Please add docket no. 09110039 SP and add a location map to the plat. 2. Please note that SDR review for each lot was part of the approval of the variances and will be required before building permits are issued. 3. Please show Rep/at of CW Wiedler's Sub Division, lots 69 71 as the main title of the plat and show Blackwell Park as the secondary title. 4. Add the source of title of ownership to the plat. 5. Please add the 45 %maximum lot cover percentage (allowed by variance) to the plat. 6. How will the road transition from the 40 -ft r/w to the 50 -ft r /w? Why did that width increase since the plan commission approval of the primary plat? 7. Verify aggregate side yard setback to make sure they comply with the R -2 standards of 15 -ft. 8. Provide a copy of the signed subdivision agreement form, as found in section 9.06 of the subdivision control ordinance. 6 )4 9. Please provide copies of all TAC members' correspondence with you. 10. Please provide a copy of the tree preservation plan and its text to the Forestry Dept and the Planning /Zoning Dept. 11. Please provide a copy of the final covenants and restrictions. 12. Once these changes have been made and comments addressed, please produce a Mylar copy (with the surveyor's and land owner's signatures) for signature by this Department. Two paper copies of the plat must be filed with the Mylar prior to recording, and two paper copies and a Mylar of the recorded plat must be submitted following recording of the document. Our approval of the secondary plat (signature) will be forthcoming upon revision of the plans according to the items listed above. 14. The subdivision shall consist of 8 single family residential lots and a common area. It is proposed to extend 3 Ave. NE north to serve the development. The subdivision shall contain public water and sanitary sewer, storm sewer and all other necessary facilities. We are respectfully requesting to be a part of the December 16 Technical Review meeting.
