I have included the text of your email below for reference as a checklist. ✓ 1. Please provide digital copies of any revised plans. ✓ 2. Please provide copies of your correspondence with the TAC members and their correspondence with you. Acknowledged 3. Remember to include the Findings of Fact sheet in your informational packets to the Plan Commission. This form can be found within the Primary Plat application. ✓ 4. Provide the filled out Notice of Public Hearing page of the application. 5. Provide the filled out and notarized Public Notice Sign Placement affidavit page of the application. 6. Provide the filled out and notarized Affidavit of Notice of Public Hearing page of the application. 7. Provide a copy of the Official List of Adjacent Property Owners from Hamilton County Auditor's Office. 8. Please add docket nos. 12100004 SP & 12100003 PP to the plat. 9. Prepare an estimated construction cost to comply with the Thoroughfare Plan & Alternative Transportation Plan; contact the Engineering Dept. for more details, at 571 -2441. This is in progress per my email to you of November 1, 2012; am awaiting a response from Gary Duncan. 12. Daren Mindham (City Forestry Dept.) will review the Landscape Plan. Please submit a plan that shows the bufferyard areas that comply with Zoning Ordinance Chapter 26.04. Addressed pursuant to our email communication of October 23 — 25, 2012. 13. Please label Six Points Rd. on the plat. 14. Please add the floodplain statement to the plat. 15. Please label the lengths of the southern and eastern property lines of Lot 1, in addition to what you show for the lengths of the entire side. 16. Please add the member names and signature blocks to the Board of Public Works and Safety signature block on sheet 2. Please see SCO Chapter 9.03 as a guide. 17. Please show /label 15 -ft or 20 -ft wide easements along the property lines, per SCO Chapter 6.06.01 18. Please add the Deed of Dedication paragraph to sheet two, per SCO Chapter 9. 19. Please submit a filled out Subdivider Agreement Form, per SCO Chapter 9. 20. Please show /add concrete monuments at all 4 exterior corners of the subject parcels. 21. Please provide a utility map showing where all the utility mains are located nearby for water, sewer, etc. ✓ 22. Please provide a Soils Map. Secondary Plat: ✓ 23. Regarding the Secondary Plat, please work with Dave Barnes in the City Engineering Dept. to get on a BPW (Board of Public Works ) meeting agenda, which has a filing deadline. His number is 571 -2441, and his email address is dbarnes @carmel.in.gov. Acknowledged 24. Staff has been advised that we cannot present a Secondary Plat to the Director for signature from this point forward until Ms. Terry Krueskamp (trueskamp @carmel.in.gov) in Information Systems has received an e-mail with an electronic file attached (MicroStation .dgn is preferred, or AutoCad .dwg of the subdivision) and it (the e-mail) has been copied to myself (aconn @carmel.in.gov). ✓ 25. Please submit a revised Secondary Plat after making above - mentioned changes. Two paper copies of the plat must be filed with the Mylar prior to recording, and two paper copies and an electronic copy of the scanned, recorded plat must be submitted following recording of the document. Our approval of the secondary plat (signature) will be forthcoming upon revision of the plans according to the items listed above. Acknowledged 26. Lastly, please submit a .pdf or .jpg file of the scanned, recorded plat. In addition to the above comments, we have also made revisions as requested by Panhandle Eastern, Clay Township Regional Waste District, and Crossroad Engineers. Those specific comments can be found in the hard copies of email communications included in the package that will be delivered to your office today. Thank you for your assistance in keeping this moving through the process. Please let me know if there are any questions about any of the above items, or of any remaining issues that need to be addressed so that they may be included in the packets for Plan Commission later this week.
