According to the landscape plan submitted to the Carmel/Clay Plan Commission on 9/21/99,
the following omissions have occurred:
1. A landscaped parking lot island is missing at the rear of the building.
2. A total of ten (10) shade trees shown on the plan (in and around the rear parking lot) have
not been planted.
The parking lot island may be omitted, if the missing landscaping is planted somewhere else
on the property. The ten shad trees must be planted somewhere on the property, preferably
around the parking lot to provide shade for the asphalt.
3. A sidewalk running along the front of the property near the street has been omitted.
4. Exterior lighting exceeds maximum illumination level. ***A Notice of Violation
regarding this item was sent to the site on January 24, 2002. A copy of that notice is
attached to this Notice of Correction. Questions regarding this item should be addressed

RESPONSE LETTER
This letter is being wdtten in response to the comment items made during the
March 17, 1999, T.A.C. meeting in regards to the above site. The following
items correlate with your comment items during that meeting and have been
revised on the construction documents as follows:
Parking space size has been revised to reflect that all spaces on the
subject site are a minimum of 180 SF of dimensions required per the
Ordinance.
2. Exterior photometric contour readings and lighting fixtures have been
revised and added to the construction documents.
3. Sign elevation revisions for the wall sign have been revised on the
construction documents on Sheet LI.
Tree preservation plan has been included in Sheet L1 in regards to the
preservation of existing oak trees which will be protected throughout
construction.
Revisions have been made to the construction documents in regards
to the setback of the structure to provide for the required foundation
plans along the front elevation of the structure.

The colored renderings submitted to Dept. of Community Services to
date have no revisions in regards to the appearance of the structure
and/or signage on the structure as reflected by the preliminary
elevations and floor plans previously submitted to your office. The
front, side and rear elevations of this structure are all identical
therefore additional colored renderings are not necessary for this
project to be reviewed. The digitally enhanced photographs have
been modeled, engineered and designed to most effectively show and
represent the true appearance of the proposed structure on the
existing site as it will be designed and built.
If you have any questions or need additional information, please do not hesitate
to contact this office at any time.
We forward to the issuance of the Docket Number on this project as soon as
possible so that we may send out the required public notices to the adjoining
property owners and the newspapers.

Architectural Design, Landscaping, Lighting, and Signage applications for 12400 Old
Meridian, LI,C. The petitioner seeks approval to construct a 15,000 square foot office
and apartment building on 2.5 acres. The site is located one lot south of the southeast
corner of Carmel Drive and Old Meridian Street (Lot 2, Block 17, Carmel Science and
Technology Park.) The site is zoned M-3/Manufacturing. (Petitioner will also be
appearing before the Board of Zoning Appeals for consideration ora Developmental
Standards Variance.)

Adam DeHart, 486 Gradle Drive, Carmel appeared before the Commission representing
the applicant. Bob Lunsford with RDJ Custom Homes and JeffWorhel of Advantage
Medical were also in attendance.
Mr. DeHart stated that the petitioner had appeared before the Board of Zoning Appeals
yesterday evening, April 26t~, and received approval for a developmental s~andards
variance to decrease the required minimum side landscape yard from 10 feet to 0 feet.
The subject site is unusual in shape and encompasses 2.43 acres. The site contains a
storm sewer easement, (a large pipe running along Meridian Street) and empties adjacent
to a box culvert bridge beneath Old Meridian into an open ditch, and flows to the east.
The easement accounts for approximately 9/10 of an acre of the property. The size of the
structure has been reduced to approximately 14,200 square feet and has allowed for the
installation of a 24 foot drive (the length of the property) that wilt connect to future
development on Lot 3 for thru traffic from Carmel Drive to Old Meridian and still
provide access to parking in the rear.
The petitioner has met with the Old Meridian Task Force to review proposals for this area
for future requirements. The building will located at closely to the street as possible.
There are approximately 20 parking spaces that will be used by clients and customers of
the professional building. Advantage Medical will occupy one-third of the building; a
tenant lease area for another third of the building, and a real estate company will occupy
the final third of the building. The second floor will consist of executive suites with two,
some three bedrooms. The petitioner states that this facility complies with the Old
Meridian outlook to the future as far as providing professional office with medium to
high density housing. The development provides for individual covered carports for each
apartment unit. The carports will be open to the north and will be used for cars only-no
storage. The carports will match the fagade of the building in brick masonry. The
basement of the facility will be used for storage only and will not have any office space.

The development has been built up with berms along the north side of the existing ditch;
the site gradually slopes to the south and east. Sanitary sewer setwice has been extended
to the site as well as water lines.
Mr. DeHa.~ stated that with the exception ora final review completion letter fi.om the
Hamilton County Surveyors office, all items have been addressed fi.om the Engineer's
office and all other TAC members.
The landscape plan has been revised to provide for the new drive alignment, parking, and
a 5 foot landscape buffer along the north side of the structure; the parking is hidden at the
rear of the building. The petitioner is installing sidewalks along the street and interior
walkways.
Members of the public were invited to speak in favor or opposition to the proposed
development; none appeared and the public hearing was closed.
Mark Monroe confirmed that the petitioner was granted a Developmental Standards
Variance by the Board of Zoning Appeals.
Ron Houck expressed concern with the type of wall pack proposed and thought the
lighting was not consistent with the class of development being proposed.
Paul Spranger concurred regarding the lighting and encouraged the petitioner to meet
with the Department to change the design.
Mr. DeHart stated that his client was willing to eliminate the wall pack and install the
same type of incandescent, lantern type brass fixture used at the other entrance. Any
signage issues will be worked out with the Department.

Ron Houck moved for the approval of Docket No. 29-99 DP/ADLS, 12400 Old
Meridian, LLC, subject to the lighting issue being resolved in accordance with the Old
Meridian Task Force guidelines. APPROVED 12-0.
12h. Commission to consider Docket No. 30-99 DP/ADLS, Development Plan and
Architectural Design, Landscaping, Lighting, and Signage applications for the C & C
Realty Company. The petitioner seeks approval to construct a 62,500 square foot office
and warehouse complex on 5 acres. The site is located on the west side of Michigan
Road, one-eighth mile north of96~ Street. The site is zoned I-l/Industrial and is located
partially within the Michigan Road (U.S. 421) Overlay Zone. (Petitioner may also be
appearing before the Board of Zoning Appeals for consideration of Developmental
Standards Variances.)

DOCS Bufferyard guidelines suggest that a bufferyard containing a minimum of 3 shade trees,
5 omamental trees and 21 shrubs per 100 linear f~et be planted between a collector street and
a retail/multi-family building. This would apply to the ama along Old Meridian.

Dear Mr. DeHarl:
We have reviewed the plans submitted to this office on February 16, 1999. We offer the following
comments:
1. Streets - City of Carmel
2. Water- Indianapolis Water Comp#ay
3. Sanitary Sewer- Clay Regional Waste District
4. Storm Drainage - Hamilton County and City of Cannel
5. Plan Coimnissinn approval is required prior to Board of Public Works and Safety submissions.
6. The project will require commercial curb cut approval from the Board of Public Works and Safety (if
not obtained with Lot #1).
7. Provide a certified engineers cost estimate and Peffonnance Guarantees for sidewalks in the Old
Meridian Street right-of-way. Bonds must be posted prior to Board of Works approval of the plat. The
plat must be recorded in order to obtain a building permit. If Lots #1, #2 and 03 are platted together,
all sidewalk Performance Guarantees must be posted.
8. Indicate Old Meridian Street East right-of-way line and indicated oae-half right-of-way distance.
9. Please include an entrance plan. Indicate acel/deeel lane dimensions, pavement cross sections, saw
cutting details, etc.
10. Is the 8-inch water main in Ihe right-of-way existing or proposed? We assmne it is proposed.
11. We require a Street Cut/Right-of-Way Permit and Street Cut Bond for-
