I am sending this update so you are aware of Carmel's progress on Illinois Street in the area
of your proposed U.S. 31 medical office project. In meeting recently with the Illinois Street
project engineer, American Consulting Inc., the City has been able to finalize the alignment
of Illinois Street between Carmel Drive and the
While I am confident in the location of the finalized stares, please realize this alignment
must still be field verified and refined per a detailed site survey, which is ongoing and
should be completed in the next couple of weeks. Carmel has requested that American
Consulting work as quickly as Possible to complete the survey work in order to keep the
project moving forward.
In the meantime, we are providing you with the most current aerial photo showing the
aligranent. Carmel will continue to provide you with updated drawings as we refine our
design of the corridor from updated field information.

hearing for the above-ret'erenced petition be continued from the August 17, 2004 meeting ofthe
Carmel Plan Commission to their September 21, 2004 meeting. Clarian representatives are still
having discussions regarding the proposed Cancer Center addition with Clarian'$ Board members
and desire to conclude these discussions prior to any public hearing.
We understand that no further actions are required to continue the public hearing
on the above-referenced petition. Consequently, representatives of Clarian Health Partners, Inc.
will not be present at the August 17, 2004 meeting of the Plan Commission.

Dear Ms. Poczekay:
The City has received.your comment letter dated May 26, 2004 and revised drawings received in
this office on June 3, 2004. Thank you for addressing the City's comments. Based on a review of
your comments and the submitted drawings, the issues from the City's first review on May 18,
2004 were satisfactorily addressed. The City is prepared to approve the drawings.
This office requires a minimum of three drawing sets to stamp for approval. The drawings will
be stamped "approved" and will be signed by the City Engineer. The distribution of the approved
drawings is as follows:
Developer/Owner. Maintain approved set on the construction site at all times.
· City's Construction Manager
· Department of Engineering
If more than the minimum of three-sets is submitted for approval, the additional sets (maximum
total of two additional) must be submitted with the three required sets.
Work which would require furore approvals include:
· Consent to Encroach Agreements. Future installation of signs, walls, irrigation systems,
or other permanent improvements within dedicated right of way and/or dedicated
easements wfll require a Consent to Encroach Agreement between the Owner/Developer
and the City of Carmel. This Agreement requires Board of Public Works and Safety
approval.
· Street Cut/Right of Way Permit and Bonding Requirements. At this time, it is expected
that the entrances along the proposed Illinois Street will be constructed as a part of that
project and that no work is to be completed in the 116th street fight-of-way. Any work in
the dedicated fight of way of 116th Street or Illinois Street (including curb cuts, open
pavement cuts, etc.) will require a Right of Way Permit and a License and Permit Bond.
The bond amount will be determined by the number of instances of work in the right of
way (commercial curb cuts, storm sewer work, etc.) ~ $2,000.00 per instance. Please
contact Fred Glaser, Department of Engineering Right of Way Inspector/Manager for
details regarding this issue. Open pavement cuts and new commercial curb cuts in
dedicated streets require approval by the Board of Public Works and Safety.

Sheet C3.1 Overall Grading Plan. Please provide calculations which
demonstrate that the increase in impervious area'from the proposed building
and parking expansion was accounted for in the previously approved
drainage and detention system, including pond sizing and piping
downstream of the improvement area.
Please see enclosed drainage report.

Sheet C3.2 Grading/Drainage Plan. Please indicate the piping that is to be
installed as a part of this approval. Please indicate previously approved
storm piping which is to be modified as a part of this approval.
The project limits have been added to the plans. Structures over #100
are new to this project. Structures under #100 have been approved with
the previous ,submittal. Those structures under #100 with an "M" have
been modified slightly.

Sheet C3.3 Grading/Drainage Plan. Based on the design inverts, it is
apparent that the detention area to the west of the parking expansion is to
serve as a overflow pond which will fill and drain based on the water
surface elevation of the larger pond to the west. Is this correct? Please
provide drainage calculations of this entire system if this is the case.

GENERAL INFORMATION

The project site is located within current City of Carmel Corporate Limits. The following
jurisdictions will apply.

3. Board of Public Works and Safety Approval Requirements:
· Commercial Curb Cut. When appropriate, Board approval for the curb cuts should be
requested. Please provide 81/2 x 11 exhibits of the curb cuts and include all dimensional
data including throat width, overall width at the street, radii, etc. Show all existing and
opposing curb cuts and the alignment bet~veen existing and proposed.
· Temporary Construction Entrance. Based on a conversation with the Project Engineer on
May 18, 2004, the existing construction entrance off of 116th Street at the proposed
intersection of the furore Illinois Street will be used. Approval of furore entrances needed
off of 116th Street or the proposed Illinois Street may be included with curb cut requests.
· Consent to Encroach Agreements for any permanent improvement to be installed within
dedicated fight of way and/or dedicated easements.
· Dedication of fight of way if applicable.
Any submission to the Board requires prior final approval by the Carmel Clay Plan
Commission attd/or the Board of Zoning Appeals and completion of review by the
Technical Advisory Committee. All written requests to be place on the Board's agenda
must include the appropriate Docket Numbers attd the date (or dates) of approval by the
Plan Commission and/or BZA.
4. Water and Sanitary Sewer Availability (acreage) and Connection Fees

5. TAC Revie~v/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in
writing and be accompanied by a drawing reflecting requested revisions. Final drawings will
not be approved for construction until all issues have been resolved. The design engineer
must certify all drawings submitted for final approval. This office will require a minimum of
three-sets of approved drawings after all issues have been resolved and the construction
drawings have been approved. The drawings will be stamped approved and signed by the
City Engineer. The Developer/Owner will receive one-set which is to be maintained on the
construction site at all times. Our Construction Manager will receive one-set and one-set will
be maintained in the Department of Engineering. If more than the minimum three-sets are
requested to be approved, the additional sets (maximum total of two additional) must be
submitted with the three required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
right of way and/or dedicated easements will require a Consent to Encroach Agreement
between the Owner/Developer and the City of Carmel. This Agreement requires Board of
Public Works and Safety approval.

It does not appear Performance and Maintenance Guarantees will be required.
Street Cut/Right of Way Permit and Bonding Requirements
Any work in dedicated right of way of 116th Street or Illinois Street (including curb cuts) will
require a Right of Way Permit and a License and Permit Bond. The bond amount will be
determined by the number of instances of work in the right of way (commercial curb cuts,
storm sewer work, etc.) ~ $2,000.00 per instance. Open pavement cuts itt dedicated streets
require approval bi: our Board o[Public Works and Sa£e~. Please contact Fred Glaser,
Department of Engineering Right of Way Inspector/Manager for details regarding this issue.

· Based on the design inverts, it is apparent that the detention area to the west of the
parking expansion is to serve as a overflow pond which will fill and drain based on the
~vater surface elevation of the larger pond to the west. Is this correct? Please provide
drainage calculations of this entire system if this is the case.
13. Sheet C4.1 Utility Plan
· There is a fire hydrant located in the proposed building expansion.
Please indicate any utilities that are to be installed as a part of this apprOval.
Please indicate any previously approved utilities that are to be modified as a part of this
approval.

Sheet C5.4- Add the following note to the two storm water manhole details:
- Install_ O-Ring gaskets between all riser and cone sections plus ½" extmdable
gasket (Kent Seal or equivalent)
- Install butyl rubber base exterior backplaster material ¼" + 1/16" minimum
thickness from 2" below top of cone to casting.

Are all of the erosion control details to be implemented as a part of this approval?
We reserve the right to provide additional comments upon further review by the City Engineer.
We have also engaged Crossroad Engineers PC to review all drainage plans and drainage
calculations. We will share these comments upon receipt.
The above comments are based upon the Engineering Department's initial review. Please provide
our office with a follow up letter regarding this review and revised plans, indicating all revisions.
A second review will focus only on detailed construction plans. It is critical that our office be
made aware of all modifications made on the plans being re-submitted, particularly if any such
changes are considered "new" or fall outside of our initial review. If you have questions, please
