Dear Mr. Fleming:
The City of Carmel has received your comment response letter and revised drawings dated June
7, 2004 for the above.referenced project. Thank you for addressing the City's comments. Based
on a review of your comments and the submitted draxvings, the issues from thc City's second
review on June 3, 2004 were satisfactorily addressed. This Department is prepared to approve the
Drawings.
This office requires a minimum of four sets of certified drawings for approval. The drawings will
be stamped approved and will be sigmed by the City Engineer and the Utility Department. The
Developer/Owner will receive one-set which is to be maintained on the construction site at all
times. The City's Construction Manager will receive one set, the Department of Engineering will
maintain one set and the Utility Department will receive one set. If more than the minimum of
four sets is requested for approval, the additional sets (a maximum total of two additional sets)
must be submitted with the four required sets.
The following outlines the necessary items to be completed prior to.starting construction. These
items were outlined in previous review comment letters.
1. Obtain Board of Public Works and Safety approval for the following:
· Water Availability for the Development. If an irrigation system is planned for this
development, additional Board of Public Works and Safety Water Availability
approval will be required. This also applies to planned swimming pools and/or
clubhouse.
· Open cuts of 141st Street for the storm sewer and sanitary sewer crossings.
· Consent to Encroach Agreement for any permanent improvement (including
irrigation systems) to be installed within dedicated right of way or dedicated
easements.
· Secondary Plat approval.
2. Provide individual Performance Guarantees for the following subdivision and right-of-
way improvements:
· Streets
· Entrance improvements, auxiliary lanes, stone shoulders, etc.
· Curbs

· Water Mains
· Street Signs
· Sidewalks and/or Asphalt Paths (builders lot sidewalks, right of way sidewalks
adjacent to common areas and perimeter right of way sidewalks/paths may be bonded
separately). Bonding is not required for asphalt paths and/or sidewalks located
within common areas.
· The Hamilton County Surveyor will require bonding for Storm Sewers and
Monuments & Markers. The City will not require duplicate bonding for these items.
Sanitary Sewers are under the jurisdiction of Clay Township Regional Waste District.
The amount of the Performance Guarantee is based upon a certified Engineer's Estimate
for 100% of the cost of labor and materials to construct the individual improvements, to
be provided by the design engineer. Please provide detailed Engineers Estimates for each
improvement including unit costs, quantities, materials and types of materials, etc. Upon
completion and release of individual Performance Guarantees, a three-year Maintenance
Guarantee will be required. The Maintenance Guarantee amount is based upon 15% of
the Performance amount for Streets and Curbs and 10% of the Performance amount for
all other improvements. Performance Guarantees may be Performance or Subdivision
Bonds, Irrevocable Letters of Credit or Cashier's/Certified/Official Checks. Please
reference the enclosures for more detailed explanation o four procedures.
Pay Availability Fees and Connection Fees. These fees must be paid after all other
Engineering Department requirements have been satisfied and prior to start of
construction activities (the exception is that you are allowed to move dirt on site).
· Availability Fees are based upon total platted acreage for the development at the
rate of $1,010.00 per acre for Water Availability. B'ased upon the primary plat
indicated acreage, the following Availability Fees would apply:
Water- 22.089 Acres (~ $1,010.00/Acre = $ 22,310.00
· Connection Fees are $1,310.00 for water per single-family residence.
Connection Fees are paid when the infrastructure has been completed,
satisfactory test results obtained and the subdivision has been released for
permits. Connection Fees are paid on a per-lot basis. If an irrigation system ia'
planned is planned for this, or any future section of the develot~ment, additional
Water Connection Fees will be assessed based upon the recommendation of the
Director of Carmel Utilities.
The work in the right of way of 141~t Street will require a Right of Way Permit and a
License and Permit Bond. The bond amount will be determined by the number of
instances of work in the right of way ~ $2,000.00 per instance. Any construction
covered by Performance Guarantees would not be included in the Right of Way Permit
Bond requirements. Please contact our Right of Way Manager, Fred Glaser, to arrange
permitting and bonding.

Dear Mr. Fleming:
The City of Carmel has received your comment response letter and revised dra~vings
dated April 8, 2004 for the above referenced project. Thank you for addressing the City's
comments. Based on a review of your comments and the submitted drawings, the issues
from the City's first review on March 15, 2004 ~vere satisfactorily addressed. Based on
the City's second review, the City provides the following comments:
1. General.
a. Add note to all Holey Moley details to contact Carmel Utilities at 571-
2648 for locations of all water mains.
b. Will all flow in the existing legal drain be captured and conveyed as a part
of the Hayden Run development?
c. What is the status of obtaining additional right-of-way required for the
roadway improvements and off-site easements for the sanitary sewer
connection? Are we correct to assume that the off-site storm sewer
installation will be installed within the existing legal drain easement with
permission from the Surveyor's Office?
2. Secondary Plat.
a. There are a number of typos or text over posting of the bearings and
distances on the street centerlines.
3. Sheet C200 and C201.
a. Is future sanitary service anticipated along Street C to the west and Street
D to the east?
b. General Note la. The City suggests that "suitable fill" be defined by some
type of material specification if not defined in Project Specifications.
c. General Note 5. Add "C" as follows: "Contact Carmel Utilities at 571-
2648 for locations of all water mains."

d. Ensure the median on Street D is allowed to continue at full width into
Section 2. There is an apparent taper at the western limit of Section 1
Street D.
e. Please label the slope of all proposed swales. Swales shall be 1%
minimum slope.
f. Revise note that reads "All pads should be tested...' to read "All pads
shall be tested..."
g. Please indicate edge striping on the auxiliary lanes.
Sheet C300.
a. Please indicate the location of the concrete swale noted on this sheet. How
much water is being discharged to this swale? Is this the intended pond
overflow into the existing legal drain? If so, how will this water flow when
the legal drain is relocated?
b. Fuel storage and filling area. Please ensure that the project site is not in a
wellhead protection area. Please ensure that all applicable state laws are
followed and that all permits that would normally be required for such an
application are obtained and kept on-site.
Sheet C302.
a. Will filling in the existing pond as a part of Section 2 impair that pond and
drainage system?
Sheet C403.
a. Revise note which reads "A sawcut is needed to remove the edge line
along the north side of 141st Street..." to read "Sawcut the existing
pavement where new pavement is to be placed adjacent to the existing
pavement on the north and south side of 141 st Street and to remove the
edge line along the north side of 141st Street...".
b. Please indicate edge striping on the auxiliary lanes.
c. Edit pavement section on Road Cross-Section to match pavement section
on Sheet C802. The paving section on Sheet C802 is required to be edited
per this letter.
d. Edit the note which reads "141st Street to be resurfaced across the entire
frontage" to read "The entire roadway of 141st Street shall be resurfaced
across the frontage of the proposed acceleration and deceleration lanes as
a part of this contract."
e. Are the power poles noted for relocation to be relocated by the utility
company?

f. What is the intent of Note 3 in the Typical Handi-capped Ramp at Comer
detail? Please ensure that all sidewalk ramps are to be constructed by the
Site Contractor and not the individual home owners.
7. Sheet C404.
a. Ensure the median on Street D is allowed to continue at full width into
Section 2. There is an apparent taper at the western limit of Section 1
Street D.
8. Sheet C406.
a. The City suggests the deletion of the "Typical Applications..." detail if it
is not intended to close any of the traffic lanes. Ifa lane is being closed,
please provide a specific plan. Lane closures require Board of Public
Works and Safety approval.
b. Please add sign references (XW20-1, e.g.) to the Traffic Maintenance
Plan.
c. Some of the notes reference Hazel Dell Parkway and 146th Street. Please
revise or delete.
9. Sheet C407.
a. The City recommends the deletion of details that do not apply.
10. Sheet C600.
a. It is apparent that the minimum cover of 12" from the top of the pipe to
the bottom of the roadway aggregate section is not maintained for
Structures 131 and 130.
11. Sheet C603.
a. Docs the existing pond design account for the proposed stom~ water
discharge?
12. Sheet C801.
a. Please add a bedding detail for the storm pipe.
13. Sheet C802.
a. Edit Pavement Compositions Detail to be the following section: 7"
compacted #53 stone, 5" H.A.C. Base #5D, 1" H.A.C. Surface #11
b. Are the three different curb sections noted delineated on the Site
Development Plan?
c. Add a street cut patch detail (attached).
14. Crossroads drainage review was completed on May 16, 2004 and was submitted
to your office on May 24, 2004. Please provide documentation that their issues
and those of the other governing agencies have been satistied.

Dear Mr. Fleming:
We have reviewed the construction plans submitted to the Hamilton County Surveyor's
Office on May 19, 2004 for this project and have the following comments:
Per conversation with Doug Elmore with Platintun Properties, it was agreed a
better .solution to connecting the existing Stultz and Almond regulated drain
would be to extend a 12 RCP south fi:om 125A and set a 2' x 2' concrete box at
the interception point. This will avoid tearing up the common area in the future to
crash the tile and will not require installation of a riser pipe at thc transition point
fi:om old to new. Depth and cover appear to work and this should be a better
option to explore.
Please refer to detail D-7 as shown in the print, h clearly states the "Emergency
Spillway at 100 Year' and this office requires storm water from a 100+ year
storm event to utilize the emergency overflow and spillway. While I do not have
detailed topo information east of this site, it is apparent the routing will be across
the field and under 141~t Street in the existing culvert. Please try to lower the
spillway elevation from 913.8 to 912.8 but maintain a positive routing to get the
water way. Also, correct the elevations on the overflow cross-section for pond 1.
3. Please show the pipe from stmeture 142 to pond 2 being installed with this
section, since the pond in being constructed now.
4. Please submit a copy of the final plat to this office prior to final approval by the
City of Carmel so we can verify the regulated drainage easements.
Once thcse above issues are addressed and the City of Carmel is acceptable with
the plans, please submit 7 set for approval to Cannel Engineering and then our
office. Once that is complete, a pre-construction meeting will be held and the on-
site work can begin.

Dear Mr. Fleming:
We have reviewed the construction plans submitted to the Hamilton County Surveyor's
Office on April 21, 2004 for this project and have the following comments:
Please submit the following procedural items to this office, petition for regulated
subdivision, outlet permit, engineer's estimate, and all associated bonds for 100%
of construction cost (for the subdivision). The application is available on our
website at http://www.eo.hamilton.in.us, go to Departments, Surveyor, and
Drainage Permits.
The invert of the existing Stultz and Almond Regulated Drain on the west
property line will be to be field verify for positive outfail before the plans will be
approved for consLruction. Please provide this in the construction plans, along
with the exact location of the tile that was field verified.
Please specify Neenah R-3501-N on all curb inlet structures with flow from both
dlreetions or Neenah R-3501- TR or TL for directional flows. The intent of this
comment is to eliminate the dilemma on which way to face the vanes in the
casting when the structure is in the sag of the roadway. I do see places where
directional (TR or TL) casting will be needed, like the double inlet structures
and 127, 128, 140, & 141, plus maybe others. Please specify for each inlet
which casting will allow the water to enter the inlet unobstructed.
Was the location of the existing tile that is to tie into structure 109 field located?
To verify this will work, please provide the field location and inverts in the
construction plans.
5. A mad cut permit f~om HCHD for the open cut of Towne Road will be needed
and a copy provided to us before that mad crossing can begin.
The emergency overflow and emergency flood routing for both lakes must be set
at the 100 year flood elevation, as shown of the lake cross-section details. The
flood routing must be a positive fall outlet for a distance that can carry the water
away from the development.
7. Please correct the I00 foot bust in the design for the top of bank and spillway
elevations.
8. The emergency overflow and flood muting for lake #2 will not function properly
at the 100 year flood elevation as required.
The landscape plan has been revised and does seem to be improved, but in places
trees still are show very near the pipe. Also, along the west property line, trees
are shown over the existing Stultz and Almond regulated drain. This will still be
a regulated drain until further development happens in the area. No landscape
planting will be permitted or allowed within the proposed regulated drainage
easements per Indiana Code 36-9-27-33(d).
10. Please be aware that the Hamilton County Soil Survey shows a large portion
of the Westside of section 2 as containing Patton Soil. This may cause
problems with the future roadway nnd building pads without addressing the
soil type,
11. Please note that further comments may be necessary at a later date.

collected by the proposed collection system that is the subject of the application, plans, and

We have reviewed the construction plans submitted to the Hamilton County Surveyor's Office
on March 8, 2004 for this project and have the following comments:
Please submit the following procedural items to this office, petition for
Reconstruction and Relocation of the regulated drain (52.0 form for public hearing),
petition for regulated subdivision, outlet permit, ~_gineer's estimate, and all
associated bonds for 100% of construction cost. Please separate the estimate and
bonds for the subdivision and the reconstruction. The applications are available on
our website at http://www.co.hamilton.in.us, go to Departments, Surveyor, and
Drainage Permits.
The reconstruction of the Stultz & Almond Regulated Drain will require a public
hearing by the Hamilton County Drainage Board because the drain crosses several
individually owner parcels.
3. Structure 116 need to be changed to a manhole and the invert lowered to decrease
the slope in pipe run 116-117 and bring the velocity down from 9.50 ft/s.
4. The following curb inlets will need to be manhole, instead of boxes, due to the
depth of the inverts: 120, 121,127, 128, 138, and 139.
5. The invert of the existing Stultz and Almond Regulated Drain on the west property
line will be to be field verify for positive outfall before the plans will be approved
for construction.
The easement from lots 76-79 does not allow for 15' per half from the center line of
the pipe as required by I,C. 36-9-27-33. Please reloeatad the pipe to center of
proposed easement or increase the easement to 40',
7. Please make the sub-surface lateral on lots 14, 29, 74, and 76 more visible.
The building pads on lots 2 & 3 encroach into the regulated drainage easement.
Please make sure the builder has sufficient room for the house as it will NOT be
allowed to encroach into the regulated drainage easement.
9. The easement on lots 10-13 will need to be increase to a 30' Regulated Drainage
Easement.
10. Please specify Neenah R-3501-N on all curb inlet structures with flow from both
directions or Neenah R-3501- TR or TL for directional flows,

Please add HCSO Standard Details 0-5, D-21, M-I, and M-3 to the construction
prints, Please make sure the manhole structures are sized correctly in accordance
with our standard details.
12. Please remove the reference of Towne Road near structure 103 in the plan view on
sheet C603. I believe this to be the access road to the electric substation.
13. No trash guard is needed on structure 100 as labeled on sheet C603.
14. Please add a note that the existing Stultz & Almond tile must be crushed or
abandoned per the approved method by the City of Carmel Engineer.
15. Please add a box note with the following language to the grading plan and the utility
plan: "All existing off-site drain tiles that are encountered shall be tied into the
proposed storm sewer system with a positive outflow."
16.
Please move structure 109 to the west approximately 30 feet and show the existing
portion of the regulated drain connecting to this manhole. This connection must be
made to allow the portion of the drain from the Stamper's west property line to the
structure to continue to fimction. Also lower this structure to match existing grade
and do not place a riser ring on this structure (to allow it to be raised to match future
grade in the area). Change the manhole lid to an R-4342 casting.
17. Please remove the reference in the language that refers to Trail of Hayden Run on
sheet 602 & 603, This drain is being reconstructed/abandoned with this project.

Please show full depth granular backfill on the profile view for the 3 private
driveways not shown and Towne Road. A road cut permit from HCHD for the open
cut of Towne Road.
19.
Please lower TC of 105 to match existing grade and do not place a riser ting on this
structure. Also, show a temporary swale from Towne Road west to this structure
and place an R-4342 casting to drain the area around Towne Road.
20. Please add a manhole structure in the low spot on the eastside of Towne Road with
an R-4342 casting for drainage of the area.

The drainage report shows the 100 year maximum flood stage for lake #1 at 914.788
while the construction plans say it is 912.87. The ICPR report also says the 10 year
max flood stage is 913.23 which do not match the construction plan elevation of
911.23. The drainage calculations do not seem to prove the 15" RCP dram
reeonstmotion is large enough to handle the flow volmnes needed. The Harnilton
County Ordinance 4-26-99-C requires the site to detain and release through the
outlet up to the 24 hour 100 year storm event. The emergency overflow is only for
the 100+ storm event.
22.
The emergency overflow and emergency flood routing for both lakes must be cross
section and profiled to an adequate positive outflow point. This also will be
required as part of the as-built phase.
23. The emergency overflow and flood routing for lake #2 will not function properly at
the 100 year flood elevation as required.
24. The drainage calculation represent that the 100 year discharge rate is 3.92 cfs and
the l0 year is 3.93 cfs. This would lead me to believe the pipe is being used as the
outlet restriction and is undersized for the watershed area it is serving.
25. To best address the drainage calculation concerns, maybe a meeting with Chad
Miller or the hydrology department would be the most efficient method to resolve
the concerns.
26. The landscape plan has a ton of trees show within the regulated drainage easement
and over the regulated storm pipes. No landscape planting will be permitted or
allowed within the proposed regulated drainage easements per Indiana Code 36-9-
27-33(d).
27. Please note that further comments may be necessary at a later date.

BONDING REQUIREMENTS
9. Individual Performance Guarantees will bc required for the following subdivision improvements:
Streets
· Curbs
· Water Mains
· Street Signs
· Sidewalks and/or Asphalt Paths (builders lot sidewalks, right of way sidewalks adjacent to common
areas and perimeter right of way sidewalks/paths may be bonded separately). Asphalt paths and/or
sidewalks located within common areas are not required to be bonded.
· Thc Hamilton County Surveyor will require bonding for Storm Sewers and Monuments & Markers. We
will not require duplicate bonding for these items. Sanitary Sewers.are under the jurisdiction of Clay
Township Regional Waste District.
The amount of the Performance Guarantee is bascd upon a certified Engineer's Estimate for 100% of the cost
of labor and materials to construct the individual improvements, to be provided by the design engineer.
Please provide detailed Engineers Estimates for each improvcmem including unit costs, quantities, mater/als
and types of materials, etc. Upon completion and release of individual Performance Guarantees, a three-year
Maintenance Guarantee will be rcquired. Thc Maintenance Guarantee amount is based upon 15% of the
l:'erformance amount for Streets and Curbs and 10% of thc Performance amount for all other improvements.
Performance Guarantees may be Performance or Subdivision Bonds, Irrevocable Letters of Credit or
Cashier's/Certified/Official Checks. Please reference thc enclosures for more detailed explanation of our
procedures.
AVAILABII.ITY AND CONNECTION FEES
