We will provide comments for [he drawings received on Febmary 1Q, 2004 as soon as possible. GENERAL INFORMATION 1. The project site is curzently located within current City of Carmel Corporate Limits. The following jurisdictions will apply. 2. Jurisdictions: • Street and Right of W ay - City of Carmel (131" Street). • Water-Indianapolis Water Company service area. • Sanitary Sewers -City of Carmel • Storm Sewer- City of Comae] • J.R. Collins Legal Drain -Hamilton County Surveyor 3. Board of Public Works and Safely Approval Requirements: • Sanitary Sewer Availability • Commercial Curb Cut for new and/or modified 131` Street Entrances. We will require an 8 '/: x 11 drawing of each entrance/curb cut to be used as exhibits for your BP W request. These drawings should include all pertinent dimensions including accel/decel lanes, drive lane and median widths, opposing curb cuts, radii, etc. • Temporary Construction Entrance (if located at a site other than the existing or proposed curb cuts). Same requirements as for the Commercial Curb Cut. • Consent to Encroach Agreements for any permanent improvement to be installed within dedicated easements and/or dedicated right of way. • Right of Way dedication (if applicable). Sanitary Sewer Availability (acreage) and Connection Fees: • Availability (acreage) Fees are based on [he legal description of the property. The church originally paid (on June 2Q, 1994) Availability Fees on 9.25 acres. However, this was later adjusted to 6.6 acres due to the fact that the church sold 2.6 acres to the State of Indiana fox completion of the U.S. 31 project ST- F-222 (9). The church was reimbursed for the value of the difference of 2.6 acres. Availability Fees will be due on the difference between the origina16.6 acres and the total acreage of the church expansion. We note the legal description on the Cover Sheet of the drawings submitted indicate a total of 10.144 acres more or less. It appears that this acreage is for the expanded site. We also note [here is a legal description for Tract "A" {n the ADLS submission that indicates 9.553 acres. What is the difference? • Connection Fees are based upon the average user factor tables from Section 9-200 (sanitary) of our Utility Ordinance. The Connection Fees are 5795.00 per EDU for sanitary sewers. Again, the church originally paid (on June 2Q, 1994) Connection Fees based upon a seating capacity of 325 and 2.6 EDU's. However, this was later adjusted to a capacity of 180 and 1.4 EDU's. The church was reimbursed for the value of [he difference of 1.2 EDU's. We will need usage information for the expanded site in order to calculate sanitary sewer connection fees. I am enclosing copies of our Average User Factors for Sanitary Sewers and our current fee schedule. I have also highlighted the Average User Factors [ha[ might apply to this development. Note: Any deviation from the Connection Fee calcuiatiars listed in our Ordinance will require Cannel (Itilities and Board of Public Works and Safety approvals. 4. TAC Review/Drawings submitted for approval: We request that all comments and comment letters generated by this office be answered in writing and be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for construction until al] issues have been resolved. All Carmel Utility issues mast also re resolved. The design engineer must certify all drawings submitted for final approval. This office will require a minimum of four-sets (maximum of six-sets) of approved drawings after all issues have been resolved. The drawings will be stamped approved and signed by both the City Engineer and Drrector Carmel Utilities. The Developer/Owner will receive one-set which is to be maintained on the construction site at all times. Carme] Utilities will receive one-set. Our Public Works Inspector will receive one-set and one-set will be maintained in [he Department of Engineering. If more than the minimum offour- sets is requested to be approved, the additional sets (maximum total of two additional) must be submitted with [he four required sets. 5. Please be advised [hat any installation of signs, walls, irrigation systems, etc. within dedicated easements and/or right of way wilt require a Consent to Encroach Agreement between the Owmer/Developer and the City of Carmel. This Agreement requires Board of Public Works and Safety approval. 6. Cannel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all sanitary sewer main locations. 7. Carmel Utilities will provide a separate review of this project for sanitary sewer issues. 8. I am including copies of the following with [his correspondence: • Commercial Project Approval Procedures. • Performance Release Procedure. • Permit Da[a, Contacts, etc. • Commercial Permitting Procedures. • Dedication of Right of Way documents. BONDING REOUIREDIENTS 9. Upon initial review, it appears the following bonding requirements may apply to this project: Mr. Tim Fleck March 10, 2004 Page 3 Performance Guarantees • There do not appear to be any Performance Guarantee requirements for this expansion. Street Cu[/Right of R'ay Permit and Bonding Requirements There appear [o be at least three instances of work in the right of way of 131" Street associated with demolition activities (Sheet C1.01): I) Remove existing pavement and/or walk 3) Saw cudremove pavement 5) Remove power poles/guide wires (mayor may not be right of way issues). Also, new and/or modified 131" Street entrances will be included in the Right of Way Permit requirements as will [he closure/elimination of any existing residential curb cuts. Details regarding the Right of Way Permit and associated Right of Way bonding requirements should be directed to Fred Glaser, Department of Engineering Right of Way Inspector/Manager. Please note that anv open pavement cuts in dedicated streets require approval by our Board of Public R'orks and Safetv. PROJECT COMMENTS 10. Please provide an estimated construction schedule for the three phases. 11. Will the existing church building remain after the completion of the expansion? If so, in what capacity? 12. There is a proposed development on the south side of 131 ~` called Alexandria of Carmel. Please indicate on your site plans [he proposed location of their curb cut. The plans for this development have been prepared by Paul I Cripe Engineers. The contact at Cripe is Paul Donica. Sheet C1.01 Demolition Plan 13. Demolition permits will be required through our Building and Code Enforcement Department. If there are any wells to be abandoned or removed, contact Morris Hensley of Carmel Utilities. 14. Please dimension the apparent north right of way line of 131" Street. Sheet C2.O1Site Plan 15. Is the 60-foot half right of way indicated on this sheet, right of way to be dedicated to the City of Carmel? We assume that this is not existing right of way. If it is not, please indicate, identify and dimension the existing right of way line. 16. Are [he patterned areas intended to indicate sidewalks? This pattern should be shown and identified as a Legend on the sheet. 17. If these are sidewalks, what are you planning in [he right of way? Is this stamped and colored concrete? 18. Are there additional existing/plannedeosements other than the 5' Utility Strip? Sheet C2.03 Phase 2 Site Plan 19. Please distinguish Phase 2 from Phase 1 bycross-hatching or shading Phase 1 construction. Sheet C2.04 Phase 3 Site Plan 20. There are no underground storm pipes planned for [he parking lots or other areas other than those indicated? 22. Are the initials FL intended to designate flow lines or "invert elevations"? 23. It appears that the outlet for the dry detention pond discharges into the right of way of 131 °' Street. This is not an acceptable outlet location. No storm sewer conveyance system exists along 131" Street at this time that can accommodate the storm drainage from this site. Please modify plans ro identify an acceptable outlet and allow detention basins to discharge to that location. Mr. Tim Fleck March 1Q 2004 Page 4 24. Is the %:" and'/a"pipes found on the west boundary and on the right of way line associated with the legal drain? What is the pipe material? 25. How do you plan to serve the expansion area with water and sanitary sewer if this expansion happens prior to Carmel Utilities extension of utilities? 26. The water service is indicated [o be "existing Carmel water main extension". Don'[ think that is the case. The above comments are based upon the Engineering Department's initial review. Please provide our office with a follow up letter regarding this review and revised plans, indicating all revisions. A second review will focus only on detailed construction plans. It is critical that our office be made aware of all modifications made on the plans being re-submitted, particularly if any such changes are considered "new" or fall outside of our initial review. This project will not be released for construction until all Engineering Department and other local agency issues have been addressed. Specifically, all issues outlined by the Hamilton County Surveyor's Office and the Hamilton County Soil and Water Conservation District must be addressed.
