Appropriation 0211-401; P.O.1126554 Contract Not To Exceed$17,500.00 ADDITIONAL SERVICES AMENDMENT TO rOpt u A AGREEMENT FOR PURCHASE OF GOODS AND SERVICES tat THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and between the City of Carmel and A & F Engineering (the"Vendor"), as City Contract number 040710.08, dated April 7, 2010 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A". Furthermore, the terms of the Agreement shall be amended to include the E-verify requirement as stated in Indiana Code§ 22-5-1.7 et seq., which is attached hereto and incorporated herein as Exhibit "B", as well as the required E-verify Affidavit, attached hereto and incorporated herein as Exhibit"C". The terms and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and effect. VICE PRESIDENT September 26, 2012 Mr. Michael McBride City Engineer City of Carmel One Civic Square Cannel, IN 46032 Re: Supplemental Request for Roundabout Design 1 16°i Street & Gray Road CMAQ Funding Dear Mr. McBride: As discussed, there have been two significant requested revisions to our current design due to unforeseen circumstances within the past few weeks. First,the City has requested that A&F Engineering engage a landscape architect to perform landscaping and irrigation designs into the final package. A&F Engineering has received a proposal from Rundell Ernstberger Associates LLC, which has been attached to this letter. These fees include the design elements for both the roundabout at 1 I6th & Gray and 11 I'I'& Pennsylvania. By incorporating these items into the federally funded contract, the City will be able to take advantage of the 80/20 federal/local funds for the landscaping items. Secondly, due to the current reconstruction of Duke Energy power lines at the intersection. A&F Engineering will need to shift the center of the roundabout and rework all the Stage I design element, associated with this relocation to facilitate these new obstacles. Some of the new Duke facilities are located within easements and will be very expensive for the City to relocate, if the roundabout plans are not revised. Ike fees to pit-vide these Services are as follows: I. Landscape Design Services: Lump Sum $17,500.00 2. This proposal is based on review of the A& F Engineering Co., LLC construction drawings, as well as conversations we • have had with you. PROJECT UNDERSTANDING: O We have the following understanding of the scope of work for this project: 1. A & F Engineering wishes to engage the services of REA to prepare landscape design • development and construction documents for the two RAB's located at 1160" Street and Pennsylvania Street and 111th Street and Gray Road in Carmel, Indiana. Our work will include schematic design and construction drawings, and includes landscape and • irrigation treatments for the roundabouts and associated splitter islands. The project scope does not include hardscape elements within the medians, such as walls, urns, sculpture, etc. V 2. Our work will include the services of Landtech Irrigation Consultants for the design of the • roundabout irrigation system. Electrical engineering services, if required, will be provided by A&F Engineering. 3. Plant selections will conform as much as possible to the City of Carmel Urban Forester's preferred plant species. Low maintenance plantings are preferred. 4. Construction Documents will consist of roundabout planting plans, irrigation plans and details, planting details and schedules, specifications, pay item list and an engineer's estimate. We understand the work will be prepared in accordance with INDOT standards and guidelines. 5. REA will utilize base plan drawings to be provided by A&F Engineering in the preparation of • our drawings. REA will coordinate the timing of our work with the construction document and bidding schedule to be provided by A&F Engineering. It is anticipated that final construction drawings for the 111th and Pennsylvania Street RAB will be issued in October, 2012. Final construction drawings for 116th and Gray Road are anticipated for spring, 2013. SCOPE OF WORK: Based on the above understanding, we propose the following scope of work: 1. Schematic Design: REA will develop preliminary plans showing the extent of landscape enhancements. These will include, but are not necessarily limited to, plantings, lighting, and topographic elements. A preliminary cost opinion will be prepared to ensure design ideas are in line with the project budget. Preliminary plans, details, and cost opinions will be coordinated and reviewed with the City for final approval. For the purposes of this project, we have estimated a total of four(4) review meetings with the City of Carmel. 2. Final Plans: Upon approval of preliminary plans, REA will prepare the final plans, details, pay items, quantities,estimate, and special provisions required for final plan submittal for each RAB. 3. Bidding: REA will provide services during the bidding process to issue clarifications and answer questions pertaining to REA's work. REA will also attend a prebid conference for the project, if required. All responses to bidder's inquiries will be coordinated with the City and will be distributed to all bidders. 4. Construction Observation: Construction administration services are not a part of this scope of work. COMPENSATION: Design, Construction Documentation&Bidding: We propose a lump sum fee of$17,500 for this work. • Services will be invoiced on a monthly, percentage complete basis. • The above fee includes an allowance of$500.00 for project related expenses(printing, mileage, postage, etc.).These expenses will be invoiced against this amount according to the attached schedule. • Costs for special renderings or presentation drawings are not included in the fee. If requested by the client,these items will be billed at our direct cost. • Additional services and expenses beyond those indicated herein will be billed per our standard rates in effect at the time services are rendered or on a negotiated fee basis.
