The applicant seeks primary plat approval for 2 residential lots on 0.53 acres. Also, the
following waiver is requested from the Subdivision Control Ordinance:
Docket No. 11080015 SW SCO Chptr 6.05.01: lot width at road right of way
The site is located at 420 First Ave NW (on the Monon Greenway) and is zoned R-2/
Residence within the Character Subarea of the Old Town Overlay. Filed by Jeff King, owner.
Jeff King presented for the Petitioner. Property in question is located at 420 First Ave NW and is
currently .53 acres. We are proposing to plat the single lot into two lots which would make one lot .27
acres and the other .26 acres.

Concerns regarding access to both lots via public streets. There will be access on Fourth Ave
as well as access through the approved alley, which will provide wide enough room for the
fire apparatus.
Daren NTindham -DOCS Urban Forestry
-Met on side concerning issues and tree preservation, will be required to submit a landscape
plan.

9:10 a.m. Docket No. 11080016 ADLS Amend: Millennium Foods —Mayflower Park, Block 1, Lot
2. The applicant seeks approval for exterior changes to the building and site, to be used for
dry food processing warehousing. The site is located at 9760 Mayflower Park Dr. and is
zoned 1 -1 /Industrial. Filed by Charlie Frankenberger of Nelson Frankenberger, on behalf of
Millennium Foods, LLC.
Jon Dobosiewicz, John Raile, and Charlie Frankenberger presented for the petitioner. The site location
for the project is at 9760 Mayflower Park Dr. Currently there is an existing industrial building with four
overhead doors located on the east facade of the building. The proposal includes the addition of
overhead doors, an addition of two dry storage silos that would be positioned adjacent to the building, an
extension of the existing stormwater pipe located along Mayflower Park Dr., and a re- direction of the
sanitary line into the building. Currently the sanitary interface is located in the office area. That will be
moved to an area with floor drains.

-Have not made any comments at this time, still waiting to get an understanding of what the
byproduct is going to be in order to see if an interceptor /separator will be needed on site. The
fats, oil and grease personall have been in contact with the owner to work through this
situation of the project.

Concerns about access to the site and changes to the hydrant/sprinkler systems within the
building. Site access will remain the same and all fire related system connections will not be
affected.

-Met with John on site to review landscaping, sent review comments. Will need a list of what
will be added or replaced. Landscaping will have to be installed per plan.

requested adjacent to the parking lot, will receive an updated plan reflecting that.

9:20 a.m. Docket No. 11080018 DP /ADLS: The Bridges PUD HealthSouth Rehabilitation
Hospital. The applicant seeks site plan and design approval for a rehabilitation
facility /hospital on 6.5 acres. The site is located at approximately 11405 Springmill Rd. and
is zoned PUD /Planned Unit Development. Filed by Elizabeth Liles Mann of HealthSouth.
Jon Dobosiewicz and Charlie Frankenberger presented for the petitioner. HealthSouth is a rehabilitation
hospital that will be occupying approx. 6.3 acres. The location is along the west side of future Illinois
St., currently it is just south of 116' St. and future Illinois St. The proposal is a 60 bed hospital split into
two phases. Phase one consists of 40 beds and a potential phase two of 20 beds in a future addition. All
utilities on site will be part of the initial phase of development. The site will have two points of access
from the future Illinois St. as noted on the plans. The master developer has proposed an access and
construction of an east west connector driveway between Springmill Rd and the future Illinois St. The
Plan Commission will see this proposal on the Oct. 18' hearing.

Gas mains are located at Springmill and 111 St. for availability. Within the project we will
be dedicating a utility easement along the access road from Springmill Rd.

The entrance at 111 h St. is not part of this proposed project. Developer will have to provide
easements for service going in and coming out of the proposed site. A potential point of
access is Springmill Rd but it has not yet been finalized.

and or issues.

Utilities contacted Elizabeth Mann. The developer will receive a comment letter from Steve

Left a message for Andy Taylor. Will meet after TAC to address several questions regarding
operations of the facility.

Sent comment letter, waiting for response. Developer is aware of easement requirements,
either by sidewalks or walkways internally through the site. Understanding it is required
along the major thoroughfares and along access roads. Regarding the primary driveway east
west, developer has proposed a sidewalk on the south side of the driveway due to the
potential activity north of that location. They do not want to introduce the public to the truck
activity that is proposed on site adjacent to the driveway. The developer is aware that the
PUD grant will give them the ability to install one sidewalk on one side of the drive.
Developer provided an up to date PUD Pedestrian Plan that will continue to be updated as
new projects begin on this site.

department by Oct. 7 will be making any adjustments after that for the Plan Commission
packets.
9:30 am. Docket No. 11080017 TAC: The Bridges PUD Grading, Drainage Utilities Master
Plan. The applicant seeks approval of their mass grading, drainage, and utilities for 64 acres.
The site is located at 11405 Springmill Rd. and is zoned PUD /Planned Unit Development.
Filed by Charlie Frankenberger of Nelson Frankenberger for JEC Partnership LP.
The mass grading and drainage at this point will be filed with future plans and taken to TAC on the areas
north of the connector road. The mass grading and drainage for this site, at this time, include a small area
just north of the east west connector driveway South bound to 111' St. It will include the construction of
a mound along Springmill and a mound along l l I th St. This part of the project will also include the
drainage facilities appropriate to address HealthSound and the utilities extended under the connector
roads.

No comments at this time. Developer will be submitting and going through TAC and Plan
Commission with additional landscaping and development plans for that area. One of the
requirements for the project will include landscape along the berms of 111 St. and
Springmill Rd. at the time of construction of the first development, HealthSouth. Additional
plans will be filled around Oct. or Nov. for review.
