Dear Mr. Burke:
The City has received your comment letter and drawings dated March 3Q, 2009 based upon the Ci[y review
of March 24, 2009.
After incorporating the following comments into the plans and incorporating any revisions necessary to
obtain storm water quality approval, please submit all copies necessary for approval. All bonds and
performance guarantees (if required) must be posted prior to Engineering Department approval. Also,
Board of Public Works and Safety approval and any other governing agency approvals (if required) must
be obtained prior to Engineering Department approval.
CONSTRUCTION DRAWING REVIEW COMMENTS
39. General Comments
g. Please provide definitions of the MLAG and MPPG. Please revise note 12 on Sheet
C102 as follows: "Lowest adjacent grade is the elevation of the lowest grade
adjacent to a structure, where the soil meets [he foundation around the outside of
the structure (including structural members such as basement walkout, patios,
decks, porches, supportposts or piers and rim of the window well)."
Based on additional review, the City provides additional comment:

Amanda- Please find enclosed revised construction documents per the City of Carmel Dept of Engineering and Stormwater Quality. A
couple of general notes were revised for typos, construction sequencing was revised for vegetated Swale, a few erosion control details
were revised. In addition, a future location for a bicycle rack is denoted on sheet C101. This will be installed in the future once the
future parking lot is expanded and curbing/storm sewer is placed along the edges of pavement for this portion of parking lot.

Dear Mr. Burke:
The City received your construction plans, which will be submitted to the February d 8 Technical Advisory
Committee meeting, on January 20, 2009. We offer the following comments:
GENERAL, INFORMATION
I. These commetts represent the Department of Engineering's firs[ review ofthe construction
plans For this project.
2. We request that all responses to our comments be provided in writing and be accompanied by
a drawing reflecting the requested revisions. Failure to provide written responses may result
in the delay of the review process.
3. ]t is critical that [his office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please provide revised plans including all revisions. Please notify us of
any changes and specifically state any changes, including changes resulting from Plan
Commission, BZA or other committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review. If you have not already done so, please
.provide a set of drainage plans and calculations to their office for review. We will share
Crossroad's comments as they,are received.
5. Final drawings will not be approved for construction until:
a. All Engineering Department and~Utility Department and Hamilton County Surveyor
issues have been resolved.
b. All bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) are obtained.
d.. All off-site easements necessary to install utilities to serve the development are secured.
e. SWPP is approved.
f. All fees are paid.
6. The Deparhhehtteserves the right to provide additional comments based upon subsequent
reviews.
7. An approved Stone Water Management Permit is required prior to commencing any earth
disturbing activity.
8. An approved right-of--way permit is requued prior to commencing any work in the public
right-of--way.
9. if it will be necessary to relocate existing utilities, the costs for such relocations are to be

10. The Department requires that the construction drawings be developed in accordance with the

and by Cannel Utilities. The Owmerwill receive 3-sets,onc of which must be maintained
on the construction site at all times. If this project is subject to review and approval by
the Hamilton County Surveyor's Office, a total of 9-sets will be required for final
approval.
13. Carmel Utilities will provide separate reviews of this project for water issues. Please assure
that copies of all drawings are sent to:

BOARD OF PUBLIC WORKS AND SAFETY -
16. A schedule for Board of Public Works and Safety.meeting dates and agenda deadlines will be
sent electronically for your use upon request. Please nse the Engineerins Department
deadlines for submissions to the Board.
17. Any submission to the Board requiresprior approval by the Carmel Clay Plan
Commission and/or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Doeket Number and thedate (or dates) of
approval by the Plan Commission and/or the Board of Zoning Appeals (if applicable).
18. Water Availability approval from the Board will be required. This is an EDU approval based
upon the proposed ttse of the site. Reference Items #34 to #38 below for additional
details/explanations.
19. Commercial Curb Cut Approval - It appears that this approval will not be applicable to this
project.
20. Temporary Construction Entrance - If a location other than [he existing Broughton Street
curb cut is to be utilized as a construction entrance, Board approval will be required.
2I. The installation of any permanent, privately owned and/or maintained improvement (signs,
decorative street signs, walls, streetlights, etc.) within dedicated right of way or dedicated
easements requires the execution of a'Consent to Encroach Agreement beriveen the Owrter

and the City df Cannel: Such`'agreements are executed by the Board of Public Works and
Safety. The City Engineer may approve imigatiop system agreements.
22. Secondary Platapproval if applicable. All performance guarantees must be posted prior to
subtttission of secondary plats for Board of Public Works and Safety approval.
23. Dedication of Right of Way if not platted. This is based upon the City of Carmel 20-Year
Thoroughfare Plan requirements. Dedication documents are available upon request.
24. Any open pavement cuts of Broughton Sheet will require Board approval.
BONDING REQUIREMENTS
25. Upon initial review it appears performance guarantees shall be provided for the following:
a. Right of Way Concrete'Sidewalk
b. Erosion and Sediment Control and Storm Water Quality. Please contact Amanda Foley,
Engineering Departmenr,S[omtwater Administrator, for details regardine engineers
estimates/perfomrance guarantee requirements for these items.
26. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100%of the cost of labor and nta[erials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer'sEstimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
27. Upon completion and release of individual Performance Guarantees, athree-year
Maintenance Guarantee will be required (see Street Sign comments above). The
Maintenance Guarantee amount is based upon 15% of the Performance amount for Streets
and Curbs and 10% of [he Performance amount for all other improvements.
28. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
29. Please reference the enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
30. Any work in [he.dedicated right of way of Broughton Street will require an approved Right
of Way Permit and a License & Permit Bond
31. The bond amount is detenniped by our Right of Way Manager. However, if the work is
included in the scope of work of a required and posted Perfornance Guarantee, the
Performance Guarantee maybe used to satisfy the bond requirements of the Right of Way
Permit.
37. Please contact our Right of Way Manager, Fred Glaser, to arrange right of way permitting
and bonding:
33. Use of existing entrance for construction traffic will require aright-of--way permit and
appropriate bonding.

than one customer requiring a water connection.
36. Water Connection Fees
a. The-Water Connection Fee will be calculated on the assumption that this facility wiH be
an office facility. Please. provide [he total square footage of the building. The Water
Connection Fee is calculated as follows: square feet @ 0.5 EDUs/1000 ft' =Total EDUs
x $2,701.00.($2,615.OOWa[erCgnnection Fee plus $86.OOWa[er Tap FeeiEDU).

37. tf an entryway or overall site irrigation system is planned for this development, Water
Connection Fees will be assessed based upon the size and usage of the system and upon the
recommendations of the Director of Carmel Utilities.
38. Water Availability and Connection Fees are required to be paid prior to final approval of
construction plans by Engineering and prior to issuance of building permits by~Building
Codes Services. Please confirm these fees and calculations with Cannel Utilities.

The following letter represents comments for this project specifically addressing the area
of alternative transportation. 1 have reviewed the drawings submitted for the February 18,
2009 Technical Advisory Committee meeting and offer the following comments:
ALTERNATIVE TRANSPORTATION REVIEW COMMENTS
]) It is highly recommended that bicycle parking be included in your plans. Bicycle
parking spaces are required at a rate of five (5) bicycle parking spaces per one
hundred (100) automobile parking spaces with a minimum of fow' (4) spaces. In
this instvrce it is recommended that at least four (4) bicycle parking spaces are
provided. Please revise your plans to show the bicycle parking spaces on your site.
Please see the City of Carmel's Parking Ordinance in order to determine
appropriate locations and construction details for the above.
2) Please indicate ADA compliant handicap ramps at all crosswalks and sidewalk
termination points.

1. The proposed project falls in the incorporated area and MS4 jurisdiction of the City of
Carmel.
2. The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone.
3. The proposed project falls in The Village of West Clay Regulated Drain Subdivision
and The Village of West Clay Regulated Drain Watershed.
4. Detention volume for this site is provided in the master plan for Village of West Clay.
5. Please submit the following procedural item: outlet permit for the indirect discharge
into the Village of West Clay Regulated Drain. The application is available on our

I received the landscape plan for the Prudentia Building on February 2, 2009. In my review I noticed a couple things:
1. There needs to be a plant schedule on the plan detail the plant material in species, size and number.
2, Species: Cornus kousa - Kousa Dogwood seem to be a species that do well here. Sugar Maples may not be the
best species for the limited soil volumes and tight urban spaces located here. You may try a mixture of London Plane tree
(Platanus x acerifolia ), ginkgo (Ginkgo biloba), and/or a red oak (Quercus rubra) variety.
3. Please make sure that your planting details agree with the public domain details I have provided, especially tree
planting with root flare at grade level. There has been a problem in the past with planting trees too deep in the Village of
West Clay.

I'll send a regular written response and paper copies of the elevations. However wanted to check on the color and
materials, this is a new request for the buildings out there.
Currently, they are looking to do the following.
Stucco -grey limestone color _ ~ ~ ~S
• Gutters -weathered copper (change from dark grey)
Windows -off white (change from black)
Shingles -slate look, irregular shaped squares /rectangles
Would this response suffice for TAC? They asked if they had to find material samples.....1 haven't seen this done for
VOWC before.
I also just received a landscape plan from the architect. Will this require a full blown 24x36 plan or would this work? I can
do the plan tonight and get it over tomorrow if required.
