1. NOTICE IS HEREBY GIVEN THAT SEALED PROPOSALS FOR THE PARTIAL RE -ROOF & MISC. GENERAL
REPAIRS OF ST. MARK'S UNITED METHODIST CHURCH, 4780 E. 126 TH STREET, CARMEL, INDIANA WILL BE
RECEIVED UNTTL 2:00 PM ON THURSDAY, MAY 29TH, 1997. THE BIDS WILL BE OPENED PRIVATELY. ALL
BIDS RECEIVED AFTER 2:00 SHALL BE RETURNED UNOPENED.
2. THE OWNER RESERVES THE RIGHT TO ACCEPT OR REJECT ANY BID OR WAIVE ANY INFORMALITY OR
ERRORS IN BIDDING FOR 60 DAYS AFTER THE BID OPENING DATE.
3. CONTRACTOR'S SUBMITTING PROPOSALS SHALL BE PREQUALIFIED. A LIST OF PREQUALIFIED BIDDERS
WILL BE SUPPLIED BY THE ARCHITECT UPON REQUEST.
4. THIS PROJECT IS TAX EXEMPT. THE SUCCESSFUL BIDDER SHALL BE SUPPLIED WITH FORM SP134 FOR
USE IN PROCURING MATERIALS.
5. BIDDERS SHALL SUBMIT 2 COPIES OF THE CONTRACTOR'S STANDARD PROPOSAL FORMS IN AN
OPAQUE SEALED ENVELOPE.
6. A BID BOND IS NOT REQUIRED.
7. A 100% PREFORMANCE BOND FOR THE CONTRACTED AMOUNT IS REQUIRED PRIOR TO COMMENCING
WORK.
8. A SITE VISIT IS REQUIRED PROIR TO PROPOSAL SUBMISSION. PLEASE ARRANGE WITH CHURCH STAFF
AT 846 -4912 TO MAKE THIS VISIT. REGISTER YOUR VISIT WITH THE CHURCH STAFF.
9. INSTRUCTIONS TO BIDDERS SHALL BE AIA FORM A7011 1987 EDITION.
A. 3.3.2 DELETE IN ITS ENTIRITY, AND REPLACE WITH THE FOLLOWING: THE PLANS AND
SPECIFICATIONS ARE INTENDED TO INDICATE A STANDARD OF REQUIRED PREFORMANCE AND DESIGN
INTENT. IT IS THE BIDDERS DISCRETION AS TO THE MATERIALS USED IN COMPILING HIS BID AS LONG AS
THOSE MATERIALS MEET THE CRITERIA SPECIFIED BY THE BID DOCUMENTS. ANY PROPOSED MATERIAL
WHICH DEVIATES FROM THE LISTED STANDARDS SHOULD BE NOTED ON THE BID FORM AS A VOLUNTARY
ALTERNATE AND / OR ALTERNATIVE SYSTEM TO BE CONSIDERED. THE SUBMISSION OF VOLUNTARY
ALTERNATES AND / OR ALTERNATIVE SYSTEMS IS ENCOURAGED.
10. THE GENERAL CONDITIONS OF THE CONTRACT SHALL BE AIA FORM A201.

EMPLOYEE OF RQAW CORPORATION, RQAW CORPORATION IS NOT THE ARCHITECT OR ENGINEER AND
ASSUMES NO PROFESSIONAL OR BUSINESS LIABILITY WHETHER DIRECT OR IMPLIED.
12. THE CONTRACTOR SHALL NOT COMMENCE WORK UNTIL A CURRENT CERTIFICATE OF PROOF OF
INSURANCE IN THE NAME OF ST. MARK'S UNITED METHODIST CHURCH IS ON FILE WITH THE OWNER.

1. THE CONTRACTOR SHALL PROVIDE APPROPRIATE STORAGE, LOCKERS, OR SECURITY FOR THE SECURE
STORAGE OF TOOLS AND MATERIALS.
2. THE CONTRACTOR SHALL PROVIDE APPROPRIATE TEMPORARY TOILET FACILITIES.
3. THE CONTRACTOR SHALL PROVIDE APPROPRIATE REFUSE CONTAINERS FOR THE LEGAL DISPOSAL OF
REFUSE, CONSTRUCTION DEBRIS, AND / OR DEMOLISHED MATERIALS.
4. PROTECT ADJACENT AREAS AND MATERIALS FROM DAMAGE AND / OR WEATHER.
