The City has received your comment letter and revised drawings dated August 31, 2004 based upon the City review of
August 23, 2004.
The comments from the City revie\v have been satisfactorily addressed.
\Ve have enclosed. a copy of the September 1, 2004 drainage review from Crossroad Engineers, PC. Please verify top
of casting elevations, invert elevations, and pipe sizes as recommended by Crossroad Engineers.
If you make all the changes and agree \vith our. comments, please make the revisions and submit all copies necessary
for approval and we \vill approve. \Ve will need a minimum of four copies of the plans for approval. If the developer
needs additional copies, \ve will approve a maximum of t\VO additional copies for a total of six. If you have questions,

The City has received your comment letter and revised drawings dated July 2, 2004 based upon the City review of June

The comments from the City review have been satisfactorily addressed with the following exceptions:
PROJECT COMMENTS-CONSTRUCTION PLANS
12. Sheet C400 - Utility Plan
. Comment on previous revie\v \vas: "Is sanitary structure # 1 an existing structure? If so, label as such."
Please edit sanitary sewer profile to reference Str.No. 100 and Str. No.1 0 1.
. Comment on previous review \vas: "\Vhy are there t\vo 8" sanitary mains run on-site?" Please confirm with
the Utility Department that the long building laterals are acceptable.
. Comment on previous review \vas: "Is 8" too large for sanitary service laterals? Normally, these laterals or
service lines are 6"." Please confirm with the Utility Department that the sewage flows provide adequate
flushing velocity and that the 8-inch service lateral is acceptable.
. Slope between storm structures 9 and 7 is not 0.55% based on information on drawings. Please revise.
13. Sheet C602 - Details
. Comment on previous revie\v was: "There is a detail for a temporary construction entrance but the location
of this entrance is not indicated on the plans." Thank you for indicating the western entrance. Please show
the temporary construction entrance approved by the Board of Public Works and Safety on the Erosion
Control Plan if this is an additional .entrance.
We have engaged Crossroad Engineers PC to review all storm drainage plans and drainage calculations ~ubmitted to
this office for review. We will share these reviews \vith you as they are received.
Please provide plans on 24" x 36" sheets. We cannot fit larger size sheets in our files.

Dear Mr. Hoyt:
Enclosed please find one set of revised construction plans for the subject project.
The plans were revised per your comment letter faxed to us on June 15, 2004. Your
specific comments were addressed as foIIows:
1. An existing hydrant is located approximately 100 feet from the proposed
siamese or Storz connection on the north side of the building. Please confirm
that another hydrant will be necessary in this vicinity.
2. Please contact Larry Richardson at Duke Construction (808-6000) with
questions related to the fire alarm system.
3. The building heights win be provided by the architect. Please call Aaron
Haschel at 848-7800 if you need this information in the near future.
4. A Knox box and Knox fire department connection caps are planned for this
building. It was noted at the T AC meeting that the department is considering
switching to Storz connections. An exterior door to the fire sprinkler room
will be added to the building plans.

Enclosed please find one set of revised construction plans for the subject project.
The plans were revised per your comment letter dated June 8, 2004. We have
addressed the water related comments by adding notes about Posi-Caps, tracer wire
and warning tape. In addition, your specific sanitary sewer comments were
addressed as follows:
1. The stub was extended and a manhole set. Due to minimal cover, we are
proposing 8" laterals at 0.45% slope.
2. The clean out on the northern line was deleted from the plan. The clean out
from the southern line will remain in order to avoid the foundation of the
building.
3. A note was added to your standard detail regarding coating of the manholes.
4. The correct lids and castings are now shown on the detail sheets and the
utility plan.

11. Sheet C300 - Grading Plan: Additional information was added as requested.
12. Sheet C400 - Utility Plan: All comments were addressed. Additional survey
information was obtained which should clarify some of your concerns and
comments. Eight-inch sanitary laterals at 0.45% are proposed due to
minimal cover over the proposed pipe.
13. The temporary construction entrance was added to the erosion control plan.
PROJECT COMMENTS - SECONDARY PLAT
14. The legal description of the plat is the Secondary Plat for Replat of Common
Area Block A, Common Area Block D and Lot 2, North Haven Subdivision
.as recorded in Plat Cabinet 2, Slide 781, Instrument Number 2002-00028201
in the Office of the Recorder of Hamilton County, Indiana. This is shown on
the plat.
15. The reference to Mayflower Park was removed from the plat.
16. Proposed monumentation is now shown on the plat.
17. The correct reference to the City is now shown on the plat.

This letter is in response to your Secondary Plat and Development Plan Amendment Applications for

members at the TAC Meeting on June 16. Please complete the following items:
Secondary Plat
1. Change the signature block/paragraph text from the Co. Commissioners to the Board of
Public Works (see Example).

3. Change references from Hamilton County to the City of Carmel.

4. Sanitary Sewer A. vailability and Connection Fees:
Sanitary Sewer Availability Fees - This fee will not be applicable for this project. Availability Fees, in the
amount $32,828.00 for 24.05 net acres (18.89 acres exempted/100 year floodway), were paid on November 2,
2001.
Sanitary Sewer Connection Fees - The Sanitary Sewer Connection Fee is currently $795.00 per EDU. Based
upon the information provided on Sheet C200 of the drawings, the following estimated Sewer Connection Fees
might be applicable, assuming these will be office buildings:

21,114 ft2 @ 0.5 EDU / 1000 ft2 = 10.56 EDU @ $795.00/ EDU = $ 8,395.00

certify all drawings submitted for final approval. This office will require a minimum of four-sets (maximum of
six-sets) of approved drawings after all issues have been resolved. The drawings will be stamped approved and
signed by both the City Engineer and Director Carmel Utilities. The Developer/Owner will receive one-set which
is to be maintained on the construction site at all times. Carmel Utilities will receive one-set. Our Public Works
Inspector will receive one-set and one-set will be maintained in the Department of Engineering. If more than the
minimum of four-sets are requested to be approved, the additional sets (maximum total of two additional) must be
submitted with the four required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated easements and/or
right of way will require Consent to Encroach approval.
7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all sanitary sewer main
locations.
8. Carmel Utilities will provide a separate review of this project for sanitary sewer issues. Please assure that Carmel
Utilities receives copies of all drawings and revisions for their review and approval.

BONDING REQUIREMENTS
10. The following bonding requirements may apply to this project:
Performance Guarantees/Engineer's Estimates
It appears that there will be requirements for Performance Guarantees for sanitary sewers, particularly if sanitary
structure number 1 is a new structure. Please provide a Certified Engineer's Cost Estimate for 100% of the cost of
labor and materials for the construction of the following improvements:
. Sanitary sewers including structures, service laterals, etc.
. Monuments if applicable.
Individual Performance Guarantees will be required in the amount of the Engineer's Estimate.
Street Cut/Right of Way Permit and Bonding Requirements
Any work in the dedicated right of way of Gray Road, not included in construction covered by a Performance
Guarantee, will require a Right of Way Permit and a License and Permit Bond. The bond amount will be
determined by the number of instances of work in the right of way @ $2,000.00 per instance. Open pavement cuts
in dedicated streets require approval by our Board of Public Works and Safety. Please contact our Right of Way
Manager, Fred Glaser, to arrange permitting and bonding.
PROJECT COMMENTS-CONSTRUCTION PLANS
11. Sheet C300 - Grading Plan
Please provide additional contourslelevations on the plans, particularly at top of curb and gutter in the parking
areas and show how drainage will work in the green spaces around the parking areaslbuildings, especially on the
east and south sides of the development. Also, indicate surface drainage flow with flow arrows. Please indicate
existing structures on the plan.
12. Sheet C400 - Utility Plan
. Is sanitary structure # 1 and existing structure? If so, label as such.
. It appears that reverse slopes are indicated on westside of the parking area associated with Lot #2. The
inverts for storm structure 9 (737.05) and storm structure 8 (737.09) are lower than the invert for the
receiving storm structure 7 (738.74). The invert for structure 7 is hard to read. Should the 8 be a 6 making
the invert 736.74?
. Structure 7 is indicated as discharging to an end section/existing storm drain. This existing storm drain is
identified as and 18" RCP on Sheet C20D. Please use the same identification on this sheet.
. What is the invert on the existing end section? When the end section is removed and the existing storm pipe
is connected to structure 7, what will the cover be over the pipe?
. Please indicate the TC/Invert for the existing storm structure west of storm structure 7.
. Is the storm structure northwest of proposed structure 2 an existing or new structure? Please indicate on the
plans. In either case, show the TC/Invert for this structure.
. Please indicate the TC/Invert for the existing storm structure west of the unidentified structure referenced

. There is a detail for a temporary construction entrance but the location of this entrance is not indicated on the

14. Shouldn't the plat contain a legal description?
15. The Certificate of Survey references Mayflower Park.
16. Difficult to determine where/type of monuments that are to be installed with this development.
17. The City of Carmel Board of Public Works and Safety, not the Board of Commissioners of Hamilton County, will
approve this plat. Please make the appropriate revisions.
We have engaged Crossro.ad Engineers PC to review all storm drainage plans and drainage calculations submitted to
this office for review. We will share these reviews with you as they are received.
The above comments are based upon the Engineering Department's initial review. Please provide our office with a
follow up letter regarding this review and revised plans, indicating an revisions. A second
review will focus only on detailed construction plans.- It is critical that our office be made aware of all modifications
made on the plans being re-submitted, particularly if any such changes are considered "new"

A18- The soils information does not provide information about the soil conditions and how to overcome any limitations.
In this case the soil is fill from the prior work. Soil testing or construction inspections would verify that the soils are
compacted for building foundations and the proposed drainage plan will address drainage issues. I would not recommend
a basement unless it were above the static water table.
B6 & 10- A practice detail is needed for paved area inlets. I would recommend inlet protection that fits into the catch
basin for those inlets in the driving lanes. Curb inlets can be protected with the external devices.
B 12- Specifications needed for permanent seeding and soil preparation.
B 13- The plan does not address- concrete washout, cleaning agents, paints or solvents, solid wastes or equipment repair.
An action plan is needed if a spill does occur.
C3- The reasoning or justification is very weak. If I didn't know the site, I wouldn't know that a large pond with abundant
storage exists on the site. The plan needs to document why the proposed practices are appropriate for the identified
stormwater pollutants.
