Explanation of Claim to reimburse Sarah Lillard
On or about February 18' 2008, the City paid $149.00 to Fred Pryor Seminars for Sarah
to take a class on organizing and maintaining files and records. She eventually had to
cancel attendance at the class due to illness. Credit was given for a future class.
On or about May 28`", 2008, Sarah enrolled in a replacement class in records retention.
This class fee was $299.00. Since we had a $149.00 credit with the company, the amount
due was $150.00, which Sarah paid with her debit card.

billing period you can multiply the average daily balance times the number of days in the billing period times the daily periodic rate. The average daily balani
is computed by taking the sum of the principal credit advances owing on each day of the billing period (excluding FINANCE CHARGES and fees, if any) at
dividing by the number of days in the billing period. The FINANCE CHARGE continues to accrue on the unpaid principal balance after the statement has be(
printed and mailed to you.

If you think your statement is wrong, or if you need more information about a transaction on your statement, on a separate sheet, write us at Harris, Attn: Billir
Department, Servicing Dept. D, P.O. Box 5043, Rolling Meadows, IL 60008. We must hear from you no later than 60 days after we sent you the FIR:
statement on which the error or problem appeared. You can telephone us, but doing so will not preserve your rights.
In your letter, give us the following:
1. Your name and account number.
2. The dollar amount of the suspected error.
3. Describe the error and explain why you believe there is an error. If you need more information, describe the item you are unsure about.
You do not have to pay any amount in question while we are investigating, but you are still obligated to pay the amounts on your statement that are not
question. While we investigate your question, we cannot report you as delinquent or take any action to collect the amount you question.
Credit Information
We may report information about your account to credit bureaus. Late payments, missed payments, or other defaults on your account may be reflected in yo
credit report.
