revised Drainage Calculations dated November 1, 2004 based upon the Department's review of November
5, 2004. The comments from the November 5"' review and all previous Department of Engineering reviews
have been satisfactorily addressed.
When the items from the Department `s fax on October 28, 2004 have been addressed, the Department will
approve the drawings that have been submitted.

The City has received your letter and Phase 2 drawings dated October 29, 2004 based upon the
City review of October 26, 2004. The comments from the City review have been satisfactorily
addressed with the following exceptions:
1. The maximum design water surface elevation noted on Sheet 3.01 does not match the
Phase 2 elevation noted on Sheet C2.01. C2.01 states 100-YR = 870.2; C3.02 states
869.1.
2. There is an invert elevation and pipe slope discrepancy between the drainage calculations
and the plans for Pipe #12. Please verify the design diameter of this pipe and revise the
plans or drainage calculations as necessary.
3. The City would still expect to see additional spot shots on the grading plan that shows
additional spot spots, specifically for purposes of construction staking. Proper
implementation of the proposed drainage plan relies on accurate construction. Without
adequate information for the Contractor to use, the integrity of the design could be at risk.
If it is your opinion that the Contractor can construct the proposed improvements from
these plans, the City will be satisfied with the information provided as far as the approval
of the drawings. However, the City shall not be considered responsible for an improperly
constructed drainage system.
4. Thank you for adding the drive culvert and for relocating the pond outfal] pipe. The City
understands that in accommodating this request, the minimum pipe cover required for
pipes in the City right-of-way could not be satisfied without changing the pavement
section at this location. The proposed 6-foot wide reinforced concrete pavement section
over the piping does provide the minimum required cover, but results in the installation
of a strip of concrete. across the otherwise asphalt driveway. As this area is temporary
until Grand Boulevard is constmcted, the City is willing to waive the minimum cover
requirement. Please delete the proposed concrete strip and indicate the installation of the
right-of-way pavement section for the entire driveway
Provided that the above comments are addressed, the City does not anticipate any further
comments related to this project. Please provide our office with a follow up letter regarding this
review and how the comments were addressed.

When these revisions have been made and the items from the City's fax on October 28, 2004
have been addressed, please provide this office with a minimum of 4-sets of certified civil
construction drawings and a final drainage report for approval/signatures. The City Engineer and
the Director of Carmel Utilities will sign the sets. One set will be returned and it must be
maintained on the construction site at all times. If additional approved sets are desired, we will
approve an additional 2-sets, but they must be submitted with the required 4-sets.

Sheet C2.01 Master Site Plan
3. Comment on previous review: "Please label the multi-use path as an Asphalt Multi-Use path and
show the back edge of the path 1-foot inside die right-of-way line. Please change the hatching for
this path as it differs from what would be considered sidewalk." Please label as "Asphalt multi-
use path to be constructed as a part of Phase 3".
5. Comment on previous review: "What is the status of the right-of-way dedication? Was this
dedicated as a part of Phase 1? Dedication of this right-of--way will be a condition for obtaining a
building permit if it has not already been dedicated." Thank you. Please keep us posted on the
status. Right-of--way dedication requires Board of Public Works and Safety approval and a
building permit will not be issued until right-of--way is dedicated.

The City of Carmel (City) has received your letter and Phase 2 drawings dated July 22, 2004. The
following comments are provided based on the City's first review of this Phase of the project.
Sheet C2.01 Master Site Plan
1. Please provide a plan that shows only the Phase II construction. The Master Site Plan provided is a
great reference and should be left in the set.
2. Please provide more dimensions of the parking lot to the plan. Specifically radii dimensions.
3. Please label the multi-use path as an Asphalt Multi-Use path and show the back edge of [he path
1-foot inside the right-of--way line. Please chance the hatching for this path as it differs from what
would be considered sidewalk.
4. Please delineate the limits of the right-of-way pavement section and City standard straight curbing
on the proposed entrance.
5. What is the status of the right-of--way dedication? Was this dedicated as a part of Phase I?
Dedication of this right-of--way will be a condition for obtaining a building permit if it has not
already been dedicated.
6. Aright-of--way permit and bonding is required for the proposed driveway construction. Please
contact the City's right-of-\r ay inspector to obtain the pemut and for information on posting the
required bond.
Sheet C3.01 Phase I Grading, Utility and Erosion Control Plan
7. As Phase 1 has already been approved and assumed to be under constmction, please remove this
sheet from the drawing set. Has anything been changed on this sheet from the previously approved
drawing set?
8. The proposed rip-rap stilling basin is not the detail that the City was expecting. If the stilling basin
has not been installed at this point, please install per the detail provided by the City at prior
meetings.
9. Have you obtained Board of Public Works and Safety approval for the proposed open pavement
cut for the Sanitary Sewer connection? This approval is required for the work shown.
Sheet C3.02 Phasc 2 Grading, Utility and Erosion Control Plan
10. This Department recommends the installation of cleanouts at each of the bends for the sanitary
sewer lateral that runs to the grease trap. Please check with the Utility Department to see if
installation of cleanouts at each bend is a requirement.
11. This Department is of the opinion that the contours will not be able to be constructed as they are
drawn. Were these contours utilized to determine the pond volume available for detention storage?
If so, it is likely that the ponds would not be constmcted with the same effective area between
each contour line, resulting in inadequately sized ponds in the As-Built condition. .

12. The proposed entrance roadway, areas to the east of the entrance roadway, and areas north and
west of the on-site development are being allowed to direct discharge without being detained.
These areas need to be excluded from your calculations of the allowable release rate in the existing
condition if they have not already been excluded.
13. The maximum design water surface elevation noted on this sheet does not match the Phase 2
elevation noted on Sheet C3.01.
14. Please provide more spot shots and flow arrows for the parking lot, walks and areas around the
buildings.
15. Please install the pond outlet pipe at the location indicated on the Phase 3 drawing. This will
require a culvert under the proposed driveway.
Sheet C3.03 Phase 3 Grading, Utility and Erosion Control Plan
16. The Ciry will not review and approve this sheet at this time. The plan is contingent on the
available of City storm sewer. Please remove this sheet from the set.
17. 1s the church pursuing a drainage outlet into the Collins Drain in the event that the City storm
sewer is not available when Phase 3 is constmcted? In [he event that the City storm sewer was not
consmtcted, the City was anticipating the construction of a detention pond as a part of the Phase 3
construction that would have an outfall to the Collins Drain and an equalization pipe between the
Phase 2 and Phase 3 detention ponds at an elevation that would allow stored run-off for all rain
events above the SO-year event to drain equally between the two ouifalls. Since the Phase 3
drawings show no development or pond in the Collins Drain watershed, the City would not expect
the constntction of a third detention pond. However, the City would still expect an equal discharge
between the rivo outfalls for al] rain events above the SO-year event if the City sewer was not
available.
Sheet C4.01 Erosion Control Plan
18. Please show only the erosion control features pertinent to Phase 2 construction.
19. Detail 1 was not the Detail that the City was anticipating. Please replace with the Detail provided
by the City at prior meetings.
Sheet C5.01 Site Details
20. Please add aright-of--way pavement detail that requires the following pavement section: 1.5"
H.A.C. Surface #11 over 2" H.A.C. Binder #9, over 3" H.A.C. Base #SD over 7" Compacted #S3

Please show pavement over the trench (possibly in a second detail) and add a note that

24. Please provide drawings on 24"x36" sheets. The City's flat files will not accommodate the sheet
size submitted.
25. Crossroads has produced their drainage review comments. These were submitted to your office
under separate letter on August 24, 2004. Please address the comments and respond to Crossroads.
We reserve the right to provide additional comments on fi~rther reviews. Please provide our office with a
follow up letter and revised plans indicating all revisions. Second and subsequent reviews will focus only

Provide riprap-stilling basin in place of shave bales at the discharge point of the 12-inch pipe from the
parking area. This basin is to remain in place until the detention basin is installed.
General
Add, "Phase I Construction Only" to the Civil Cover Sheet, C0.01.
When these revisions have been made, provide this office with a minimum of 4-sets of certified civil
constmction drawings for approvaUsignatures. The City Engineer and the Director of Carmel Utilities will

This letter is in response to yow ADLS Application for SL Christopher's Episcopal Church. This is a preliminary letter to
let you begin on revisions. Please complete the following items after you have received additional comments made by
members at the TAC Meeting on October 15.
Site Plan•
1. Please label sign(s) distance from road R/W.
2. Please place adjacent land uses and zoning on zoning map.
3. Please add the boundary of the 600-ft US 31 Overlay as the S-2/Old Meridian Line.
4. Please place dimensions on buildings
5. A cwbed parking lot is required; curbing does not need to exist where futwe parking lot expansion will take
place. Please label "curbed asphalt parking" on site plan.
6. Please give a parking breakdown for classroom, fellowship, and worship buildings: school = 1 per employee
and 2 per classroom (elementary or 10/classroom (secondary), meeting hall= 1/200 square feet, and worship =
'b seats in main assembly.
7. Please label easements.
8. Please label buffer yards (See Sec 26.4).
9. Please show the location of mechanical equipment and trash receptacle with proper screening.

The Sanitary Sewer Avai[ability Fee is $1,365.00 per acre. The church orlgirtally paid (on Tune
20, 1994) Availability Fees on 9.25 acres. However, this was later adjusted to 6.6 acres due to the
fact that the church sold 2.6 acres to the State of Indiana for completion of the U.S. 3 ! project ST-
F-222 (9). The church was reimbursed for the value of the difference of 2.6 acres. Availability
Fees will be due on the difference between the origina16.6 acres and the total acreage of the
church expansion. W e note the legal description on the Cover Sheet of the drawings submitted
indicate a total of 10.144 acres more or less. irappears that this acreage is for the expanded site.
We also note there is a legal description For Tract "A" in the ADLS submission that indicates
9.553 acres. What is the difference?
G • Connection Fees are based upon the average user factor tables from Section 9-200 (sanitary) of
J our Utility Ordinance. The Connection Feos are $795.OD per EDU for sanitary sewers. Again, the
-church originally paid {on June 20, 1994) Connection Fees based upon a sealing capacity of 325
and 2.6 fiDU's. However, this was later adjusted to a capacity of 180 and 1.4 EDU's. The church
was reimbursed for the value of the difference of 1.2 EDU's. We will need usage information for
the expanded site in order to calculate sanitary sewer connection fees. I am enclosing copies of
our Average User Factors for Sanitary Sewers and our aerrent fee schedule. [have also
highlighted the Average User Factors that might apply to this development.
Note: Any deviation from the Connection Fe¢ calculations listed in our Ordirzauce wiU require
Carmel UtUities med Board of Publk Works and Safety approvals.
4. TAC Review/Drawings submitted for approval:
We request that all comments and corntnent letters generated by this office be answered in writing and
be accompanied by a drawing reflecting requested revisions. Final drawings will not be approved for
construction until all issues have been resolved. All Carmel Ufitity issues must also re resolved. The
design engineer must certify all drawings submitted for frnal approval. This office will require a
minimum of four-sets (maximum o fsix-sets) of approved drawings after all issues have been resolved,
The drawings will be stamped approved and signed by both the City Engineer and Director Carmel
Utilities. The Developer/Owner will receive one-set wlmich is to be maintained on the construction site
at all times. Carmel Utilities will receive one-set. Our Public Works inspector will receive one-set
and ono-set will be maintained in the Department of Bngineeriug. If more titan the minimum of four-
sets is requested to be approved, the additional sets (maximum total of two additional) must be
submitted with the four required sets.
5. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
easements andlor right of way will require a Consent to Encroach Agreement between the
' Owner/Developez and the City of Carmel. This Agreement requires Board of Public Works and Safety

The technical review and comments are intended to evaluate the completeness of the erosion and sediment control plan for tl~e
The erosion and sediment control plan submitted was not reviewed jor the adequacy of the engineering design. All
included in the plan, as well as those recommended in the comments should be evaluated as to their feasibility by a
qualified individual with structural practices designed by a qualifzed engineer. The plan has not been reviewed for local, state,
or federal permits that may be required to proceed with this project. Additional information, including design calculations may
be requested to further evaluate the erosion and sediment control plan.
The erosion and sediment control plan has been reviewed and it has been determined that the plan:
X Satisfies the minimum requirements and intent of 327IAC 15-5 (Rule 5). Notification will be forwarded to the
Indiana Department of Environmental Management.
Proper implementation ofthe erosion and sediment control plan and inspections of the construction site by the developer or a
representative are necessary to minimize off-site sedimentation. The developer should be aware that unforeseen construction
activities and weather conditions might affect the performance of a practice or the erosion and sediment control plan. The plan
must be a flexible document, with provisions to modify or substitute practices as necessary.

I have reviewed the site plans and ADLS for St. Christopher's Episcopal Church, submitted to
our office on September 19, 2003, and have the following comments:
1. The site for the proposed church falls within the City Limits of Carmel. It appears that a
portion of the site may fall within the J.R. Collins watershed. The proposal is to take all on-
site and off-site water to the 131" Street road site ditch. Please coordinate the proposed
drainage with the City of Carmel Engineering Department.
2. Tt appears there may be some discrepancy on the location of the J.R. Collins regulated drain.
Please field locate the drain near the proposed development and correctly show the location,
including the easement, on all site and utility plans,
3. Please note that fitrther comments may be necessary at a later date.

This letter is in response to your ADLS Application for St. Christopher's Episcopal Church. This is a preliminary letter to
le[ you begin on revisions. Please complete the following items after you have received additional comments made by
members at the TAC Meeting on October 15.
Site Plan
1. Please label sign(s) distance from road R/W.
2. Please place adjacent land uses and zoning on zoning map.
3. Please add the boundary of the 600-ft US 31 Overlay as the S-2/Old Meridian Line.
4. Please place dimensions on buildings
5. A curbed parking lot is required; curbing does not need to exist where future parking lot expansion will take
place. Please label "curbed asphalt parking" on site plan.
6. Please give a parking breakdown for classroom, fellowship, and worship buildings: school = 1 per employee
and 2 per classroom (elementary or 10/classroom (secondary), meeting hall= 1/200 square feet, and worship =
'/. seats in main assembly.
7. Please label easements.
8. Please label buffer yards (See Sec 26.4).
9. Please show [he location of mechanical equipment and trash receptacle with proper screening.
10. Per the ATP, a 10-ft separated pazallel multi-use path is required along 131 s` Street; walkways are only 8-ft.
11. Streetlights must be spaced no les than 100 feet apart. Please show along 131" St.
12. What is the height of the light lamp? Must be less than 25 feet.
13. Foot-candles must be no greater than .5 at [he property line. Also show distribution south to 131 St.
14. Show parking spaces as being 9' x 20' or 10' x 18'.
15. Bicycle parking shall be provided at 1 space per 100 feet of building frontage (20G.04.05-G).
16. Buildings must be faced in brick or stone with trim in other materials (20G.05.06-D.(2)).
17. Please show a 6-ft landscape strip around parking [ha[ is adjacent to the street.

i . Mr. Fleck presented changes made to the proposed site plan in response to Mr.
Dobosiewicz's comments in his Apn7 22 meeting with Mr. Fleck (Drawings dated May 30,
2003). Affer review of the proposed plans and reference to the Cannel/Clay Zoning
Ordinance and Old Meridian Sheet DisMct regulations, Mr. Dobosiewicz indicated overall
satisfaction with the changes and wflh the design.
1.1. Mr. Dobosiewicz said that he would support administrative approval for gravel
surfacing and the omission of curbs for the temporary drive between the current
parking lot and the phase 1 and 2 parking lot, so long as it was submitted with a
proposed time by which R would be improved to city standards or removed.
1.2. Mr. Dobosiewicz said that the parking ds~ drawn is within the requirements of the
ordinance, except for four to sa spaces wtych are south of the fronT line of the phase
two terrace. He said he would support allovdng those parking spaces by
administrative waiver, rather than requiring a variance.
1.3. Mr. Dobosiewicz said that the bcafions of the curb cuh onto Main Street are
acceptable as drawn, with the following cjuafifications:
1.3.1. When the west curb cut is developed, the church's existing curb cut would be
dosed. -
1.3.2. When Main Street is improved and center idonds developed, the west curb
cut maybe requced to be right-tumin~ right-ium-out only (no break in the center

developed area north and west of the church site.

north and west parcel, the City of Carmel wpuld support rezoning of this parcel to
B-5.
4. The meeting was adjourned at 11:30 AM. Mr. Flepk secured the necessary forms and-wit
prepare them for filing in advance of the relevant deadlines.
