 13. The following items listed aze a starting point, and more comments will be made at the TAC
Meeting. The following issues must be resolved prior to signature of the plat by the director:
Plat:
1. Please provide the submission date and/or latest revision date.
2. Please provide the subdivider's contact information.
3. Place deed instrument No. on plat.
4. Place a floodplain note on plat.
5. Please show plat at 1" = 50' or 1" = 100'
6. Please show surveyor's signature and date on plat.
7. Please provide a copy of covenants/restrictions.
8. Please label building setback lines.
9. Change BPW signature block to County Commissioner Block with correct names.

10. Provide service reports/statements from: utilities, police, fire, soil & water conservation, etc.
11. Forwazd a copy of approval letters from Urban Forester and City Engineer.
