Angie we are revising the plats that you previously saw for this submittal and will re- submit to you very soon.
The previous plat will be split into 2 separate plats.
The first plat will have Blocks G, H R.
The second plat will have Blocks A E.
We are going to add a 3 plat that will be just Block V.
All blocks are in Section 6001.
We previously submitted paperwork and a check in amount of $1,510.00 o ou for the 5 blocks, per the submittal
attached to this email. You said the fee would be $302.00 block, thu 1,510.00 Q
Now we want to add the one (1) block V to this submittal. I assume we will need to revise the application to reflect
that. Would we need to get you another check for this block V addition in amount of $302.00

6. Staff cannot present a Secondary Plat to the Director for signature until Ms. Terry Krueskamp
(tkrueskamp @carmelan.gov) in Information Systems receives an e -mail with an electronic file of'the plat.. attached
(MicroStatiOn .dgn is. preferred, or AutoCad..dwg), and it (the e- mail) has been copied to myself and Alexia Donahue
Wold (awold @carmel.in.gov).
7. Please submit a revised Secondary Plat after making above mentioned changes. Once these changes have been
made and .continents addressed: please produce, a Mylar copy (with the surveyor's and land owner's signatures) for
signature by this Department. Two paper copies of the plat must be filed with the Mylar prior to recording, and two.

paper copies and a Mylar of the recorded plat must be submitted following recording of the document. Our approval
of the secondary. plat (signature) will be forthcoming upon revision of the plans according to the items listed above.
8. Lastly, please submit a .pdf or .jpg file of the scanned,.recorded plat.
