Ramona Hancock, no later than 12:00 PM, Friday, May 25th, 2012. If filing fee and materials are not delivered by
this time, this application will be continued to the Wednesday, June 27th, 2012 meeting

a) Pages 1 -2 of your ADLS Amendment application
b) Site plan showing where the sign will go on the property
c) Illustration showing what the sign will look like on the building
d) Color drawing of the sign with dimensions, materials, height from ground to bottom of sign and lighting

6 This Item will appear on the Tuesday, June 5, 2012 agenda of the Plan Commission Special Studies Committee
at 6:00 pm in the Caucus Room, Second Floor, City Hall Please have a representative attend to present your
request.
7. After approval, a sign permit will also be required before the sign can be installed. Here is a link to the sign
permit application. Please have this ready to be turned in after your Plan Commission Committee approval is
granted.
