Petitioner, please note the following:
1. This item will be on the March 17 meeting agenda of the Technical Advisory Committee (TAC). Plans should be
emailed and /or delivered to all TAC members ASAP, if not done so already. The TAC members list is online at:
http: /www.ci.carmel.in.us/ services /DOCS/TAC2009/ TAC %20members %207.08.09.pdf (Failure to submit plans
and review materials to TAC members will result in the tabling of this petition.)
2. Mailed and Published Public Notice needs to occur no later than Friday, March 26. Published notice is required
within the Indianapolis Star. Get the public notice ad to the IndyStar by Noon, two days prior to the public notice
deadline, since it takes about two days for them to publish. Also, the placement of a public hearing sign on the
property is required; see application.
3. The Filing Fee and Fifteen (15) Informational Packets must be delivered to Plan Commission Secretary
Ramona Hancock no later than Noon, Friday, April 9.
4. Proof of Notice will need to be received by this Department no later than Noon, Friday, April 16. Failure to
submit Proof of Notice (green cards, newspaper affidavit, etc.) by this time will result in the tabling of the petition.
5. This item will appear on the Tuesday, April 20 meeting agenda of the Plan Commission (under "Public
Hearings A representative must be present.
6. This item might also appear on the Wednesday, May 5, meeting agenda of a Plan Commission Subcommittee.
A representative must be present.
7. Refer to your instruction sheet and checklist on the application for more detail.
Preliminary Review Comments from the Planning/Zoning Dep't: (Please reply to each of the review comments via
email or letter correspondence before April 9.)
1. Remember to include the Findings of Fact sheet in your final plan commission info packets.
2. Please include photos of the existing building in the plan commission info packets.

3. Provide the filled out and notarized affidavit of notice of public hearing page of the application.
4. Provide the filled out Notice of Public Hearing page of the application.
5. Provide the filled out and notarized Public Notice Sign Placement affidavit page of the application.
6. Provide a copy of the Official List of Adjacent Property Owners from Hamilton County Auditor's Office.
7. Please provide copies of your correspondence with the TAC members and their correspondence with you.
8. Bring material /color samples to the Plan Commission meeting.
9. Attached is the Carmel Green Building checklist to use as a guide. Please consider using additional 'green'
building /site practices.
10. Please label the building dimensions on the site plan, sheet C200.
11. Please label the mechanical equipment on the site plan or building elevations, and show details on how it is
screened.
12. Please provide a Lighting Plan with photometrics, and provide design details /specs any exterior lights.
13. Will there be any new signage? If so, please provide details on the location, material, design, and color.
