PLEASE NOTE Please be advised that "Additional Services" performed on City contracts must be billed separately from the original contract and other additional services. More importantly, the invoice must reference the appropriate Additional Service and P.O. shown above. Invoices received without the Additional Service referenced will be returned for proper identification. However, due to the water main work (which needed to proceed the path work) not being completed as scheduled, this project was started late in the 2010 construction season and was not completed. Therefore, there has been some work over the winter and the contractor intends to work all of March...and none of these days count against the contract. Therefore, we hereby respectfully request a supplemental fee of $35,000 to complete the construction inspection for this project. Please see the attached breakdown to support this request.
