RE: Carmel Elementary School Kindergarten Addition-Project Review #1
Dear Mr. Miller:
The City has received your construction plans dated December 19, 2008. This prbject will be reviewed at
the January 21, 2009 Technical Advisory Committee meeting. We offer the following comments:
GENERAL INFORMATION
1. These comments'represent the Department of Engineering's first review of [he construction
plans for this.p~oject.
2. We request that all responses to our comments be provided in writing and be accompanied by
a drawing reFlecting the requested revisions. Failureto provide written responses may result
in the delay of the review process.
3. It is critical that this office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please-provide revised plans including all revisions. Please notify us of
any changes and specifically state any changes, including changes resulting from Plan
Commission, BZA orothe~ committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review. If you have not already done so, please
provide a set of drainage plans and calculations to their office for review. We will share
Crossioad's comments ss they are received.
5. Final drawings will not be approved for construction until:
a. All Engineering Department and Utility Department and Hamilton County Surveyor
issues have been resolved.
b. All bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) are obtained.
d. All off-siteeasements necessary to install utilities to serve the development are secured.
e. SWPP is approved.
f All fees are paid.
6. The Department reserves the right to provide additional comments based upon subsequent
reviews.
7. An approved Storm Water Management Permit is required prior to.commencing any earth
disturbing activity.
8. An approved right-of--way permit is required prior to commencing any work in the public
right-of--way.
9. If it will be necessary [o relocate existing utility poles, they must be relocated to within one
foot of the proposed right-of--way. Costs for such relocations are Yo be borne by the
developer.
10. Compliance with the Thoroughfare Plan includesconstructidn of the improvements
prescribed by the Thoroughfare Plan across the frontage controlled by the development
(including sections with no access). Typically, developers provide a commitment equal to

the value to construct these improvnnents for deposit into the City's non-reverting
thoroughfare fund. Q~edit is typically eiven towards this commitment for any widening,
milling and resurfacing of the perimeter streets.
I I . The Department requires that the construction drawings be developed in accordance with the
City of Carmel digital submission standards and that all required submittals for primaryrplat,
secondary plat, and construction drawings be made. The digital files must be submitted to
the Department of Engineering prior to the approval of the constmction plans. Please contact
the City GIS Department for the requirements.
12. Jurisdictions:
a. The project site is located withincunent City of Carmel Corporate Limits.
b. Streets and Righi of Way -City of Cannel (4"'Avenue SE)
c. Water - City of Cannel Utilities
d. Sanitary Sewers -City of Carmel Utilities
e. Storm Sewers/Drainage -City of Cannel.
f ~ Legal Dmins -Hamilton County Surveyor's Office.
13. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require 9-sets of drawings for approval after all issues have been
resolved. The drawings will be stamped as approved and signed by the City Engineer
and by Cannel Utilities. The.Owner will receive 3-sets, one of which must be maintained
on the construction site at.all times. Ifthis project is subject to review and approval by
the Hamilton County Surveyor's Office, a total of 1 I-sets will be required for final
approval.
I4. Cannel Utilities will provide separate reviews of this protect for water and sanitary sewer
issues. Please assure that copies of all drawings are sent to:

I5. Carmel Utilities does subscribe to "Holey Moley" who should be contacted directly for all
water main locations.
16. The following items will be sent electronically upon request regarding this coiTespondence

d. Utility Jurisdictions/Right of Way Permits
e. Availability (acreage) Fees
BOARD OF PUBLIC WORKS AND SAFETY
17. A schedule.forBoard of Public Worksand Safety meeting dates and agenda deadlines will be
sent electronically for your use upon request. Please use the Engineering Department
deadlines for submissions to the Board.
18. Any submission to the.Board requires prior approval by the Carmel Clay Plan
Commission and/or [he Board of Zoning Appeals (if applicable) and completion of
review by the'1'echnical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Docket Number and the date (or dates) of
approval by the Plan Commission and/or the Board of Zoning Appeals (if applicable).
19. Water and Sewer Availability apptoyal from ttie Board is an EDIJ approval. Please contact
Cannel Utilities to determine if additional Water and Sanitary Sewer EDUs will be assessed
for this baildine addition.
20. Coinntercial Curb Cut Approval -This approval will not be applicable to this project.

21. Temporary Construction Entrance approval will be required to access the construction site
via the existing private drive curb cut on 41h Avenue SE.
22. The installation of any permanent. privately owned and/ormaintained improvement (signs,
decorative street signs, walls, streetlights, etc.) within dedicated right of way or dedicated
easements requires the execution of a Consent to F,ncroach Agreement between the Owner
and the City of Cannel. Such agreements are executed by the Board of Public Work and
Safety. The City Engineer may approve irrigation system agreements.
23. SecondaryPlai approval if applicable. All performance guarantees must be pvsted prior [o
submissionof secondary plats for Board of Public Works and Safety approval.
24. Dedication of Right of Way if not platted.
2i. Any open pavement cuts of existin_ 4'h Avenue SE will require Board approval.
BONDING REQUIREMENTS
26. Upon initial review, it appears performance guarantees shall be provided for the following:
a. Please contact Carmel Utilities to de[ertnine if a performance guarantee will be required
for the Sanitary Sewer modifications.
b. Erosion and Sediment Control and Stone Water Quality. Please contact Amanda Foley,
Engineering Department S[onnwater Administrator, fordetails regarding engineer's
estimates/performance guarantee requirements for these items.
27. The amountofthe Performance Guarantee is based upon a certified.Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer Pleaseprovide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
28. Upon completion and release of individual Performance Guarat[ees, a three-year
Maintenance Guarantee will be required (see Street Sign comments above). The
Maintenance Guarantee amount is based upon 15% of the Performance amowtt for Streets
and Curbs and 10"/0 of the Performance amount for all other improvements.
29. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
30. Please reference the enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
31. Any work in the dedicated right of way of 4'~' Avenue SE will require an approved Kight of
Way Permit and a License & Permit Bond. !t does not appear that this will be a requirement
for this project.
32. The bond amount is determined by our Right of Way Manager. However, if the work is
included in the scope of work of a required and posted Performance Guarantee, the
Performance Guarantee may be used [o satisfy the bond requirements of [he Right of R'ay
Permit.
33. Please contact our Right of Way Manager, Fred Glaser, to arrange right of way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
34. Please contact CanneLUtilitiesrggarding the application of Water and Sanitary Sewer
Connection Fees to this project. Water and Sanitary Sewer Availability (acreage) Fees will
not be applicable.
35. Connection Fees, if applicable, will be tequired to be paid after all other Engineering
Departmenfrequirements have been satisfied and prior to approval and start of construction
activities.

We have reviewed the construction plans submitted to the Hamilton County Surveyor's Office
on December 22, 2008, for this project and have the following comments:
1. The proposed project falls in the incorporated area and MS4 jurisdiction of the City of
Carmel.
2. The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone.
3. The proposed project does not fall in a Hamilton County Regulated Drain Watershed.
4. No permits will be needed from our office.
5. Please direct storm sewer and drainage questions to Carmel Engineering Department.
