Kittle JAMES BRAINARD, MAYOR innovative Engineering and Consulting, Inc. 4937 Fieldstone Drive Whitestown, Indiana 46075 RE: Auto Zone Store #4533 Project Review #1 Supplemental Comments Dear Mr. Kittle: Please note the following supplemental comments: 1. Have you confirmed with 1NDOT the need for additional right -of -way across the Michigan Road frontage? 2. Please confirm that the detention is off -site and master planned and that the proposed "C" value conforms to that value anticipated during master planning. 3. The Department is of the opinion that the easternmost curb cut is too close to Michigan Road. Please indicate why two curb cuts are necessary to serve this site. 4. Please indicate 30 -foot minimum return radii at the entrances. 5. 30 -foot is the maximum width for commercial curb cuts. 6. Please confirm if there is curbing along the north side of Northwestern Drive. If there is not curbing, the City requests that curbing be installed as a part of this project and that the portions of the entrances within the right -of -way be curbed and depressed curbing be installed across each entrance. 7. Please hatch the portion of the proposed entrances in the right -of -way to be concrete. 8. Please verify with the DOGS the need for sidewalk or path across the Northwestern Drive frontage. 9. Please replace Details I and 2 on Sheet CLA with City standard details. I0. Please revise Detail 4 on Sheet CLA to require 6 -inch minimum thickness concrete for paving within the right -of -way. This thickness shall be increased if the largest vehicle expected to regularly accesses the site warrants a thicker pavement section. 11. Please include City standard ADA ramp details for any ramps that are required in the right -of -way. 12. Please include the City standard sidewalk detail if the DOCS requires the installation of a sidewalk along the Northwestern Drive frontage. If the City detail differs from the current detail, the different details shall be differentiated on the site plan. 13. Please include the City standard path cross section if the DOCS requires the installation of multi- use path across the Northwestern Drive frontage. 14. All BMP's shall be contained in easement per Section 701.07 of the Storm Water Technical Standards Manual. 15. Please indicate the on -site flood route. This flood route and resulting water elevations shall be considered when establishing the MFPG and MLAG elevations. 16. Please establish the MFPG and MLAG elevations and include the definitions for each in the plan set. 17. Please confirm if the building will be a slab on grade. 18. Will the 90- degree turn in the swale at the north end of the eastern parking area be problematic in the long term? 19. If the detention system is remote, what is the flood route from this property to the detention facility? If not, please revise Note 12 on Sheet C1.3. 21. The following note shall be added to all appropriate sheets. "ALL PAVING WITHIN THE EXISTING AND PROPOSED CITY RIGHT -OF -WAY SHALL CONFORM TO THE REQUIREMENTS OF THE DEPARTMENT OF ENGINEERING. THE CONTRACTOR SHALL CONTACT THE DEPARTMENT OF ENGINEERING TO SCHEDULE A PRE CONSTRUCTION MEETING TO REVIEW THE DEPARTMENT'S CONSTRUCTION REQUIREMENTS, STAFF NOTIFICATION REQUIREMENTS, REQUIRED INSPECTIONS FOR CERTAIN STAGES OF THE WORK AND TO REVIEW THE AUTHORITY OF THE DEPARTMENT AS IT RELATES TO WORK WITHIN THE EXISTING AND PROPOSED RIGHT -OF- WAY." 22. Please add the following note to Sheet C 1.5: "The storm sewer system shall be constructed per design specified and as approved by the City of Carmel on the final approved construction plans. Deviations from the approved design shall only be permitted due to special circumstances or difficulty during construction and will require prior field approval from a designated representative of the City of Carmel in addition to supplemental approval by the design engineer. An explanation of any such deviation shall be included as a requirement on as- built/record drawings submitted for release of performance guarantees. Approved design slopes identified as generating velocities of 2.5 fps or less or 10 fps or greater (at full flow capacity) shall require as- built certification at the time of construction, prior to backfiiling the pipe. The contractor is instructed to as -built each section of storm pipe as it is being installed to ensure compliance with the design plans and as approved by the City of Carmel." 23. Please provide a maintenance of traffic plan for the construction of the entrances. 24. Please rep] ace Detail 2 on Sheet C 1.7 with the City standard detail. 25. Please revise Detail 11 on Sheet CL7 to include a dual wall sub surface drain. All swales are required to have a sub surface drain. Subsurface drain shall be Double Wall Hancor Hi -Q or equivalent. 26. Please revise Section 03000 on Sheet C1.9 to require paving in the right -of -way to conform to the City's paving policy. 27. Please include the City's curbing and paving policies in the plan set. 28. Please indicate the zoning of the property and the surrounding properties. 29. Please indicate any setbacks required by zoning. 30. Is any portion of the irrigation system proposed to be installed within the right -of -way? 31. Please indicate the Panhandle Eastern Pipeline Easement on all sheets. 32. Are all the easements shown existing easements? If they are not, the City prohibits landscaping easement to be overlapped by utility or other similar easements. 33. Please confirm that all known utilities on the property and within the adjacent right -of -way are shown and that all TC's, IE's, pipe sizes and flow directions are noted. 34. Please provide a drainage summary that conforms to the following: "A drainage summary, which summarizes the basic conditions of the drainage design, including site acreage, off site /upstream acreage, allowable release rates, postdeveloped 10 -year, and 100 -year flows leaving the site, volume of detention required, volume of detention provided, and any release rate." Please contact this office if you have any questions regarding this comment. 35. Please confirm if the proposed entrances were included in the master plan of this development. 36. Please confirm if any direct discharges are proposed. 37. City standard sub surface drain is required under all curbing in the right -of -way. 38. With the exception of storm sewer inlets, new frames and castings cannot be located within pavement, curb, drive aprons, path, or sidewalk within the right -of -way. 39. Curb joint spacing shall be 10 -foot maximum on tangent sections and 5 -foot maximum on radii. Expansions joints shall be placed a maximum of every 50 feet. 40. Any existing curbing demolished as a part of this project shall be to the nearest existing joint. A note to this effect shall be added to the plans. Mr. Jerry W.
