RE: Regions Bank Construction Plans Project Review #1
Dear Mr. Kelly:
The City received your construction plans on February 10, 2011. We offer the following comments:
GENERAL INFORMATION
1. These comments represent the Department of Engineering's first review of the construction
plans for this project.
2. We request that all responses to our present and future comments be provided in writing and
be accompanied by drawings reflecting the requested revisions. Failure to provide written
responses may result in the delay of the review process.
1 It is critical that this office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our
previous reviews. Please notify us of any changes and specifically state any changes,
including changes resulting from Plan Commission, BZA or other committee meetings.
4. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review.
5. Final drawings will not be approved for construction until:
a. All Carmel Engineering Department, Carmel Utility Department, Hamilton County
Surveyor's Office and Hamilton County Highway Department issues have been resolved.
b. All bonds and performance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) are obtained.
d. All off -site easements necessary to install utilities to serve the development are secured.
e. Any required fees are paid in full.
f. Required digital files have been submitted.
6. The Department reserves the right to provide additional comments based upon subsequent
reviews or as more detailed infonnation is submitted.
7. An approved Storm Water Management Permit is required prior to commencing any earth
disturbing activity.
8. An approved right -of -way permit is required prior to commencing any work in the public
right -of -way.
9. if it will be necessary to relocate existing utilities or other infrastructure, the costs for such
relocations are to be borne solely by the developer. Any utility poles required to be relocated
within the right -of -way shall be relocated to within one foot of the proposed right -of -way.
10. The Department requires that the construction drawings be developed in accordance with the
City of Carmel digital submission standards and that all required submittals for primary plat,
secondary plat, and construction drawings be made. The digital files must be submitted to
the Department of Engineering prior to the approval of the construction plans. Please contact
the City GIS Department for the requirements.
IL Jurisdictions:
a. The project site is located within current City of Carmel Corporate Limits.

12. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require a minimum of 11 sets of drawings for approval after all issues
have been resolved. The drawings will be stamped as approved and signed by the City
Engineer and by Carmel Utilities. The Owner will receive 3 approved sets, 1 of which
must be maintained on the construction site at all times.
13. Carmel Utilities will provide separate reviews of this project for water and stntitary sewer
issues. Please assure that copies of all drawings and revisions are sent to:

Failure to do so may result in the delay of final approval of the construction plans and the
project.
14. Cannel Utilities subscribes to "Holey Moley" who should be contacted directly for all water
main and sanitary sewer main locations.
15. Digital versions of the following information are available upon request. Please share these
requirements with the developer and future contractors associated with the project:
a. Project Approval Checklist
b. Performance /Maintenance Guarantee Requirements
c. Performance /Maintenance Guarantee Release Procedure
d. Engineering Requirements for Building Permit Approval
e. Commercial Permitting Procedures
f. Utility Jurisdictions /Right of Way Permit
g. Water and Sanitary Sewer Fees
BOARD OF PUBLIC WORKS AND SAFETY
16. A schedule for Board of Public Works and Safety meeting dates and agenda deadlines is
available upon request. Please use the Engineering Department deadlines for submissions to
the Board.
17. Any submission to the Board requires prior approval by the Carmel Clay Plan
Commission and /or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Docket Number and the date (or dates) of
approval by the Plan Commission and /or the Board of Zoning Appeals (if applicable).
18. Water and Sewer Availability (EDU) approval from the Board is based upon the usage of the
proposed buildings within the development. Reference items #32 through 434 below for a
more detailed explanation. If an irrigation system is planned for this development, approval
for additional Water Availability must be obtained from the Board.
19. Commercial Curb Cut Approval 146 °i Street is under the jurisdiction of the Hamilton
County Highway Department. All curb cut requests should be directed to their attention. It
is our understanding that Lowe's Way is still under Hamilton County Jurisdiction and has not
been transferred to the City of Cannel. That being the case, all curb cut requests should also
be directed to the Highway Department. If this is not the case regarding Lowe's Way, then
the BPWS would have approval authority over any curb cuts or right of way issues on
Lowe's Way.

21. Should it be determined that Lowe's Way is under City of Carmel Jurisdiction, the
installation of any permanent, privately owned and /or maintained improvement (signs,
decorative street signs, walls, streetlights, irrigation systems, etc.) within dedicated right of
way or dedicated easements requires the execution of a Consent to Encroach Agreement
between the Owner and the City of Carmel. Such agreements are executed by the Board of
Public Works and Safety. The City Engineer, representing the Board, may approve irrigation
system agreements.
22. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats for Board of Public Works and Safety approval.
23. Dedication of Right of way if applicable. Appropriate r/w dedication documents are
available upon request.
24. Any open pavement cuts of 146` Street or ofLowe's Way will require Hamilton County
approval.
25. Closure of the existing path on 146 Street will also require Hamilton County approval.
BONDING REQUIREMENTS
26. Please contact Mr. Dave Barnes to review performance guarantee requirements. Please
contact Mr. John Duffy to review water and sanitary sewer bonding requirements.
27. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes and materials, etc.
28. Upon completion and release of individual Performance Guarantees, a three -year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based upon
15% of the Performance amount for Streets and Curbs and 10% of the Performance amount
for all other improvements.
29. Performance Guarantees may be Perfonmance or Subdivision Bonds or Irrevocable Letters of
Credit.
30. Please reference the available enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
31. Since all dedicated right -of -way is under Hamilton County jurisdiction, all Permits and
Bonding must be obtained from them.
AVAILABILITY AND CONNECTION FEES
32. We defer to Cannel Utilities regarding this issue.
33, If an entryway or overall site irrigation system is planned for this development,
additional Water Connection Fees will be assessed based upon the size and usage of the
system and upon the recommendations of the Director of Carmel Utilities.
34. These fees are required to be paid prior to final approval of construction plans by
Engineering and prior to issuance of building permits by Building Code Services. Please
confirm these fees and calculations with Carmel Utilities.
CONSTRUCTION PLAN REVIEW COMMENTS
35. General Comments
a. This project is subject to the City's Storm Water Management and Storm Water Quality
Ordinances.
b. All sheets shall be stamped "Approval pending -not for construction
c. Please verify that all existing easements on -site are indicated on the plans. Are any tree
preservation or landscaping easements proposed?
d. Please verify that all known above and below grade utilities and all known drainage
infrastructure (including bridges, culverts and any other drainage structures) are indicated

on the plans. Top casting elevations shall be noted. All gravity systems shall include
information as to pipe size, invert elevations and direction of flow.
e. Please indicate both existing and proposed right of way lines and dimension on all
appropriate sheets.
f. Please indicate how Item 1 of email dated 12/22/08 will be addressed? Will we be
provided these plans?
g. Please confirm that Item 2 of email dated 12122/08 has been addressed.
h. If approval from IDEM is needed for construction in Lowe's Way Right -of -Way: Please
provide.
i. Please indicate that the existing pavement in Danbury Drive shall be saw cut to a clean
edge before work.
j. Proposed BMP's shall be contained in easements per Section 700 of Storm Water
Technical Standards Manual.
k. All swales required dual wall SSD unless such SSD will impair function of BMP if swale
is intended to function as a BMP. If required, please indicate on all swale details.
1. Please provide principal dimensions from property lines to building.
m. Key note 25 on Sheet C1.1 Detail 3/C4.1 references integral curb and walk.
36. Sheet C2.0
a. Please delete Article 15 of Section 02510.
37. Sheet C2.1
a. Is a curb turnout required for the area of Danbury Drive that is not being extended?
b. Please update the drainage summary on sheet C2.1. The summary references
Townhomes At Providence.
38, Sheet C3.1
a. Please confirm that all known utilities are indicated on this sheet. Please label all top of
castings, invert elevations and pipe sizes,
39. Detail 2 /Sheet C4.1
a. Please replace with City Standard Details
40. Detail 3 /Sheet C4.1
a. Please indicate an addition 4" of buried curb to satisfy City Standard Standing Curb
Detail.
b. Please include City Standard Sidewalk Detail if the Department of Community Services
has approved the proposed sidewalk in Lowe's Right -of -Way. If this facility is to be
asphalt path, please add City Standard Path detail.
41. Detail 6 /Sheet C4.1
a. City Standard detail required in right -of -way and specifically referenced on site plan if
different ramps used on -site.
42. Section 02528, Sheet CS.4
a. Please revise to remove all curbing and gutter specifications. This work shall follow
City curbing policy.
