Please add docket no. 10080018 HPR to the document. The docket no. has been included on the plans /plat. 2. Please add a location map. This map has been included on the plans /plat. 3. Please add the zoning to the plat: C -1 /City Center District. This designation has been included in the plans /plat. 4. Please make sure all instrument numbers are filled in, before recording the documents. Before recording, we will include the requested instrument numbers. 5. Please provide a copy of the recorded declaration of covenants and restrictions for City Center, if possible. As soon as we record the declarations, we will provide a copy of this document. 6. On page 1, the Certificate of Plan Commission should be amended to read something like this: Certification of Zoning. On Oct. 17, 2008, the Carmel Plan Commission Hearing Officer granted approval for this multi -story multi -use building project, under Docket No. 07120001 DP /ADLS, on this site. I have reviewed the condominium plat of Carmel City Center Condominiums and verify that it is consistent with the aforementioned approval. Plan Approved by: Michael P. Hollibaugh, Director of Department of Community Services, Carmel, IN. [date]. This certification has been included on the plans /plat. 7. Please remove the board of public works signature block from page 1, as it is not required. (I'm verifying this, but am pretty sure it's not required.) This signature block has been removed. 8. On Page 19, please add the Certification of Addressing signature block. It should read something like: I hereby certify on behalf of the City of Carmel, Indiana that the street names set forth herein are consistent with those approved by the Carmel Plan Commission docket no. 07120001 DP /ADLS; and that the addresses set forth herein are consistent with those assigned by this Department. Plan approved by: William Akers, Dept. of Communications, [date]. We have included this on the last page of the plan /plat (near a detailed listing of the addresses). 9. Provide electronic copies of the application and plat, as .pdf or .jpg files. I will provide this information later today. 10.
