10071.01 Subject: Addendum #1 Date: June 9, 2011 Date Distributed: ADDENDUM #1: This Addendum forms a part of the Contract Documents issued for UPA Pediatric G.I. Clinic and Infusion
Center (Project No. 10071.01) and modifies the original Construction Drawings and Specifications dated 5/19/11 as noted below. General: Item G-1: All deliveries through the building
dock and must have a representative from the GC be present to receive materials. The dock manager does not receive materials nor will any materials be accepted and or stored on the dock
for any reason without an attendant present. Item G-2: The service elevator is to be the only elevator used to come and go from the floor for all workers. Item G-3: All deliveries to
the project site must be made before 7:00 am or after 3:30 pm. See item G-1 for additional information regarding deliveries. Item G-4: Any demolition or build back work that has the
potential to disrupt the practices below, next to or above should be scheduled for after hours work. If any complaints are received from tenants or hospital staff for a specific task
that is being performed the contractor will be requested to finish after hours. Item G-5: Any work that involves tools, equipment or labor to be within the common area hallways, restroom
or elevator lobby will have to be after hours (no exceptions). Item G-6: All fire systems work has to be approved ahead of time and coordinated with Hospital and MOB maintenance staff
(no exceptions). Item G-7: All contractors will be required to have all crews working on site to go through a building safety orientation program. Item G-8: The project is not sales
tax exempt. Item G-9: Infection control measures are required via an Infection Control Risk Assessment Report (ICRA) to be completed by the hospital and coordinated by the selected general
contractor. Minimum requirements include but are not limited to: HEPA air filtration, negative air environment w/gauge and daily documentation, walk off mats, containment walls as needed
to contain all dust within the work area, daily vacuum and trash haul (all trash hauled in a covered trash cart). Item G-10: There will not be a common allowance amount for Infection
Control for the project. Each contractor is responsible for estimating the work required in Item G-9 above.

such bid must be received in a sealed envelope by Kristy Carter with Hokanson Building Management, no later than 2:00 PM, Tuesday, June 14, 2011. Bids are to be turned in to the following
address: Hokanson Companies, Inc. 201 West 103rd St., Suite 400 Indianapolis, IN 46290 Architectural Item A-1: See attached sketch SK-A1 removing the full height shelving in Hallway
201B. In its place, please provide a 24”deep countertop at 42”A.F.F. Countertop finish to be PL-3. See attached sketch SK-A3 for casework elevation. Provide a grommet in the top of the
work surface and add (1) quadraplex receptacle and (1) dual data & phone jack under counter. Item A-2: See attached sketch SK-A2 modifying the wall layout on the south side of Check-In/Check-Out
202 and adding a 24”deep work surface west of door 202A. Work surface finish to be PL-3. See attached sketch SK-A4 for casework elevation. Provide a grommet in the top of the work surface
and add (1) quadraplex receptacle and (1) dual data & phone jack under counter. Item A-3: Built in exam room casework (countertop, leg, and upper cabinets, shown in detail 5F/A600) is
to be deleted in its entirety. There are (7) instances of this casework to be removed from the project scope. Item A-4: Detail 1C/A701 and detail 6C/A750 are no longer used in the project.
Item A-5: All plastic laminate countertop supports are to be changed to PL-3. Item A-6: Norton 7500/7700 Series Institutional Door Closer is an acceptable alternative. MEP Item M-1:
Fire Protection System branch piping, main piping and sprinkler heads shall be removed as necessary to coordinate with all contractors and new installations. Contractor is required to
provide a complete and operable system for project areas indicated. Item M-2: Reuse of existing equipment where allowed is identified on drawings, i.e. fire alarm devices, panel board,
etc. Reuse of any other equipment such as lighting fixtures, receptacles, switches, etc. is not anticipated as types and colors may deviate. However, if opportunity arises to reuse such
devices they will be entertained on a case by case basis with appropriate credit proposed by contractor.
