Surveyors Report
I
In accordance with Title 865, Article 1, Rule 12 of the Indiana Administrative Code, the following observations and opinions are
submitted regarding the various uncertainties in the locations of the lines and corners established this survey as a result of uncertainties
in reference monumentation; in record descriptions and plats; in lines of occupation; and as introduced by. random errors in
measurement C'Theoretical Uncertainty"). There may be unwritten rights associated with these uncertainties. The client should assume
there is an amount of uncertainty along any line equal in magnitude to the discrepancy in the location of the lines of possession from the
surveyed lines.

There may be differences of deed dimensions versus measured dimensions along the boundary lines shown hereon and, likewise,
there may be found survey markers near, but not precisely at, some boundary corners. In cases where the magnitude of these
, differences are less than the Theoretical Uncertainty stated below, less than the Positional Tolerance, and less than the uncertainty
identified tor the reference monumentation (discussed below), the differences may be considered insignificant and are shown only for
purposes of mathematical closure. Such differences that are greater than the Theoretical Uncertainty, Positional Tolerance and the
uncertainty in reference monumentation should be considered worthy of notice and are therefore discussed further below.

Unless otherwise noted or shown on the within survey plat, there is no evidence of occupation along the perimeter lines of the subject
tract. All survey monuments set or found on this survey are flush with existing grade unless otherwise noted.

The accuracy of any flood hazard data shown on this report is subject to map scale uncertainty and to any other uncertainty in locaiion
or elevation on the referenced Flood Insurance Rate Map. The within described tract of land does not lie within Flood Hazard Zone A as

Fifth Third Bank
Background
The proposed site encompasses approximately 2.01 acres; Fifth Third Bank will disturb
approximately 1.36 acres. The site is located at the intersection of Hazel Dell Parkway and
East 146th Street, Carmel, Indiana. The site currently consists of a mostly pervious surface.
The site is Parcel 3 of the Riverview Medical Park. The proposed site shall drain into the
storm sewer provided by the development along the private drive that runs along the south
property line.
Storm Sewer Design
Ten-year storm intensities were used in the design of the storm sewer system. The Rational
Method was used to determine the flow into each inlet. Manning's equation was used to
size all storm sewer conduits. Intensities were obtained from the National Weather
Services' Hydrological Design Studies Center (http://hdsc.nvvs.noaa.gov/hdsclpfds/).An
initial tirne of concentration of ten minutes was used. The intensities were interpolated for
time of concentrations that fell between given ti mes in the table. I nformation for the
existing storm sewer along the private drive along the southern border of the property was
provided by an AL T A survey and information provided by the Schneider Corporation, the
design engineer for the overall development.

CUITent Water Sanitary Sewer Connection Fees
Our Utility Ordinance lists an average user factor of 0.5 EDUs/I000 ft2 for an office classification,
which would be applicable to bank facilities. On the plans submitted, the building square footage is
listed as 4,134 ft.2 Prototype with a 2,079 ft.2 canopy. Does this indicate the total square footage for
just the building without the canopy as 2.055 ft.2? If this is the COITect building square footage, Water
and Sanitary Sewer Connection Fees would be calculated as:
Water - 2,055 f1.2 @ 0.5 EDUs/IOOO ft.2 = 1.028 EDUs @ $1,310.00/EDU = $1,347.00
Sanitary Sewers - 2~055 ft.2 (Q) 0.5 EDUs/IOOO f1.2 = 1.028 EDUs @ $ 795.00/EDU = $ 817.00

Any work in dedicated right of way of Hazel Dell Parkway will require a Right of Way Permit and a
License and Permit Bond. The bond amount will be determined by the number of instances of work in
the right of way (commercial curb cuts, storm sewer work, etc.) @ $2,000.00 per instance. OIJen
vaventent cuts in dedicated streets require aooroval bv our Board of Public Works and Safetv. It
appears that there is at least one instance applicable to this proj ect, namely from storm structure A 1 to
the existing structure in the right of way. Please contact Fred Glaser, Department of Engineering Right
of Vl ay Inspector/Manager for details regarding this issue.
