Riverview Hospital, by Jae Ebert, Vice President of Support Services, the
undersigned, the petitioner ("Petitioner"), authorizes the law firm of Nelson &
Frankenberger, P.C. and its attorneys, agents, and representatives, to represent the
Petitioner in connection with all proceedings (the 'Proceedings"), before the City of
Carmel Plan Commission and its Committees, to obtain an amended ADLS approval in
regard to Buildings A and B which are located on Parcel 1 of the Riverview Medical Park
Complex, commonly known as 5925 146th Street East, Carmel, Indiana 46033 (Tax Map

Changes in Riverview Hazel Dell Project Design
Building 'A' Primary Care Building (North Building):
. Reducing drive-throughs from a double drive-through to a single drive-
through.
. Lowering the entry way roof peak to the same height as the rest of the
building and keeping the chimney height in proportion to the roof line.
. Removing one (alse gable ~n South elevation.
Building 'B'Reha,b Building (South Building):
. Moved second floor occupied space to first floor.
· Expanded square footage to just slightly less than 20,000 square feet.
. Squared building footprint to facilitate structural truss system.
· From original design, added drive up canopy and trom second approved
design, changed canopy gable.
· Removed windows trom second story attic space.
. Changed trom brick gable to efface gable (East elevation).
· Changed gable and glass block configuration on West elevation.
