Item No. 4 Temporary In-Stream Silt Basin The silt basin was not constructed as
shown on the plans. (Underrun)
Item No. 6 Excavation, Waterway No waterway excavation was done because
existing riprap remained in place. (Underrun)
Item No. 8 Temporary Silt Fence Temporary silt fence was not used. (Underrun)
Item No. 9 Aggregate for End Bent Backfill Additional aggregate was used due to
over excavation for piles at the end bents. (Overrun)
Item No. 10 Compacted Aggregate, 53, Base The thickness of the #53 base was
greater than was shown on the plans. (Overrun)
Item No. 11 HMA Surface, Type A The thickness of the HMA surface was less than
was shown on the plans. (Underrun)
Item No. 12 HMA Binder, Type A The thickness of the HMA binder was greater than
was shown on the plans. (Overrun)
Item No. 13 HMA Base, Type A The thickness of the HMA base was less than was
shown on the plans. (Underrun)
Item No. 18 Geotextile Less geotextile was used because no geotextile was placed
under the existing riprap at the end bents. (Underrun)
Item No. 19 Riprap, Class 1, 24 Inch Depth Less riprap was required because the
existing riprap was used. (Underrun)
Item No. 20 Seed Mixture, R More seed was used because a greater area was
disturbed in the wooded area at the southeast corner of the bridge. More of the erosion
control blanket was used to provide erosion control protection. (Overrun)
Item No. 21 Fertilizer Less fertilizer was used than was shown on the plans. More
erosion control blanket was used to provide erosion control protection, (Underrun)
Item No. 22 Mulched Seeding, Type T, Undistributed No Mulched seeding was used.
More erosion control blanket was used to provide erosion control protection, (Underrun)
Item No. 23 Temporary Erosion Control Blanket More seed was used because a
greater area was disturbed in the wooded area at the southeast corner of the bridge. More
of the erosion control blanket was used to provide erosion control protection. (Overrun)
Item No. 24 Mobilization Demobilization for Seeding All seeding operations were
performed at one time. An additional mobilization was not required. (Underrun)
Item No. 25 End Bent Drain Pipe, 6 Inch Diameter Less pipe was required that was
shown on the plans because of the changed slope of the spill cones at the end bents.

Item No. 26 Less saw cutting was required that was shown on the plans. (Underrun)
Item No. 27 Piling, Steel "H" Piles, HP 12x53 Less piling was required because the
end bents were redesigned to eliminate a conflict with a 30" water line. One pile was
eliminated at each end bent. (Underrun)
Item No. 28 Epoxy Coated Reinforcing Steel More epoxy steel was required because
the end bents were redesigned to eliminate a conflict with a 30" water line. (Overrun)
Item No. 33 Field Tile, Undistributed No field tile was encountered during
construction. Underrun)
EWA 1 Was for the use of a trench box during pile driving operations at the end bents.
The box was never used. (EWA 1 Void)
EWA 2 GPR Utility Locating (Water Main) Underground radar was used to locate
the existing 30 inch water main as directed by the City. (Overrun)
EWA 3 Existing End Bent Riprap The existing riprap was reshaped around the new
end bents and additional riprap was used to fill in low areas in the slopes. (Overrun)
EWA 4 Anchor Bolts Anchor bolts were not provided with the beams. These anchor
bolts were to be provide to HIS by the City. (Overrun)
EWA 5 Cut and Treat Timber Rail Cutting and treating the timber paving strip on
the bridge deck was required to allow the placement of the HMA surface. (Overrun)
