Development in order to amend the breakdown of unit types. The site is located northeast of 116 St Guilford Rd and is

Petitioner, please note the following:
1. This Item does not need to be on agenda of the Technical Advisory Committee.
2. Mailed and Published Public Notice needs to occur no later than Dec. 25. Published notice is required within the
Indianapolis Star. Try to get the public notice ad to the IndyStar by Noon, two days prior to the public notice
deadline, in order for them to publish it on time. Also, the placement of .a public hearing sign on the property is
required; see application.
3. The Filing Fee and Fifteen (15) Informational Packets must be delivered to Plan Commission Secretary
Ramona Hancock no later than. Noon, Friday, Jan. 7, 2011.
4. Proof of Notice will need to be received by this Department no later than Noon, Friday, Jan. 14, 2011. Failure
to submit Proof of Notice by this time will result in the tabling of the petition.
5. This item will appear on the WEDNESDAY, Jan. 19, 2011 meeting agenda of the Plan Commission (under
"Public Hearings which begins at 6 pm. (A representative must be present.)
6. This item might also appear on the Feb. 1, 2011 agenda of the Plan Commission Special Studies Committee,
which begins at 6 pm. (A representative must be present.)

Statement of compliance with the Carmel /Clay Comprehensive Plan (use additional pages if
necessary): The property is presently zoned Park Place Planned Unit Development District "PUD
and will remain as zoned: The PUD Ordinance adopted 2/9/07 by the Carmel City Council and
amended March 2, 2009, by Ordinance Z- 526 -09, includes paragraphs 5.1(e) "There shall be a
maximum of one hundred seventy -one (171) independent residential living units, sixty -two (62)
assisted living units, and twenty -eight (28) nursing care units." The Applicant seeks to amend
paragraph 5.1(e) as follows: "There shall be a maximum of one hundred forty -one (141) independent
residential living units, fifty -six (56) assisted living units, twenty -six (26) memory support assisted
living units and thirty-eight (38) nursing care units." The Applicant may vary the unit mix up to 10%
so -long'as the total units does not exceed two hundred sixty -three (263) units.
