It is the following: ADLS Amend Application Fee: $298.00 per sign fee (59.50 x 1): $59.50 Total Fee: $357.50 (PAID) Docket No. 10080012 ADLS Amend: Generations in Dentistry Signage The applicant seeks approval to replace an existing ground sign with a new sign. The site is located at 13331 Illinois St. and is zoned B -5. Filed by Jeff Gibbons of Pro Med Signs and Doug Peet of Generations in Dentistry. Petitioner, please note the following: 1. This Item will not be on an agenda of the Technical Advisory Committee. 2. Mailed and Published Public Notice does not need to occur. 3. Proof of Notice is not needed. 4. The Filing Fee (PAID) and Eight (8) Information packets must be delivered to Plan Commission Secretary, Ramona Hancock, no later than 12:00 PM, Friday, August 27, 2010. If filing fee and materials are not delivered by this time, this application will be continued to the October 5, 2010 meeting. 5. The rendering of the sign with dimensions and lighting information 4. Site plan 5. Photos of existing sign 7. The Department has the following outstanding concerns /comments: 1. Photographs are not permitted on identification signs. The definition of Identification Sign is: "A Sign which is limited to the name and /or address of a building, institution or person; to the activity carried on in the building or institution; the occupation of the person; and /or the logo." Please remove the photo of the woman and girl and resubmit before packets are due Friday, August 27th. This item may be tabled to the October 5th meeting if a new design can not be submitted by August 27th 2. Please submit more detailed information on what material will be used for the base of the sign. 3. Please submit which landscaping materials will be used around the sign. 4. Please submit more information on which parts of the sign will be illuminated. We ask if only the text of an internally illuminated ground sign shows through.at night, not the entire panel. This helps with aesthetics and does not give the full visual appearance of a "cabinet" sign. Mr. Gibbons can be reached at 800 800 -2193. 8/24/2010 Page 2 of 2 Thanks, Zachel/3aone, Sign Reviewer City of Carmel Department of Community Services 3rd Floor One Civic Square Carmel, IN 46032 317.571.2417 317.571.2426 fax http://www.ci.carmel.in.us/services/communityservice.html Please consider the environment before printing this e -mail 8/24/2010
