As we discussed, the City of Carmel has a need for additional professional services and pursuant to our current
contract with USI Consultants, Contract #070710.13 dated July 7, 2010, I have requested and received from you the
proposed scope of services and fee estimates for the following work task:
Auman and Newark Neighborhood improvements
Design; Pleases 2 and 3
See "Attachment A" Not to Exceed: $17,000.00
Per our contract procedural requirements, 1 hereby approve these Additional Professional Services estimates, and
request that the Board of Public Works ratify the above referenced Contract Amendment.
PLEASE NOTE
Please be advised that "Additional Services" performed on City contracts must be billed separately from the original
contract and other additional services. More importantly, the invoice must reference the appropriate Additional
Service and P.O. shown above. Invoices received without the Additional Service referenced will be returned

AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT "Agreement entered into by and
between the City of Carmel and Eagle Ridge Civil Engineering Services, LLC (the "Vendor), as City Contract
070710.13 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor
consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A The terms and conditions
of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force
and effect.

Dear David:

Attached is a draft scope of services and fee estimate for the additional services requested to date on the
Auman Newark Phase 2 Project. These changes have been noted in the monthly status reports and in
our correspondence and coordination meetings as we have progressed on the project. The three items
involved include the following work:
1) Additional Project Area:
The expansion of the project area to include Nappanee Drive from Pawnee Road to Oswego Road, and
also Oswego Road from Nappanee Road to Ute Drive. This 1,000' addition to the project was designed in
addition to the project limits described in the agreement once it was discovered that the pipes in
Rangleine Road were deep enough from storm sewers to extend into this area.
2) Stormwater Filtration /Quality Features Design and Permitting:
This task was specifically omitted from our original scope in order to reduce the fees and because the City
deemed it unlikely it would be needed. We were later directed to add this to the project.
3) Splitting Project into Two Phases:
After we had reached 95% completion on the project, the City requested the design be split into two
separate biddable projects due to budget limitations. This required revisions to the plan set as prepared,
and will require the bidding support activities be conducted two separate times, likely one year apart.
These will now be called Phase 2 and Phase 3.

GENERAL
ATTACHMENT A
SCOPE OF ADDITIONAL ENGINEERING SERVICES ENGINEERING FEES SCHEDULE
The City of Carmel has requested various changes to the scope of work in the project previously identified as
"Auman Newark Phase 2 This project includes drainage and sidewalk improvements along Nappanee Drive,
Oswego Road, Winona Drive, and includes a replacement of storm sewer along Range Line Road.
These additional services require the completion of new tasks and the extension of others in order to complete the
project as directed. The changes are described as follows:
1. ADDITIONAL PROJECT AREA
Design and plan development was extended to include the section of Nappanee Drive between Pawnee Road and
Oswego Drive, and also Oswego Drive between Nappanee Drive to Ute Drive. This 1,000' section along these two
roadways had been previously omitted because it is in lower ground and it was not thought it could be drained to
the areas already part of the project. After survey was completed and it was determined that this area can be drained
to the west in new storm sewers. Therefore, the area was added to the design and has included all typical design
tasks including plan development, storm sewer and sidewalk design, quantity estimating and utility coordination.
2. STORMWATER FILTRATION /QUALITY FEATURES
Designer was originally directed to not include services to design and prepare hydraulic computations for
stormwater quality filtration structures. These features were not believed to be needed. The City later changed this
guidance in response to comments from the Hamilton County Soil and Water Conservation District. Professional
completed the required hydraulic analyses, design, and related plan and specification development to incorporate
these features into the project.
3. SPLIT INTO TWO PROJECTS FOR BIDDING
Due to budgetary reasons, the City has directed that the project now be split into two separate projects or phases,
tentatively identified as Phase 2 and Phase 3. Phase 2 shall now be the portions of the project along Rangeline Road
and Nappanee Drive. Phase 3 shall be the portions of the work along Oswego Road and Winona Drive. For the
Professional, this requires a revision to several plan sheets, the cost estimate, and the preparation of two customized
sets of bidding documents. The bidding phase services, which have not yet been completed, must now be
performed twice, the second time likely to be in 2012. The Rule 5 permit must be sought twice and published twice,
once for each area.
The tasks and estimated fees for these additional services are described in greater detail in the attached fee
estimate spreadsheet.

COMPENSATION
These Additional Services are to be conducted on a Hourly basis with an agreed Maximum Cost of $17,000. The
overall contract maximum is thus increased to S53,000.
