IT IS REQUESTED THAT A CONTRACT CHANGE BE MADE TO THE ABOVE REFERENCED CONTRACT.
1. SCOPE OF WORK
Mill existing asphalt and incorporate millings into the embankment.
2. REASON FOR CHANGE:
Unexpected thicknesses of the existing asphalt required the contractor to perform extra
work not covered in the original pay quantities.

We hereby propose to make the following changes:
Overages on Common-Millings
Per the bid documents,a total of 2807 CYS of common/millings were to be milled and spread for use on the project. The
depth of the asphalt that required milling was significantly deeper on all three phases of this project,thus resulting in an
overrun of 2,674.48 CYS of asphalt that needed removed. Typical asphalt is 8"-12"thick;however, in certain places on
this project the asphalt was as deep as 22",which was not shown on the plans as so.
This was a unexpected direct expense to Poindexter Excavating due to incorrect quantities on the bid form.
If the quantities were close,we could see this being a minor miscalculation;however,the actual milled quantity was
substantially larger than the bid quantity.
Poindexter is not looking to make money on the additional milling that was needed,we are only looking to cover the
additional costs that we incurred due to the incorrect bid quantity.
We know our price for common is$55.00/CY,which includes ALL costs associated with this line item,but we are only
seeking$9.00/CY to cover our subcontractor cost and the fuel cost for the additional grading of the extra millings.
This would result in a Change Order Request for an additional$24,070.32.
Please consider this a good faith effort from Poindexter to the City of Carmel,to remediate an engineering oversight.

Original Contract Amount $1,500,948.98
This price is good for 60 days. If conditions change,this price is void.
We are requesting a time extension of 0 days in conjunction with this change.
