The applicant seeks approval to update the sign package and add an awning over the new Davita Dialysis entrance.
The site is located at 118 Medical Dri and is zoned B -8.

Petitioner, please note the following;
1. This Item will not be on an agenda of the Technical Advisory Committee.
2. Mailed and Published Public Notice does not need to occur.
3. Proof of Notice is not needed.
4. The Filing Fee and Eight (8) Information packets must be delivered to Plan Commission Secretary, Ramona
Hancock, no later than 12:00 PM, Wednesday, December 22, 2010. If filing fee and materials are not delivered
by this time, this application will be continued to the February 1, 2011 meeting.,
5. This Item will appear on the January 4, 2011 agenda of the Plan Commission Special Studies Committee at
6:00 pm in the Caucus Room, Second Floor, City Hall. Please have a representative attend.
6. Petitioner, please address the following items:
1. The directory sign must. be 5' tall or less.
2. Please provide the dimensions of the copy area of the main ground sign for Carmel Rehab.
3. The Davita entrance looks cramped with the parking spaces right up to the edge of the door. Will the
parking configuration change at all with this new entrance?
4. Please provide more photos showing a wider angle view of the entrance so we can see how the awning
addition will look compared to what's there now
7. Petitioner, please include the following information in your packets (staple the packets no fancy
bindings por binders required):
1. .Pages 1 of the. ADLS Amendment application
2. The rendering of how the signs will (and currently) look on the property
3. A rendering of each sign with dimensions
4. Site plan (or aerial photo, i.e. Google or Yahoo maps) showing where your tenant space is within the
building
5. Photos of existing signs
