Re: Geotechnical Engineering Investigation Proposed Three Sided Culvert Replacement River Road Carmel, Indiana ATC Proposal Number PE -11 -0793 Gentlemen: We are pleased to submit this proposal for performing a geotechnical engineering investigation for the referenced project. It is our understanding that the project will consist of the construction of a new three -sided to replace an existing culvert structure beneath River Road approximately 400 ft north of Tall Timber Run in Carmel, Indiana. The objectives of this investigation are to determine the existing subsurface conditions at the site and to develop recommendations necessary for the design and construction of the soil supported elements of the proposed project. The proposed subsurface exploration consists of drilling two soil test borings (one test boring in the southbound lane of River Road immediately south of the existing culvert and one test boring east of River Road immediately north of the creek). The borings will be drilled to a depth of 30 ft. Split -spoon samples (ASTM D -1586) will be obtained at 2.5 ft intervals to a depth of 20 ft below the existing ground surface and at 5 ft intervals elsewhere. Ground water level observations will be made during drilling operations and after withdrawal of the augers from the borings. In accordance with Carmel Aquifer Protection Guidelines, we will backfill the boreholes with bentonite chips and place a concrete plug at the surface to prevent subsidence. A reconnaissance of the project site will be made by a geotechnical engineer from our staff and the boring locations will be established in the field at that time. The cost estimate does not include establishing ground surface elevations at boring locations. ATC will contact Indiana Underground Plant Protection Service to locate underground utilities that are owned by the member utility companies; however, it is assumed that the owner's representative will accurately mark the locations of all underground utilities that are part of the owner's physical plant (i.e, all underground utilities that are not part of the IUPPS locate service members). This proposal does not include fees for a private underground utility locate service or the repair of damaged underground utilities. Laboratory tests will be performed as necessary to establish the significant characteristics and parameters of the foundation soils. The laboratory testing program tentatively includes moisture content tests, Atterberg limits tests and unconfined compressive strength tests on samples selected by the geotechnical engineer; however, the actual laboratory testing program will depend upon the specific subsurface conditions encountered in the test borings and the soil samples obtained. After completion of the field investigation and laboratory tests, an engineering report will be prepared containing recommendations to guide design and construction of the foundations and wing walls for the proposed three -sided culvert. The report will include a recommendation regarding "Site Class" based on Sections 1613.5.2 and 1613.5.5 of the 2006 International Building Code. We propose to perform the scope of work described herein for a lump sum of $4,000.00 unless unexpected subsurface conditions are encountered or the project characteristics are changed significantly. It has been assumed that the test borings are readily accessible to an ATV mounted drill rig. Any additional work requested and approved will be charged in accordance with the unit rates for the applicable items as shown on the attached fee schedule. If any changes in the program are indicated by the initial findings, we will consult with you and, with your approval, make such changes as are considered necessary. In any case, the cost estimate will not be exceeded without additional approval from you and changes to the scope of work will require a written change order. Along with the geotechnical investigation, ATC will retain the services of Blood Hound, Inc. to "pothole" the existing utilities near the culvert location. Currently, eleven (11) locations have been indicated by the Client's representative, which include seven (7) locations within the existing stream and four (4) locations on the bank of the stream. A PVC pipe will be place within the pothole excavation to aid in determine the top of utility depth. We propose to perform this scope of work based on an hourly rate of $275.00/hr. It has been estimated by Blood Hound, Inc. that this scope of work will take no more than 24 hours to complete. A total cost of $6,600.00 will not be exceeded without your approval. This proposal is based on the following assumptions: The test borings are readily accessible to an all- terrain- vehicle (ATV) drill rig. No special safety orientation or training will be required in order to be present on site or to perform work on site. All soil cuttings that cannot be placed to baclfill the boreholes and potholing can be distributed on -site. The work can be performed without interference from or an agreement with a labor union. The test borings will not be required to penetrate concrete. The scope of this study is based on the assumption that the proposed facility can be supported on shallow foundations. If the results of the field investigation indicate the need for deep foundations, it may be necessary to extend borings deeper in order to develop recommendations for deep foundations. The subsurface investigation outlined in this proposal assumes that there are no hazardous materials in the soil or in the ground water underlying the site and this study is not designed to detect or identify such materials. A If it becomes apparent during the field investigation that hazardous materials may be present, field operations will be terminated. The investigation would be resumed only after renegotiation of the scope of services and fees to cover appropriate health and safety precautions and proper consideration of the new information. ATC can provide complete services relative to the assessment of contaminated sites. Our estimate covers the work needed to present our findings and recommendations in a report form. Not included are fees for the preparation or review of construction documents (e.g., plans and specifications), special conferences and any other work requested after submittal of our report. This proposal does not include costs for restitution for crop or landscape damage and ATC will not be responsible for damage to crops and landscape. In order to authorize this work, please sign and return one copy of the following Proposal Acceptance form and provide the necessary instructions regarding billing and other matters as appropriate. This proposal, including the terms and conditions, constitutes the contract and is subject to credit approval by ATC Associates, Inc., Corporate Credit Department. We can normally begin the field investigation within about three weeks after authorization, if the weather and site conditions permit.
