We have reviewed the construction plans submitted to the Hamilton County Surveyor's Office
on September 16, 2011, for this project and have the following comments:

1. The proposedproject falls in the incorporated area and MS4 jurisdiction of the City of
Carmel.
2. The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone.
3. The proposed project falls in the Taylor Trace Regulated Drain Watershed.
4. The building expansion is part of the master drainage plans previously completed for
this site.
5. No permits will be needed from our office.

The following letterrepresents comments for this project specifically addressing the area of alternative
transportation. I. have reviewed your- response and the drawings submitted on October 21$` and the
comments from the Alternative Transportation review have been satisfactorily addressed.
We request that all responses to our comments be provided in writing. Failure to provide written
responses may result in delay of the review process.
It is critical that this office be made aware of all modification made on the plans being re- submitted,
particularly if any such changes are considered "new" or fall outside of our previous reviews. Please
provide revised plans indicating all revisions. Please notify us of any changes and specifically state any
changes, including changes resulting from Plan Commission, Special Studies or other committee
meetings.
The Department of Community Services reserves the right to provide additional comments based on
subsequent reviews.

This letter is in response to your comment email dated October 11, 2011 for the above referenced
project. We have prepared the following responses to the plan review comments listed below.
Review Comments
1. Comment: The Bicycle and Pedestrian Plan requires the construction of a 10'
asphalt path along Carey Rd. Please revise the plans to include and label
this path.
Please see revised plans showing 10' path.
Please confirm when the required multi -use path will be constructed.
The multi use path will be constructed with this phase of the project.
The City of Carmel Parking Ordinance requires that all new developments
include bicycle parking. This development will be required to have a
minimum offour (4) spaces. Please revise your plans to show the
required bicycle parking spaces on your site. Please see the City of
Carmel 's Parking Ordinance in order to determine appropriate locations,
specifications and construction details for the above.
Response: There is existing bicycle parking located near the canopy drive
through on 'the north east side of the existing building, constructed
with Phase 2.

Good Morning David,
I'm preparing a response letter to your comments. I have a question on item 1, regarding the 10' asphalt path. The path
was proposed and approved as a 6' multi -use path with the phase 2 plans, dated
March 30, 2010. Carmel hope has a Bond in place to construct this path as approved, 6'. Do these previously approved
plans supersede the 10' requirement you are referring too?
Attached is the approved site layout plan for Phase 2.

Engineering Review for Development Drainage. Designs
The stormwater review of the proposed construction plans and drainage calculations for the above
referenced project has been completed. The submittal is in need of additional information in order to
be in compliance with the City of Carmel Stormwater Technical Standard Manual: Please revise the
plans in accordance with the comments below.
1. Per Section 102.02:xi.m of the 'manual, please revise the plans to include a drainage summary, in
addition to the area,of'disturbance table that is included. The drainage summary should be
sufficient to summarize. the basic condition of the drainage design, and to verify conformance of
the proposed improvements with the requirements of the previously approved master plan.
Please include with your submittal, one. (1) copy of the comments indicating the action taken or a
written explanation for action not taken. Construction plans and drainage calculations are not to be re-
submitted without im plementing changes with respect to any and all review comments from the City of
Carmel and the Hamilton County Surveyor's Office. The comments and plans may be submitted to my
